This study examined the contribution of long-term use of Lipiodol capsules, as a supplement to iodised salt to the control of iodine deficiency disorders among women in Xinjiang of China. A total of 1220 women across Kashgar, Aksu, Turpan and Yili Prefectures were surveyed in 2017. Lipiodol capsules were administered twice yearly in Kashgar and once yearly in Aksu and Turpan, but not in Yili. Urinary iodine concentration (UIC), free triiodothyronine (FT3), free thyroxine (FT4), thyroid-stimulating hormone (TSH), thyroglobulin antibody, thyroid peroxidase antibody and thyroid volume values were assessed. All the women in the four areas were in a state of non-iodine deficiency by UIC. The UIC were higher than adequate in Kashgar and Aksu (619·4 v. 278·6 μg/l). Thyroid hormone levels differed significantly in Turpan and Yili (FT3: 4·4 v. 4·6 pmol/l, FT4: 13·8 v. 14·2 pmol/l, TSH: 2·0 v. 2·7 mIU/l), but did not differ significantly in Kashgar, Aksu and Yili. The four areas did not differ significantly with regard to thyroid nodules, autoimmune thyroiditis or goitre. However, the detection rates of subclinical hypothyroidism (16·6 %) and total thyroid dysfunction (25·4 %) were higher among women in Yili. The supplementation with Lipiodol capsules had improved the iodine nutrition status of women in iodine-deficient areas of Xinjiang since 2006. To avoid negative effects of excess iodine, we suggest a gradual discontinuation of Lipiodol capsules in women with special needs based on the existing iodine nutrition level of local women.

Perchlorates, nitrates, and thiocyanates constitute environmental endocrine disruptors; however, health damage caused by absorption through the respiratory tract remains poorly studied. We investigated the effects of inhalation of these pollutants on thyroid function and structure and serum metabolomics in pregnant rats. We established a Sprague-Dawley pregnant rat model exposed to perchlorate, nitrate, and thiocyanate at different gestational stages and compared maternal serum thyroid function levels, foetal development, thyroid morphology, and pathological changes between exposed and non-exposed groups at different concentrations. We used liquid chromatography-tandem mass spectrometry metabolomics methods to investigate corresponding changes in serum metabolites and metabolic biomarkers and identify metabolism-related pathways. Free triiodothyronine levels in the medium-and high-concentration groups and free thyroxine levels in the low-, medium-, and high-concentration groups were significantly lower than those in the control group. Weight gain was significantly lower in the high-dose group than in the control group. Thyroid weight and organ coefficients were significantly higher in the medium-and high-dose groups at 15 and 19 days of pregnancy than in the control group. Thyroid epithelial cells demonstrated diffuse proliferation and lamination with increased atomisation concentrations, and thyroid cells tended to undergo apoptosis in the group with high thyroid epithelial cell concentration. Eight and 18 metabolic pathways were significantly affected in the GD15 and GD19 groups(GD15: The demise occurring 15 days into gestation,GD19: The demise occurring 15 days into gestation.), respectively, post-exposure. The involved pathways included lipid, glucose, bile acid, choline, and gamma-aminobutyric acid metabolism and peroxisome proliferation-activated receptors. Mixed thiocyanate, nitrate, and perchlorate exposure can result in subchronic toxicity in rats, affecting thyroid function and structure.

The spread of visceral leishmaniasis (VL), a serious global zoonotic parasitic disease, is mostly under control; however, several cases have been reported in recent decades in Xinjiang, China. This study aimed to analyze the epidemiological status and spatiotemporal clustering characteristics of VL in Xinjiang, China, between 2004 and 2021 to provide a basis for the development and implementation of surveillance and response measures. Data on VL incidence during 2004–2021 were collected from the National Diseases Reporting Information System of China. Global spatial autocorrelation analysis, identification of local indicators of spatial association, and spatial–temporal clustering analysis were conducted to identify the distribution and high-risk areas. A total of 2034 VL cases were reported, with a mean annual incidence of 0.50 per 100,000. There was a general decreasing trend in the incidence of VL during our study period. The majority of the cases were reported from October to February of the following year, and fewer cases were reported from April to July. Spatial autocorrelation analysis revealed that the incidence of VL was spatially clustered within a few counties. Significant differences were observed during the study period (Moran’s I = 0.74, Z = 4.900, p < 0.05). The male-to-female ratio was 1.37:1, and most patients were in the age group 0–3 years. Cases were primarily distributed in seven regions and two autonomous prefectures, and Kashgar reported the highest number of cases (1688, 82.98%). Spatial analysis revealed that the aggregation of VL was predominantly observed in southwest Xinjiang. This was in alignment with the high-risk areas identified by spatiotemporal clustering analysis. The H-H clustering region was primarily observed in Gashi, Atushi, Shufu, Injisha, Kashgar, Yepuhu, and Bachu. These findings indicate that integrated control measures must be taken in different endemic areas to strengthen the VL control program in Xinjiang, China.
