With the development of electrical stimulation technology, traditional transcranial alternating current stimulation (tACS) technology has been found to have the drawback of not targeting a specific area accurately. Studies have shown that optimizing the number and position of electrodes during electrical stimulation has a very good effect on enhancing brain stimulation accuracy. At present, an increasing number of laboratories have begun to optimize tACS. However, there has been no study summarizing the optimization methods of tACS. Determining whether different optimization methods are effective and the optimization approach could provide information that could guide future tACS research. We describe the results of recent research on tACS optimization and integrate the optimization approaches of tACS in recent research. Optimization approaches can be classified into two groups: high-definition electrical stimulation and interference modulation electrical stimulation. The optimization methods can be divided into five categories: high-definition tACS, phase-shifted tACS, amplitude-modulated tACS, the temporally interfering (TI) method, and the intersectional short pulse (ISP) method. Finally, we summarize the latest research on hardware useful for tACS improvement and outline future directions.

Exoskeleton robot is an essential tool in active rehabilitation training for patients with lower limb motor dysfunctions. Accurate and real-time recognition in human motion intention is a great challenge in exoskeleton robot, which can be implemented by continues estimation of human joint angles. In this study, we innovatively proposed a novel feature-based convolutional neural network-bi-directional long-short term memory network (CNN-BiLSTM) model to predict the knee joint angles more accurately and in real time. We validated our method on a public dataset, including surface electromyography(sEMG) and inertial measurement unit (IMU) data of 10 healthy subjects during normal walking. Initially, features extraction from each modal data achieved feature-level fusion. Then the importance of each sEMG and IMU signal feature for knee joint angle prediction was quantified by ensemble feature scorer (EFS) and the number of features required for prediction while ensuring accuracy was simplified by profile likelihood maximization (PLM) algorithm. Finally, the CNN-BiLSTM model was created by using the determined simplest features to further fuse the spatio-temporal correlation of signals. The results indicated that the EFS and PLM algorithm could remove the feature redundancy perfectly and estimation performance would become better when bi-modal gait data were fused. For the estimation performance, the average root mean square error (RMSE), adjusted $\text{R}^{{2}}$ and pearson correlation coefficient (CC) of our algorithm were 4.07, 0.95, and 0.98, respectively, which was better than CNN, BiLSTM and other three traditional machine learning methods. In addition, the model test time was $62.47~\pm ~0.29$ ms, which was less than the predicted horizon of 100 ms. The real-time performance and accuracy are satisfactory. Compared with previous works, our method has great advantages in feature selection and model design, which further improves the prediction accuracy. These promising results demonstrate that the proposed method has considerable potential to be applied to exoskeleton robot control.

Wearable exoskeleton robots can promote the rehabilitation of patients with physical dysfunction. And improving human-computer interaction performance is a significant challenge for exoskeleton robots. The traditional feature extraction process based on surface Electromyography(sEMG) is complex and requires manual intervention, making real-time performance difficult to guarantee. In this study, we propose an end-to-end method to predict human knee joint angles based on sEMG signals using a tightly coupled convolutional transformer (TCCT) model. We first collected sEMG signals from 5 healthy subjects. Then, the envelope was extracted from the noise-removed sEMG signal and used as the input to the model. Finally, we developed the TCCT model to predict the knee joint angle after 100 ms. For the prediction performance, we used the Root Mean Square Error(RMSE), Pearson Correlation Coefficient(CC), and Adjustment R 2 as metrics to evaluate the error between the actual knee angle and the predicted knee angle. The results show that the model can predict the human knee angle quickly and accurately. The mean RMSE, Adjustment R 2 , and (CC) values of the model are 3.79°, 0.96, and 0.98, respectively, which are better than traditional deep learning models such as Informer (4.14, 0.95, 0.98), CNN (5.56, 0.89, 0.96) and CNN-BiLSTM (3.97, 0.95, 0.98). In addition, the prediction time of our proposed model is only 11.67±0.67 ms, which is less than 100 ms. Therefore, the real-time and accuracy of the model can meet the continuous prediction of human knee joint angle in practice.

Background: Xbox Kinect-based virtual reality, being a novel approach, has therapeutic benefits in rehabilitation and its use is encouraged in stroke rehabilitation of upper extremities. Objective: Primary aim of the current study is to investigate the additional effects of Xbox Kinect training in combination with routine physiotherapy exercises based on each component of Fugl-Meyer Assessment Scale for Upper Extremity (FMA-UE). Moreover, effect of upper limb rehabilitation on cognitive functions was also assessed. Methods: This study was a parallel arm randomized control trial. Fifty-six participants were recruited and randomly allocated to either an Xbox Kinect training group (XKGT) or exercise training group (ETG). Measures of concern were recorded using FMA-UE, Box and Block Test (BBT), and Montreal Cognitive Assessment (MOCA). Evaluation was conducted at baseline and after completion of intervention at the sixth week. Results: There were significant differences from pre- to post-intervention scores of FMA-UE and BBT (p < 0.001) in both groups, whereas no difference was observed for MOCA (XKTG p value 0.417, ETG p value 0.113). At six-week follow-up there were significant differences between both groups in FMA-UE total score (p < 0.001), volitional movement within synergies (p < 0.001), wrist (p = 0.021), hand (p = 0.047), grasp (p = 0.006) and coordination/speed (p = 0.004), favoring the Xbox Kinect training group. Conclusion: To conclude, results indicate repetitive use of the hemiparetic upper extremity by Xbox Kinect-based upper limb rehabilitation training in addition to conventional therapy has a promising potential to enhance upper limb motor function for stroke patients.

Transcranial direct current stimulation (tDCS) can improve cognitive function. However, it is not clear how high-definition tDCS (HD-tDCS) regulates the cognitive function and its neural mechanism, especially in individuals with mild cognitive impairment (MCI). This study aimed to examine whether HD-tDCS can modulate cognitive function in individuals with MCI and to determine whether the potential variety is related to spontaneous brain activity changes recorded by resting-state functional magnetic resonance imaging (rs-fMRI). Forty-three individuals with MCI were randomly assigned to receive either 10 HD-tDCS sessions or 10 sham sessions to the left dorsolateral prefrontal cortex (L-DLPFC). The fractional amplitude of low-frequency fluctuation (fALFF) and the regional homogeneity (ReHo) was computed using rs-fMRI data from all participants. The results showed that the fALFF and ReHo values changed in multiple areas following HD-tDCS. Brain regions with significant decreases in fALFF values include the Insula R, Precuneus R, Thalamus L, and Parietal Sup R, while the Temporal Inf R, Fusiform L, Occipital Sup L, Calcarine R, and Angular R showed significantly increased in their fALFF values. The brain regions with significant increases in ReHo values include the Temporal Inf R, Putamen L, Frontal Mid L, Precentral R, Frontal Sup Medial L, Frontal Sup R, and Precentral L. We found that HD-tDCS can alter the intensity and synchrony of brain activity, and our results indicate that fALFF and ReHo analysis are sensitive indicators for the detection of HD-tDCS during spontaneous brain activity. Interestingly, HD-tDCS increases the ReHo values of multiple brain regions, which may be related to the underlying mechanism of its clinical effects, these may also be related to a potential compensation mechanism involving the mobilization of more regions to complete a function following a functional decline.
