To simultaneously investigate the psychometric properties of three recently developed health literacy measurement scales throughout adolescence in the general population. French versions of the Health Literacy for School-Aged Children (HLSAC, unidimensional) scale, the Health Literacy Assessment Scale for Adolescents (HAS-A, multidimensional) and the 16-item European Health Literacy Survey questionnaire (HLS-EU-Q16, unidimensional) were completed by 1 444 adolescents in 8th, 9th, 11th grade in general school and 11-12th grade in vocational school. Psychometric properties were studied using confirmatory factor analysis, McDonald’s omega coefficient and hypothesis testing. Structural validity was acceptable (HLS-EU-Q16) to good (HAS-A and HLSAC), no measurement invariance issue was found and internal consistency was acceptable for the three scales (0.68-0.84). Convergent validity was low (Pearson correlation coefficients<0.5) and the only scale for which results were in agreement with a priori hypotheses was the HLSAC. Our results were supportive of the use of HLSAC to assess health literacy during adolescence but the HAS-A, with a slightly better structural validity, can also be promoted due to its three measured dimensions. The use of these scales in practice will help to focus on health literacy, a critical factor for prevention and health promotion in adolescence.

Problematic Internet use (PIU) can lead to dysfunction and undesired consequences, especially in adolescents and youths. Preventive interventions can reduce them, but should be built on sound evidence. This review synthesizes the available evidence on the effectiveness of primary prevention programs for PIU in adolescents and youths. It adds to previous reviews by broadening the search to general and specific problematic behavior. Two independent reviewers performed a systematic search for published studies on PIU and internet addiction in English, French and Spanish using PubMed, PsycINFO, Cochrane Register of Controlled Trials, and Scopus. Published experimental and quasi-experimental studies that assessed the effectiveness of primary prevention programs targeting PIU behaviors were considered for inclusion. Full texts for eligible studies were retrieved and assessed for quality. Five studies were retained for narrative synthesis. Three of them based the intervention strategy on an underlying theory, one on media literacy, and one used an educational-based approach. A meta-analysis showed that all five interventions were effective in preventing internet addiction separately. However, when using the Hartung-Knapp-Sidik-Jonkman (HKSJ) estimator and removing one of the studies because of a disproportionate Hedges’ g, combined effects were no longer significant. Although PIU is a popular topic, little is known about ways to prevent it. This review demonstrates that prevention programs can be based on different theoretical approaches, but that the available evidence is too heterogeneous to derive generalizable conclusions concerning their effectiveness.

Healthcare authorities worldwide search for ways to develop integrated care and interprofessional collaboration. In Belgium, Medical-Pharmaceutical Concertation (MPC) was introduced as a format to promote constructive dialogues between GPs and community pharmacists (CPs) with a focus on pharmacotherapy. To evaluate the implementation of MPC from the perspective of healthcare authorities and GPs/CPs. Mixed-methods approach, including semi-structured interviews with stakeholders and service users, observations of MPC meetings and surveys in GPs/CPs. Quantitative data were analyzed using descriptive statistics. Qualitative data were analyzed inductively. The implementation of MPC took a slow start. Parties involved had divergent views on the goals of the MPC: stakeholders focused on measurable results, while service users aimed on improving interprofessional communication. Additionally, service users felt that the lack of local structures hindered consensus building and implementation of agreements in daily practice. Support from professional associations was considered indispensable for the implementation of MPC. In order to organize this efficiently, the establishment of an independent institution, coordinating the MPC initiative, was highly recommended. The study confirms that a thorough context assessment prior to implementation of a complex project is needed and that a step-wise approach should be respected to achieve effective interprofessional relationships.

Self-management education (SME) is a key determinant of diabetes treatment outcomes. While SME programs are often adapted for implementation, the impact of adaptations on diabetes SME effectiveness is not well documented. This study evaluated the impact of the implementation fidelity of diabetes SME programs on program effectiveness, exploring which factors influence implementation fidelity. Data from 33 type 2 diabetes SME program providers and 166 patients were collected in 8 countries (Austria, Belgium, Germany, Ireland, UK, Israel, Taiwan and USA). Program providers completed a questionnaire assessing their adherence to the program protocol and factors that influenced the implementation. Patients answered a pre–post questionnaire assessing their diabetes-related health literacy, self-care behavior, general health and well-being. Associations between implementation fidelity and outcomes were estimated through logistic regressions and repeated measures MANOVA, controlling for potential confounders. Adaptations of the program protocol regarding content, duration, frequency and/or coverage were reported by 39% of the providers and were associated with better, not worse, outcomes than strict adherence. None of the factors related to the participants, facilitating strategies, provider or context systematically influenced the implementation fidelity. Future research should focus on individual and contextual factors that may influence decisions to adapt SME programs for diabetes.

Health literacy is a key topic in public health. Several measurement tools exist that operationalize health literacy, but only a few standard tools measure health literacy at a population level, and none of those are currently available for the Indian context. This study aimed to develop and validate an Indian version of the short form of the European Health literacy Questionnaire (HLS-EU Q16). Following the translation of the English version of the questionnaire in Hindi and Kannada by language experts and confirmation of the item content by health literacy experts, the questionnaire was administered to 158 Hindi speaking and 182 Kannada speaking individuals, selected via purposive sampling. Pearson’s correlation was used to confirm test–retest reliability, and confirmatory factor analysis was used to assess the construct validity of the scales in both languages. Cronbach’s alpha was calculated for the scales and their sub-domains, and item-total correlations were used to calculate item discriminant indices. Discriminant validity was examined by comparing scores of participant groups based on educational status and training in health care. Cronbach’s alpha for the Hindi version of the tool (HLS-IND-HIN-Q16) was 0.98, and for Kannada version (HLS-IND-KAN-Q16) 0.97. Confirmatory factor analysis produced fit indices within acceptable limits. The results allowed us to conclude that the two Indian language questionnaires allow valid and reliable measurements of health literacy among the Hindi and Kannada speaking population of India.

Background The Health Belief Model (HBM) and the Theory of Planned Behavior (TPB) are used to explain screening behavior. Although reviews of each model have been conducted independently, none have compared the application of both to mammography screening. Methods A systematic review of literature published in five databases from 1974 to 2020 was performed. Meta‐analysis of the explanatory value of the HBM and TPB and effect sizes of their cognitive variables was conducted. Results Altogether, 673 papers reporting HBM studies and 577 reporting TPB studies were recovered, of which 43 HBM studies and 15 TPB studies met the eligibility criteria. Twelve studies reported on the explanatory value of either model. The explained variance for HBM ranged from 25% to 89% (mean R2 = .55), while the explained variance for TPB ranged from 16% to 81% (mean R2 = .24 [screening behavior as outcome] and 0.46 [intention as outcome]). The component of “cue to action” had the greatest effect size (mean OR 1.80 [95% CI: 1.58–2.04]). Conclusions While the HBM and TPB both demonstrated positive explanatory value, most studies examined the individual constructs of each model and failed to report consistently on the effectiveness of the models.

Digital health literacy (DHL) is the ability to search, understand and evaluate information from digital media and apply that knowledge to solve health problems. However, currently many citizens have not developed these skills, and this compromises not only the self-management of their health, but the possibility that health services are socially sustainable. The objective of this article was to present the objectives, activities and results of the IC-Health project whose objective was to develop a series of massive open online courses (MOOCs) to improve the DHL skills of European citizens. An exploratory report on DHL’s current evidence was developed. Furthermore, a survey, focus groups and group interviews were conducted to determine DHL levels and the needs of population cohorts (children; adolescents; pregnant and lactating women; the elderly; and people affected by type 1 and type 2 diabetes). A participation strategy with end users was designed through a community of practice for the creation of MOOCs with the seven European countries that participated in the consortium. Thirty-five MOOCs were developed in eight different languages and a descriptive and exploratory assessment of MOOCs was conducted with new participants. This first evaluation indicated that MOOCs can be an effective educational resource for DHL and a facilitator of shared decision-making processes. The process of co-creation of MOOCs, the components, the challenges and the opportunities identified in this European project could be useful for other developers of MOOCs who want to co-create interventions with beneficiaries in similar settings. Further longer-term actions are still needed to improve citizens’ DHL.

The aim of this study was to develop a typology of approaches towards informed decision-making (IFD) about mammography screening in Europe. All countries collaborating in the European Commission Initiative on Breast Cancer were approached to participate. Experts from 28 European countries responded to a web-based survey providing data on key organisational and policy characteristics of breast screening at the national or regional level. A total of 35 responses were received including data from regionally organised breast screening in several countries. 27 respondents, covering 21 countries, reported the existence of a policy towards IFD and stated that they communicated to women about the benefit and risks. Few countries had attempted to measure the proportion of women making an informed choice. A cluster analysis of the survey responses allowed to identify three categories: countries in a confirmation phase who have adopted a policy specific to mammography screening; countries in an implementation phase with generic polices and limited administrative support dedicated yet to IFD; and countries in a decision phase who are deliberating how to address IFD. To the best of our knowledge, this study is the first to investigate the key policy and organisational characteristics of approaches taken to facilitate IFD in Europe. The results demonstrate a broad adoption of the principle of enabling IFD but indicate heterogeneity of implementation.

Background Prebiotic vegetables such as leek and salsify may contribute to preventing obesity by changing the composition of the gut microbiota. To increase consumption of prebiotic vegetables, the aim of the study was to document the prevalence and determinants of (prebiotic) vegetable consumption. Methods An online, correlational questionnaire was administered to participants using a mixed approach (1078 online, 200 face-to-face). Participants were a representative sample (gender, age, level of education, province, population density and (un)employment) of 1278 adults of the Walloon region in Belgium. The frequency and determinants of prebiotic vegetable consumption were measured using an extension of the Theory of Planned Behaviour including habits, actual control and compensatory health beliefs. Descriptive analyses were performed followed by hierarchic multiple regression analyses. Results The descriptive results showed that for all categories (leek, salsify, vegetables in general) an improvement in both intentions and prevalence of the actual behaviour is necessary to experience the health benefits of (prebiotic) vegetables. Intentions and habits were important predictors of consumption for all types of vegetables, and hedonic attitudes and subjective norms were important predictors of intention. Perceived control and rational attitudes were predictors of intention to consume only for vegetables in general. Finally, environmental factors such as price, availability and actual control predict consumption but their influence differs depending on the vegetable. Conclusions The findings can be used to inform interventions that aim to increase (prebiotic) vegetable consumption. Umbrella terms such as ‘healthy food’ or ‘vegetables’ do not capture the differences between the specific foods regarding the demographic and socio-psychological determinants of their consumption. This is the first research to investigate the determinants of prebiotic vegetable consumption.

Health and safety education for farmers has the potential to increase the level of health, safety literacy, and thereby improve farmers’ health and quality of life. The aim of this paper is to provide a systematic review of the published literature documenting different educational interventions for agricultural workers that have the improvement of health and/or safety literacy as an outcome. A systematic search was conducted in PubMed, Embase, Scopus and PsycINFO databases for articles focusing on educational interventions for farmers’ health and safety. From the 3357 initial hits, 36 unduplicated records met the inclusion criteria. The articles included in the review used educational interventions for farmers with the purpose of preventing farm-induced diseases and injuries, increasing the health and well-being of farmers, and promoting good manufacturing practices. The educational approaches considered varied from lectures, videos, newsletters, games, and community fairs, to involving the community in designing the intervention and training farmers to deliver the intervention to the community. Interventions that used evidence-based theories, which took into account cultural aspects and individual factors, used biomarkers as a behavior change measurement, and involved the community in the development of the intervention had the best results in terms of behavior change. The strategies of educational interventions identified in this review that produced good results have the potential to inform future researchers and policy makers in the design and implementation of public health interventions, programs and policies to improve the health of farmers and their families.

This study applied the theory of planned behavior (TPB) to investigate the psychological determinants of using sexually explicit material (SEM) in sex therapy. Self-report data regarding perceived advantages/disadvantages, injunctive and descriptive norms, motivation to comply, self-efficacy, facilitators/obstacles, and intention to using SEM were measured among 59 Belgian sexologists using an online survey. Multiple linear regression analyses showed that 51% of the variance in intention and 41% in SEM use were explained by the model, with perceived advantages and descriptive norms representing the main determinants. The results highlight the need to consider various types of SEM for treating different clinical cases.
