Nasopharyngeal carcinoma (NPC) is a malignant epithelial tumor ubiquitously associated with the Epstein‐Barr virus (EBV), which is highly prevalent in South China, Southeast Asia, and North Africa. Despite being a highly radio‐sensitive and treatable cancer, a majority of NPC patients are diagnosed in their advanced stage, and locoregional and distant relapses following definitive treatment contribute largely to cancer‐specific mortality among these patients. Given that EBV‐driven NPC is the predominant variant seen in endemic regions, various EBV detection methods have been developed and are utilized in screening, prognostication, and post‐treatment surveillance of NPC patients. While the Immunoglobulin A (IgA) serology assay is the most extensively studied EBV detection method, the detection of plasma EBV DNA released during replication or cellular apoptosis has shown superior outcomes in endemic population screening, prognostication, and detection of distant relapse. Furthermore, there is emerging evidence on the use of circulating tumor cells, microRNAs, DNA hypermethylation, and combination assays in various clinical scenarios. Herein, this paper provides a comprehensive overview of the relevant studies using various EBV detection techniques in the management of NPC. Specifically, the recent advances, clinical evidence, and challenges associated with the clinical application of EBV liquid biopsies in population screening, prognostication, and surveillance of NPC are presented.

This paper presents a multi-channel SPAS robotic system consisting of two channels of flexible manipulators and an endoscope channel. Each channel of the manipulators is designed based on a parallel mechanism with three sets of super-elastic Ni-Ti rods and universal joints. This design optimizes the structure for flexibility and safety considerations with a simplified structure compared with conventional parallel mechanisms. Flexible shafts are used for torque transmission, allowing the motors to be farther away from the robotic system. The kinematics of the manipulator is derived, and the reachable workspace and dexterity are analyzed. Furthermore, a prototype of the proposed robotic system is presented. The flexibility of the manipulator is verified via stiffness characterization test. The results of the accuracy tests confirmed that the robotic system can implement manipulation arm with acceptable accuracy. The feasibility and effectiveness of applying the robotic system to the practical operation are also demonstrated through experimentations.

Mucosa-associated invariant T (MAIT) cells are abundant antimicrobial T cells in humans and recognize antigens derived from the microbial riboflavin biosynthetic pathway presented by the MHC-Ib-related protein (MR1). However, the mechanisms responsible for MAIT cell antimicrobial activity are not fully understood, and the efficacy of these mechanisms against antibiotic resistant bacteria has not been explored. Here, we show that MAIT cells mediate MR1-restricted antimicrobial activity against Escherichia coli clinical strains in a manner dependent on the activity of cytolytic proteins but independent of production of pro-inflammatory cytokines or induction of apoptosis in infected cells. The combined action of the pore-forming antimicrobial protein granulysin and the serine protease granzyme B released in response to T cell receptor (TCR)-mediated recognition of MR1-presented antigen is essential to mediate control against both cell-associated and free-living, extracellular forms of E. coli. Furthermore, MAIT cell-mediated bacterial control extends to multidrug-resistant E. coli primary clinical isolates additionally resistant to carbapenems, a class of last resort antibiotics. Notably, high levels of granulysin and granzyme B in the MAIT cell secretomes directly damage bacterial cells by increasing their permeability, rendering initially resistant E. coli susceptible to the bactericidal activity of carbapenems. These findings define the role of cytolytic effector proteins in MAIT cell-mediated antimicrobial activity and indicate that granulysin and granzyme B synergize to restore carbapenem bactericidal activity and overcome carbapenem resistance in E. coli.

A vast majority of the population worldwide are asymptomatic carriers of Epstein-Barr Virus (EBV). However, some infected individuals eventually develop EBV-related cancers, including Nasopharyngeal Carcinoma (NPC). NPC is one of the most common EBV-associated epithelial cancers, and is highly prevalent in Southern China and Southeast Asia. While NPC is highly sensitive to radiotherapy and chemotherapy, there is a lack of effective and durable treatment among the 15%–30% of patients who subsequently develop recurrent disease. Natural Killer (NK) cells are natural immune lymphocytes that are innately primed against virus-infected cells and nascent aberrant transformed cells. As EBV is found in both virally infected and cancer cells, it is of interest to examine the NK cells’ role in both EBV infection and EBV-associated NPC. Herein, we review the current understanding of how EBV-infected cells are cleared by NK cells, and how EBV can evade NK cell-mediated elimination in the context of type II latency in NPC. Next, we summarize the current literature about NPC and NK cell biology. Finally, we discuss the translational potential of NK cells in NPC. This information will deepen our understanding of host immune interactions with EBV-associated NPC and facilitate development of more effective NK-mediated therapies for NPC treatment.

This paper presents a sensor fusion-based surgical instrument tracking system which uses multiple monocular modules. The system is an optical tracking system, which has been widely utilized in the image-guide surgery because of its high accuracy and precision. However, the line-of-sight occlusion problem which remains unresolved in current systems frustrates surgeons during the operation. To address this challenge, we propose a surgical instrument tracking system based on multiple monocular modules. The rationale is to enable the system to track the surgical instruments inside the surgical site from different views. Three sensor fusion algorithms are proposed to integrate all sensor data from the multimodule system. In order to show the feasibility of the tracking system, simulations and comparison experiments have been carried out. The intensive investigation results give a practical instruction to the real implementation of the proposed system in image-guided interventions. Moreover, an image-guided surgical trial by using a cadaver head has been carried out to validate the feasibility of the proposed system and the tracking algorithms. The results from both the simulation and the cadaver trial have shown the effectiveness of the proposed robust fusion algorithm.
