Background: Papillary thyroid carcinoma is a type of indolent tumor with a dramatically increasing incidence rate and stably high survival rate. Reducing the overdiagnosis and overtreatment of papillary thyroid carcinoma is clinically emergent and important. A radiomics model is proposed in this article to predict lymph node metastasis, the most important risk factor of papillary thyroid carcinoma, based on noninvasive routine preoperative ultrasound images. Methods: Four hundred fifty ultrasound manually segmented images of patients with papillary thyroid carcinoma with lymph node status obtained from pathology report were enrolled in our retrospective study. A radiomics evaluation of 614 high-throughput features were calculated, including size, shape, margin, boundary, orientation, position, echo pattern, posterior acoustic pattern, and calcification features. Then, combined feature selection strategy was used to select features with the greatest ability to discriminate lymph node status. A support vector machine classifier was employed to build and validate the prediction model. Another independent testing cohort was used to further evaluate the performance of the radiomics model. Results: Among 614 radiomics features, 50 selected features most reflecting echo pattern, posterior acoustic pattern, and calcification showed the superior lymph node status distinguishable performance with area under the receiver operating characteristic curve of 0.753, 0.740, and 0.743 separately when using each type of features predicting the lymph node status. The results of model based on all 50 final features predicting the lymph node status shown an area under the receiver operating characteristic curve of 0.782, and accuracy of 0.712. In the independent testing cohort, the proposed approach showed similar results, with area under the receiver operating characteristic curve of 0.727 and accuracy of 0.710. Conclusion: Papillary thyroid carcinoma with lymph node metastasis usually shows a complex echo pattern, posterior region homogeneity, and macrocalcification or multiple calcification. The radiomics model proposed in this article is a promising method for assessing the risk of papillary thyroid carcinoma metastasis noninvasively.

Objective: Our aim was to propose a preoperative computer-aided diagnosis scheme to differentiate pancreatic serous cystic neoplasms from other pancreatic cystic neoplasms, providing supportive opinions for clinicians and avoiding overtreatment. Materials and Methods: In this retrospective study, 260 patients with pancreatic cystic neoplasm were included. Each patient underwent a multidetector row computed tomography scan and pancreatic resection. In all, 200 patients constituted a cross-validation cohort, and 60 patients formed an independent validation cohort. Demographic information, clinical information, and multidetector row computed tomography images were obtained from Picture Archiving and Communication Systems. The peripheral margin of each neoplasm was manually outlined by experienced radiologists. A radiomics system containing 24 guideline-based features and 385 radiomics high-throughput features was designed. After the feature extraction, least absolute shrinkage selection operator regression was used to select the most important features. A support vector machine classifier with 5-fold cross-validation was applied to build the diagnostic model. The independent validation cohort was used to validate the performance. Results: Only 31 of 102 serous cystic neoplasm cases in this study were recognized correctly by clinicians before the surgery. Twenty-two features were selected from the radiomics system after 100 bootstrapping repetitions of the least absolute shrinkage selection operator regression. The diagnostic scheme performed accurately and robustly, showing the area under the receiver operating characteristic curve = 0.767, sensitivity = 0.686, and specificity = 0.709. In the independent validation cohort, we acquired similar results with receiver operating characteristic curve = 0.837, sensitivity = 0.667, and specificity = 0.818. Conclusion: The proposed radiomics-based computer-aided diagnosis scheme could increase preoperative diagnostic accuracy and assist clinicians in making accurate management decisions.

Classification of breast ultrasound (BUS) images is an important step in the computer-aided diagnosis (CAD) system for breast cancer. In this paper, a novel phase-based texture descriptor is proposed for efficient and robust classifiers to discriminate benign and malignant tumors in BUS images. The proposed descriptor, namely the phased congruency-based binary pattern (PCBP) is an oriented local texture descriptor that combines the phase congruency (PC) approach with the local binary pattern (LBP). The support vector machine (SVM) is further applied for the tumor classification. To verify the efficiency of the proposed PCBP texture descriptor, we compare the PCBP with other three state-of-art texture descriptors, and experiments are carried out on a BUS image database including 138 cases. The receiver operating characteristic (ROC) analysis is firstly performed and seven criteria are utilized to evaluate the classification performance using different texture descriptors. Then, in order to verify the robustness of the PCBP against illumination variations, we train the SVM classifier on texture features obtained from the original BUS images, and use this classifier to deal with the texture features extracted from BUS images with different illumination conditions (i.e., contrast-improved, gamma-corrected and histogram-equalized). The area under ROC curve (AUC) index is used as the figure of merit to evaluate the classification performances. The proposed PCBP texture descriptor achieves the highest values (i.e. 0.894) and the least variations in respect of the AUC index, regardless of the gray-scale variations. It’s revealed in the experimental results that classifications of BUS images with the proposed PCBP texture descriptor are efficient and robust, which may be potentially useful for breast ultrasound CADs.

As a widely used imaging modality in the medical field, ultrasound has been applied in community medicine, rural medicine, and even telemedicine in recent years. Therefore, the development of portable ultrasound devices has become a popular research topic. However, the limited size of portable ultrasound devices usually degrades the imaging quality, which reduces the diagnostic reliability. To overcome hardware limitations and improve the image quality of portable ultrasound devices, we propose a novel generative adversarial network (GAN) model to achieve mapping between low-quality ultrasound images and corresponding high-quality images. In contrast to the traditional GAN method, our two-stage GAN that cascades a U-Net network prior to the generator as a front end is built to reconstruct the tissue structure, details, and speckle of the reconstructed image. In the training process, an ultrasound plane-wave imaging (PWI) data-based transfer learning method is introduced to facilitate convergence and to eliminate the influence of deformation caused by respiratory activities during data pair acquisition. A gradual tuning strategy is adopted to obtain better results by the PWI transfer learning process. In addition, a comprehensive loss function is presented to combine texture, structure, and perceptual features. Experiments are conducted using simulated, phantom, and clinical data. Our proposed method is compared to four other algorithms, including traditional gray-level-based methods and learning-based methods. The results confirm that the proposed approach obtains the optimum solution for improving quality and offering useful diagnostic information for portable ultrasound images. This technology is of great significance for providing universal medical care.
