Cardiovascular diseases (CVDs) are inflammatory diseases of coronary arteries accompanying atheroma formation that can spawn impairment and, in severe cases, death. CVDs are the leading cause of death in the world. In recent decades, investigators have focused their impact on CVD by periodontal disease (PD). PD is a risk factor that can trigger the formation, maturation, and instability of atheroma in the arteries. Two mechanisms have been proposed to explain this relationship: periodontopathic pathogens explicitly invade the circulation or indirectly increase systemic levels of inflammatory mediators. It has been suggested that improvement in disease state has a positive effect on others. This review summarizes evidence from epidemiological studies as well as researches focusing on potential causation channels to deliver a comprehensive representation of the relationship between PD and CVD.

Periodontal disease is caused by specific pathogens which results in inflammation of the tooth-supporting structures and subsequently causes the continued breakdown of alveolar bone and periodontal ligament. Licorice (Glycyrrhiza glabra) is a perennial herb with substantial medicinal value. Licorice extract is derived from dried, unpeeled stolons and roots of Glycyrrhiza uralensis and G. glabra. The bioactive ingredients in licorice extract such as glycyrrhizin, licoricidin, glabridin, licochalcone A, and licorisoflavan A have anti-inflammatory, antimicrobial, and anti-adherence effects that are beneficial against periodontal disease. Since periodontal disease has a complex etiology that includes the host response and microorganisms, licorice phytochemicals offer a therapeutic advantage due to their dual functionality. The aim of this review was to enumerate the bioactive compounds present in herbal licorice extract and to elucidate the beneficial effects of licorice and its derivatives in periodontal therapy. Literature review and clinical trials evaluating the effect of licorice on periodontopathogens and periodontal disease are included in this article.

Background: N-terminal-pro-brain natriuretic peptide (NT-proBNP) is an inactive hormone that is seen during inflammation and is a known biomarker of cardiovascular disease (CVD). Evidence suggests that periodontitis has a bidirectional relationship with CVD and NT-proBNP has a potential role in periodontal disease. However, there is no evidence on the impact of nonsurgical periodontal therapy (NSPT) on the levels of NT-proBNP in gingival crevicular fluid (GCF) and serum in patients with chronic periodontitis. Hence, the aim of this study was to compare the levels of NT-proBNP in GCF and serum in patients with chronic generalized periodontitis. Materials and methods: GCF and serum samples were collected in 19 patients with chronic periodontitis before and after NSPT after 6 weeks and the cumulative or reduction in values of NT-proBNP in GCF and serum was assessed. NT-proBNP levels in GCF and serum were determined by enzyme-linked immunosorbent assay. Results: The concentrations of NT-proBNP were significantly reduced in GCF and serum after NSPT. Statistically significant difference of NT-proBNP concentration between pre- and postgroups in GCF was apparent (p < 0.0001), whereas statistically nonsignificant results in NT-proBNP serum levels when compared at baseline to postoperative state with mean of 61.77 (22.6 standard deviation [SD]) preoperatively and 72.67 (51.86 SD) postoperatively (p = 0.0007) was observed. Conclusion: Significant reduction of NT-proBNP concentrations in GCF and serum in patients with chronic periodontitis subjected to NSPT was observed. This may account for a significant relation between periodontal disease, bacteremia, and CVD.

In the literature, the periodontal tissue reaction to dissimilar occlusal stress has been described, including clinical and histologic changes caused by stresses in periodontal structures. With respect to occlusal forces, periodontal assembly demonstrates varying adaptive capacity from individual to individual and period to period within the same individual. Unfortunately, these occlusal stresses are yet to be quantified. As a result, determining the effect of normal occlusal force on periodontal elements in various angulations is of interest. Based on CBCT images, one FEA of the maxillary First molar was created, consisting of tooth pulp, periodontal ligament (PDL), and alveolar bone; the effect of normal occlusal force on the pdl in alternate angulations was assessed. Occlusion will occur at three contact areas representing the centric occlusion contact points, each of which will share a 150 N force. The analysis was performed for four force inclinations (0, 22.5°, 45°, and 90°). Maximum stresses are observed in cases of 90-degree loading. These stresses, however, are insignificant and will not cause the periodontal components to rupture. These tensile stresses, which are concentrated in the apical and cervical regions, may obstruct blood flow, resulting in tooth decay or, in some cases, periodontal breakdown in PDL. There have been attempts to express numerical data of stress to be provided for normal and hyper function loads to simulate occlusal situations at various angulations that are known to be accountable for healthy and diseased periodontium.

Objective: In office methods for immediate relief of dentinal hypersensitivity (DH) has long been an area of research. This study compared the efficacy of 660 nm diode laser, 980 nm diode laser, and amorphous calcium phosphate–casein phosphopeptide (ACP–CPP) agent in the treatment of DH.Materials and Methods: A total of 39 patients with minimum three hypersensitive teeth in at least one quadrant were selected and randomly divided into three groups; Group A, B, and C patients were treated by 660 nm diode laser, 980 nm diode laser, and ACP–CPP agent, respectively. All the hypersensitive teeth were stimulated with tactile; thermal and air stimuli, and the pretreatment and posttreatment hypersensitivity scores were evaluated with the numeric rating scale (NRS) at baseline, 30 min, 1 week, 1 month, 3 months, and 6 months. The Shapiro–Wilk test was used to determine the uniformity of the data. The Chi‐square (χ2) test of significance was used to compare proportions. For intergroup comparison, the Kruskal–Wallis test and the Mann–Whitney U test were utilized. Results: At 30 min and 1 week, the 980 nm laser was more effective than the 660 nm laser, but there was no statistically significant difference between the two groups at 1, 3, and 6 months. Throughout the trial period, the 660 nm and 980 nm diode lasers were more effective than the ACP–CPP agent in lowering DH.Conclusions: 660 nm diode laser, 980 nm diode laser, and ACP–CPP agent showed positive prospective as effective desensitizers when utilized as a clinical procedure.

Background: Periodontitis is an inflammatory reaction to subgingival pathogenic microorganisms that causes gradual deterioration of the gingiva, periodontal ligament, and alveolar bone. Interleukin (IL)-21 is the most recently found member of type I cytokine family that is upregulated during inflammation. The current study aims to investigate the biological plausibility of IL-21 as a biomarker for chronic periodontitis. Materials and methods: This cross-sectional clinico-biochemical investigation included 15 systemically healthy, 15 periodontally healthy, 15 chronic gingivitis, and 15 chronic periodontitis subjects aged 25 to 60 years. Following subject enrollment, gingival crevicular fluid (GCF) and blood samples were then taken from each subject. The concentration of IL-21 in all samples was determined using enzyme-linked immunosorbent assay (ELISA) kit. The data was examined using the Kruskal-Wallis test and the Spearman correlation test. Results: Serum IL-21 levels in chronic periodontitis patients were substantially greater than in periodontally healthy individuals. GCF IL-21 levels were substantially greater in gingivitis and chronic periodontitis patients compared to periodontally healthy individuals. In terms of clinical indicators, serum IL-21 levels correlated significantly with bleeding index (BI) in the chronic periodontitis group. In chronic periodontitis group, disease severity as evaluated by probing pocket depth (PPD) and clinical attachment loss (CAL) did not correlate with serum or GCF IL-21 levels. Conclusion: According to the current study’s findings, periodontally involved patients had higher IL-21 levels than periodontally healthy patients, suggesting it can be used as biomarker. Further studies with larger sample size can shed more light on the clinical advantage of IL-21 as a possible marker for disease activity and progression.
