Published literature reports controversial results about the association of physical activity (PA) with risk of hypertension. A meta-analysis of prospective cohort studies was performed to investigate the effect of PA on hypertension risk. PubMed and Embase databases were searched to identify all related prospective cohort studies. The Q test and I 2 statistic were used to examine between-study heterogeneity. Fixed or random effects models were selected based on study heterogeneity. A funnel plot and modified Egger linear regression test were used to estimate publication bias. Thirteen prospective cohort studies were identified, including 136 846 persons who were initially free of hypertension, and 15 607 persons developed hypertension during follow-up. The pooled relative risk (RR) of main results from these studies suggests that both high and moderate levels of recreational PA were associated with decreased risk of hypertension (high versus low: RR, 0.81; 95% confidence interval, 0.76–0.85 and moderate versus low: RR, 0.89; 95% confidence interval, 0.85–0.94). The association of high or moderate occupational PA with decreased hypertension risk was not significant (high versus low: RR, 0.93; 95% confidence interval, 0.81–1.08 and moderate versus low: RR, 0.96; 95% confidence interval, 0.87–1.06). No publication bias was observed. The results of this meta-analysis suggested that there was an inverse dose–response association between levels of recreational PA and risk of hypertension, whereas there was no significant association between occupational PA and hypertension.

HIV testing rates are suboptimal among at-risk men. Crowdsourcing may be a useful tool for designing innovative, community-based HIV testing strategies to increase HIV testing. The purpose of this study was to use a stepped wedge cluster randomized controlled trial (RCT) to evaluate the effect of a crowdsourced HIV intervention on HIV testing uptake among men who have sex with men (MSM) in eight Chinese cities. An HIV testing intervention was developed through a national image contest, a regional strategy designathon, and local message contests. The final intervention included a multimedia HIV testing campaign, an online HIV testing service, and local testing promotion campaigns tailored for MSM. This intervention was evaluated using a closed cohort stepped wedge cluster RCT in eight Chinese cities (Guangzhou, Shenzhen, Zhuhai, and Jiangmen in Guangdong province; Jinan, Qingdao, Yantai, and Jining in Shandong province) from August 2016 to August 2017. MSM were recruited through Blued, a social networking mobile application for MSM, from July 29 to August 21 of 2016. The primary outcome was self-reported HIV testing in the past 3 months. Secondary outcomes included HIV self-testing, facility-based HIV testing, condom use, and syphilis testing. Generalized linear mixed models (GLMMs) were used to analyze primary and secondary outcomes. We enrolled a total of 1,381 MSM. Most were ≤30 years old (82%), unmarried (86%), and had a college degree or higher (65%). The proportion of individuals receiving an HIV test during the intervention periods within a city was 8.9% (95% confidence interval [CI] 2.2–15.5) greater than during the control periods. In addition, the intention-to-treat analysis showed a higher probability of receiving an HIV test during the intervention periods as compared to the control periods (estimated risk ratio [RR] = 1.43, 95% CI 1.19–1.73). The intervention also increased HIV self-testing (RR = 1.89, 95% CI 1.50–2.38). There was no effect on facility-based HIV testing (RR = 1.00, 95% CI 0.79–1.26), condom use (RR = 1.00, 95% CI 0.86–1.17), or syphilis testing (RR = 0.92, 95% CI 0.70–1.21). A total of 48.6% (593/1,219) of participants reported that they received HIV self-testing. Among men who received two HIV tests, 32 individuals seroconverted during the 1-year study period. Study limitations include the use of self-reported HIV testing data among a subset of men and non-completion of the final survey by 23% of participants. Our study population was a young online group in urban China and the relevance of our findings to other populations will require further investigation. In this setting, crowdsourcing was effective for developing and strengthening community-based HIV testing services for MSM. Crowdsourced interventions may be an important tool for the scale-up of HIV testing services among MSM in low- and middle-income countries (LMIC). ClinicalTrials.gov NCT02796963

Background In the past, many data collection systems were in operation for different HIV/AIDS projects in China. We describe the creation of a unified, web-based national HIV/AIDS information system designed to streamline data collection and facilitate data use. Methods Integration of separate HIV/AIDS data systems was carried out in three phases. Phase 1, from January 2006 to December 2007, involved creating a set of unified data collection forms that took into account existing program needs and the reporting requirements of various international organizations. Phase 2, from January to October 2007, involved creating a web-based platform to host the integrated HIV/AIDS data collection system. Phase 3, from November to December 2007, involved pilot testing the new, integrated system prior to nationwide application. Results Eight web-based data collection subsystems based on one platform began operation on 1 January 2008. These eight subsystems cover: (i) HIV/AIDS case reporting; (ii) HIV testing and counselling; (iii) antiretroviral treatment (ART) for adults; (iv) ART for children; (v) behavioural interventions for high-risk groups; (vi) methadone maintenance treatment; (vii) sentinel and behavioural surveillance; and (viii) local county background information. The system provides real-time data to monitor HIV testing, prevention and treatment programs across the country. Conclusion China’s new unified, web-based HIV/AIDS information system has improved the efficiency of data collection, reporting, analysis and use, as well as data quality and security. It is a powerful tool to support policy making, program evaluation and implementation of the national HIV/AIDS program and, thus, may serve a model for other countries.

Objectives: To find and compare the levels of acceptance of and barriers to voluntary counselling and testing (VCT) among adults in two different counties of Guizhou province, China, one in which the China CARES project was operating and the other in which it was not. Design: A longitudinal design with two-stage cluster sampling was employed. Methods: A total of 1012 participants were recruited in the two counties. All participants were interviewed, then given a coupon for free VCT after the interview. Participants were paid for returning the coupon within 2 months, whether tested or not. The uptake of VCT was measured within 2 months after the interview. Results: The study found that the levels of HIV/AIDS knowledge and acceptability of VCT among the adults in both counties were low. Although 459 participants (43.5%) expressed an intent to use the VCT services, only 193 (16.5%) actually visited the VCT facilities, and only 42 (3.7%) actually took an HIV test within 2 months after the interview. The use of VCT was related to occupation, age, transportation difficulties, health status, ethnicity, and high-risk behaviors. The main barriers to HIV testing included perceiving oneself as low risk, fear of unsolicited disclosure, and fear of stigma and discrimination that would result from taking the test. Conclusion: Education about HIV/AIDS and VCT needs to be improved, and levels of stigma and discrimination reduced, in order to enhance the uptake of VCT services, an essential step for the initiation of treatment.

SUMMARY: Scrub typhus is emerging and re-emerging in many areas: climate change may affect its spread. To explore the effects of meteorological factors on scrub typhus, monthly cases of scrub typhus from January 2006 to December 2012 in the Laiwu district of temperate northern China were analysed. We examined the correlations between scrub typhus and meteorological factors (and their delayed effects). We built a time-series adjusted negative binomial model to reflect the relationships between climate variables and scrub typhus cases. The key determinants of scrub typhus transmission were temperature, relative humidity and precipitation. Each 1°C increase in monthly average temperature in the previous 3 months, each 1% increase in monthly relative humidity in the previous 2 months and each 1 mm increase in monthly precipitation in the previous 3 months induced 15·4%, 12·6% and 0·7% increases in the monthly number of cases, respectively. In conclusion, scrub typhus is affected by climate change in temperate regions.

This study aimed to describe the characteristics of natural disasters and associated losses from 1985 to 2014. The Mann-Kendall method was used to detect any long-term trends and abrupt changes. Hotspot analysis was conducted to detect the spatial clusters of disasters. We found an increasing trend in the occurrence of integrated natural disasters (tau = 0.594, p < 0.001), particularly for floods (tau = 0.507, p < 0.001), landslides (tau = 0.365, p = 0.009) and storms (tau = 0.289, p = 0.032). Besides, there was an abrupt increase of natural disasters in 1998–2000. Hotspots of droughts, floods, landslides and storms were identified in central, southern, southwest and southeast areas of China, respectively. Annual deaths from integrated natural disasters were decreasing (tau = −0.237, p = 0.068) at about 32 persons/year, decreasing at 17 persons/year for floods (tau = −0.154, p = 0.239), and decreasing at approximately 12 persons/year for storms (tau = −0.338, p = 0.009). No significant trend was detected in inflation-adjusted damages while a declining trend was detected in the ratio of year damage against GDP (gross domestic product). In conclusion, there has been an increasing trend in occurrence of natural disasters in China with the absence of an increase in life and economic losses. Despite the progress in the disaster adaption, there will be great challenges in disaster control for China in the future.
