Background: Chronic inflammation is a pathophysiological cause of sarcopenia in Crohn’s disease (CD) patients. However, the potential impact of diet-related inflammation on sarcopenia has not yet been adequately investigated. We examined the associations between Dietary Inflammatory Index (DII) and sarcopenia in CD patients. Methods: A total of 140 CD patients from Ruijin Hospital in Shanghai were included in this cross-sectional study. DII scores were calculated from the dietary data collected using a validated food frequency questionnaire (FFQ). Sarcopenia was determined according to the Asian Working Group for Sarcopenia. Multivariable logistic regression analyses were performed to determine the association between DII and sarcopenia. Results: The mean DII score was 0.81 ± 2.13, ranging from −3.24 to 4.89. The overall prevalence of sarcopenia was 26.4%. The higher DII score significantly increased the risk of sarcopenia in CD patients (ORQuartile4vs1: 9.59, 95% CI: 1.69, 54.42, ptrend = 0.031) in the multivariable model after adjusting for more potential confounders. Moreover, CD patients with a lower DII had a significantly higher appendicular skeletal muscle mass index (ASMI, ORQuartile4vs1: 5.48, 95% CI: 1.51, 19.87, ptrend = 0.018) after adjusting for age, gender, BMI, smoking status and drinking status model. Yet, there were no significant differences between DII and ASMI after adjusting for more potential confounders. Additionally, no significant association was observed between DII and handgrip strength in the multivariable-adjusted models. Conclusions: Pro-inflammatory diet was associated with increased risk of sarcopenia in CD patients. CD patients should have a proper intake of energy and protein. These patients could also benefit from supplementation with enteral nutrition due to its anti-inflammatory potential.

Background: The global COVID-19 pandemic has been widely regarded as a catalyst for adopting internet health care technology (IHT) in China. IHT consists of new health care technologies that are shaping health services and medical consultations. Health care professionals play a substantial role in the adoption of any IHT, but the consequences of doing so can often be challenging, particularly when employee burnout is prevalent. Few studies have explored whether employee burnout influences the adoption intention of IHT in health care professionals. Objective: This study aims to explain the determinants influencing the adoption of IHT from the perspective of health care professionals. To do so, the study extends the value-based adoption model (VAM) with consideration for employee burnout as a determining factor. Methods: A cross-sectional web-based survey using a sample of 12,031 health care professionals selected through multistage cluster sampling from 3 provinces in mainland China was conducted. The hypotheses of our research model were developed based on the VAM and employee burnout theory. Structural equation modeling was then used to test the research hypotheses. Results: The results indicate that perceived usefulness, perceived enjoyment, and perceived complexity positively correlate with perceived value (β=.131, P=.01; β=.638, P<.001; β=.198, P<.001, respectively). Perceived value had a positive direct effect on adoption intention (β=.725, P<.001), perceived risk negatively correlated with perceived value (β=−.083, P<.001), and perceived value negatively correlated with employee burnout (β=−.308, P<.001). In addition, employee burnout was negatively related to adoption intention (β=−.170, P<.001) and mediated the relationship between perceived value and adoption intention (β=.052, P<.001). Conclusions: Perceived value, perceived enjoyment, and employee burnout were the most important determinants of IHT adoption intention by health care professionals. In addition, while employee burnout was negatively related to adoption intention, perceived value inhibited employee burnout. Therefore, this study finds that it is necessary to develop strategies to improve the perceived value and reduce employee burnout, which will benefit the promotion of the adoption intention of IHT in health care professionals. This study supports the use of the VAM and employee burnout in explaining health care professionals’ adoption intention regarding IHT.

Background: Some studies have shown that a pro-inflammatory diet may be associated with cognitive function, but their conclusions have varied considerably. We here present a meta-analysis of the current published literature on DII score and its association with cognitive health. Methods: In this meta-analysis, the PubMed, Embase, Web of Science, and Cochrane databases were searched in September 2022. The reported indexes, specifically OR, RR, and β, were extracted and analyzed using R version 3.1.0.Results: A total of 636 studies in databases were identified, and 12 were included in the meta-analysis. Higher DII was associated with an increased risk of AD and MCI (OR = 1.34; 95% CI = 1.21–1.49). Meanwhile, it may also cause global function impairment (categorical: OR = 1.63; 95% CI = 1.36–1.96) and verbal fluency impairment (continuous: OR = 0.18; 95% IC = 0.08–0.42). But there was no significant association between DII and executive function (categorical: OR = 1.12; 95% IC = 0.84–1.49; continuous: OR = 0.48; 95% IC = 0.19–1.21) or episodic memory (continuous: OR = 0.56; 95% IC = 0.30–1.03).Conclusion: A pro-inflammatory diet is related to AD, MCI, and the functions of some cognitive domains (specifically global function and verbal fluency). However, the current evidence on the role of diet-induced inflammation in different cognitive domains should be supported by further studies in the future.

Social media has become a significant platform for health knowledge popularization worldwide, especially during public health crises. However, the factors influencing healthcare professionals’ intention to engage in health knowledge popularization on social media, particularly in China, remain underexplored due to the unique socio-medical environment. This study examines determinants of Chinese healthcare professionals’ social media-based health knowledge popularization participation, evaluating the applicability of the Unified Theory of Acceptance and Use of Technology (UTAUT) and Perceived Risk Theory. We developed a survey integrating UTAUT constructs (performance expectancy, effort expectancy, social influence, facilitating conditions) with perceived risk. Data from 762 professionals across four Shanghai tertiary hospitals were analyzed using structural equation modeling. Social influence (β = 0.299, p < 0.001), performance expectancy (β = 0.221, p < 0.001), and effort expectancy (β = 0.312, p < 0.001) significantly positively predicted participation intention. Facilitating conditions directly affected actual behavior (β = 0.079, p < 0.05), while perceived risk showed no significant association with intention (β=-0.019, p = 0.581). Institutional support emerges as crucial in mediating healthcare professionals’ social media engagement, with social influence and anticipated effectiveness being primary motivators. The non-significant role of perceived risk may reflect China’s performance evaluation systems and policy safeguards mitigating professional concerns. Practical implications suggest that enhancing organizational support through resource allocation and workload adjustments could effectively promote online health knowledge popularization participation. Objective The purpose of this study is to evaluate the effectiveness of hospital appraisals, specifically the Performance Appraisal for Tertiary Public Hospitals (PATPH), and to examine its impact on the job satisfaction of healthcare professionals in tertiary public hospitals in China. Design A cross-sectional study using a multistage sampling method. Improvements induced by PATPH in the working environment, job satisfaction and other covariates were measured. A series of weighted linear regressions with weights from the inverse probability-of-treatment weighting method were used to examine the effect of PATPH on job satisfaction. Setting Nine tertiary public hospitals across three economic and geographic regions in China. Participants In August 2020, a total of 13 211 hospital employees were surveyed, and 8417 doctors and nurses fully completed questionnaires forming the primary dataset for analysis. Of these respondents, males comprised 18.64% and doctors constituted 28.15%. Results This study revealed that PATPH had a positive impact on the job satisfaction of healthcare professionals. A ‘more effective’ PATPH working environment resulted in an improvement of 9.57 points (95% CI 8.99 to 10.16) in job satisfaction scores, controlling for all other variables. The finding persisted consistently through a series of sensitivity analyses. Conclusion The findings offered insights and inspiration for improving the job satisfaction of healthcare professionals, especially in the development of macrolevel policies targeted towards organisational enhancement.

The nutritional status of older adults requires focused attention. In Chinese nursing homes, nursing assistants are often the primary caregivers responsible for the daily care of older adults, and their knowledge and attitudes toward nutrition play a fundamental role in providing adequate nutritional care. However, research on the knowledge and attitudes of nursing assistants is limited, despite the fact that these factors play a fundamental role in providing adequate nutritional practices. The aim of this study is to investigate the knowledge, attitudes, and practices (KAP) of nursing assistants in nursing homes and to analyze the influencing factors. This cross-sectional study was conducted from January to September 2023, involving 692 nursing assistants from 76 nursing homes in Shanghai. We used validated Nutrition Knowledge, Attitude, and Practice Questionnaire for nursing assistants. The findings indicated that the mean scores for nutritional knowledge, attitudes, and practices were 45.07 ± 16.18, 76.22 ± 10.03, and 69.94 ± 21.46, respectively. Notably, the mean score for nutritional knowledge was the lowest, with 66.04% of nursing assistants scored below 50% accuracy. A significant negative correlation was observed between nutritional knowledge and practices (r=-0.194, p < 0.001), while a positive correlation was found between attitudes and practices (r = 0.422, p < 0.001). Key predictors of higher nutritional knowledge included education level (B = 3.907, p < 0.001), whereas skill deficiency (B=-3.714, p = 0.006), years of caring (B=-2.789, p < 0.001) and regular supervision (B=-3.422, p = 0.018) were negatively associated. Positive nutritional attitudes were associated with higher education (B = 2.453, p < 0.001), years of caring (B = 1.177, p < 0.001), and participation in nutrition training (B = 4.138, p < 0.001). Nutritional practices were positively influenced by age (B = 3.068, p = 0.002), participation in nutrition training (B = 11.474, p < 0.001) and regular supervision (B = 14.597, p < 0.001), but negatively affected by insufficient income (B=-3.189, p = 0.030). This study reveals a significant deficiency in nutritional knowledge among nursing assistants in Shanghai’s nursing homes, which adversely impacts their nutritional practices. The findings underscore the urgent need for enhanced training programs and to improve the nutritional competencies of nursing assistants. By addressing these gaps through targeted education and consistent supervision, the quality of nutritional care provided to older adults in nursing homes can be substantially improved.
