Background: Rheumatoid arthritis (RA) is a chronic inflammatory disabling autoimmune disorder. Little is known regarding the association between the gut microbiome and etiopathogenesis of RA. We aimed to dissect the differences in gut microbiomes associated with RA in comparison to healthy individuals and, in addition, to identify the shifts in the bacterial community in association with disease activity; Methods: In order to identify compositional shifts in gut microbiomes of RA patients, V3-V4 hypervariable regions of 16S rRNA were sequenced using Illumina MiSeq. In total, sixty stool samples were collected from 45 patients with RA besides 15 matched healthy subjects; Results: Notably, RA microbiomes were significantly associated with diverse bacterial communities compared with healthy individuals. Likewise, a direct association between bacterial diversity and disease activity was detected in RA patients (Kruskal Wallis; p = 0.00047). In general, genus-level analysis revealed a positive coexistence between RA and Megasphaera, Adlercreutzia, Ruminococcus, Bacteroides, Collinsella, and Acidaminococcus. Furthermore, Spearman correlation analysis significantly stratified the most dominant genera into distinct clusters that were mainly based on disease activity (r ≥ 0.6; p ≤ 0.05). The predictive metabolic profile of bacterial communities associated with RA could support the potential impact of gut microbiomes in either the development or recovery of RA; Conclusions: The overall shifts in bacterial composition at different disease statuses could confirm the cross-linking of certain genera either to causation or progression of RA.

Objectives. Standing and walking serve an individual’s basic needs to move from place to place, and both are the most common activities that people do daily. So, this study aims to investigate the combined effect of botulinum A injection and ankle weight on excessive knee flexion in diplegic children with crouch gait. Methods. Sixty children with spastic diplegia walking with a crouch gait were included in this study. They were divided equally into three groups (twenty in each): group A received classical gait rehabilitation, group B received the same gait training while adding ankle weights, and group C received the same as group A and B plus botulinum A injection. The modified Ashworth scale (MAS) and Hoffman reflex/Myogenic response (H/M ratio) were used to evaluate the spasticity of the hamstring and gastrocnemius muscles, while two‐dimension gait analysis was used to record knee flexion angles during gait. The assessment was held one day before starting the treatment and after completing three months of the treatment program. Results. There was no significant difference between groups before treatment regarding all measured variables. group A revealed a statistically nonsignificant improvement after treatment. Patients in group B showed significant improvement after treatment for both knees regarding the H/M ratio and MAS, which was reflected in the right and left knee range of motion at initial contact (P values 0.030 and 0.001, respectively) and midstance (P values 0.030 and 0.006, respectively). However, more significant improvement was detected regarding all studied variables in both knees after treatment in group C patients with a P value <0.001. Conclusion. The combination of botulinum A injection and ankle weights was more effective in controlling excessive knee flexion in diplegic children with a crouch gait.

Background Bone abnormalities are frequently developed in hemodialysis patients and people with chronic kidney disease (CKD). It results from aberrant bone turnover and a decline in bone mineral density (BMD). Aim and objectives: With the help of dual energy X-ray absorptiometry in this study, poor bone mineral density in CKD patients at Al-Hussein University Hospital will be analysed for prevalence and severity. Subjects and methods: This Cross-Sectional research included 120 Patients (2 groups) 60 CKD Patients and 60 Hemodialysis Patients. The study has been conducted in Nephrology Unit Al-Hussien University Hospital. Results: In ESRD group there was Significant statistically (p-value = 0.008) T score femur and eGFR had a positive connection (r = 0.34). Significant statistically (p-value = 0.008) There is an r-value of 0.34 between the T score of the spine and the eGFR. While in CKD group significantly significant (p-value = 0.001) differences. T score femur and Creat have a negative connection (r = -0.41) with each other. Significant statistically (p-value = 0.001) T score femur and eGFR had a positive connection (r = 0.43). Significant statistically (p-value = 0.02) The T score forearm and the eGFR have a positive connection (r = 0.3). Conclusion: MBD are common features among CKD patients especially in late stages leading to Osteoporosis and fractures. Dual Energy X- Ray (DXA) could be utilized as efficient method for detection of the level of bone affliction in CKD patients and is essential for bone loss monitoring Keywords: mineral bone disorders, chronic kidney diseases.
