Organ fibrosis is a common pathological result of various chronic diseases with multiple causes. Fibrosis is characterized by the excessive deposition of extracellular matrix and eventually leads to the destruction of the tissue structure and impaired organ function. Prostaglandins are produced by arachidonic acid through cyclooxygenases and various prostaglandin-specific synthases. Prostaglandins bind to homologous receptors on adjacent tissue cells in an autocrine or paracrine manner and participate in the regulation of a series of physiological or pathological processes, including fibrosis. This review summarizes the properties, synthesis, and degradation of various prostaglandins, as well as the roles of these prostaglandins and their receptors in fibrosis in multiple models to reveal the clinical significance of prostaglandins and their receptors in the treatment of fibrosis.
Medical image segmentation is an important part of medical image analysis. With the rapid development of convolutional neural networks in image processing, deep learning methods have achieved great success in the field of medical image processing. Deep learning is also used in the field of auxiliary diagnosis of glaucoma, and the effective segmentation of the optic disc area plays an important assistant role in the diagnosis of doctors in the clinical diagnosis of glaucoma. Previously, many U-Net-based optic disc segmentation methods have been proposed. However, the channel dependence of different levels of features is ignored. The performance of fundus image segmentation in small areas is not satisfactory. In this paper, we propose a new aggregation channel attention network to make full use of the influence of context information on semantic segmentation. Different from the existing attention mechanism, we exploit channel dependencies and integrate information of different scales into the attention mechanism. At the same time, we improved the basic classification framework based on cross entropy, combined the dice coefficient and cross entropy, and balanced the contribution of dice coefficients and cross entropy loss to the segmentation task, which enhanced the performance of the network in small area segmentation. The network retains more image features, restores the significant features more accurately, and further improves the segmentation performance of medical images. We apply it to the fundus optic disc segmentation task. We demonstrate the segmentation performance of the model on the Messidor dataset and the RIM-ONE dataset, and evaluate the proposed architecture. Experimental results show that our network architecture improves the prediction performance of the base architectures under different datasets while maintaining the computational efficiency. The results render that the proposed technologies improve the segmentation with 0.0469 overlapping error on Messidor.
Recent evidence suggests that chloride channels are critical for cell proliferation, migration, and differentiation. We examined the effects of transforming growth factor (TGF)-β1 on chloride channel expression and associations with human conjunctival fibroblast (HConF) biology. To investigate the potential role of chloride channel (CLC)-2 in migration, transition to myofibroblasts and extracellular matrix (ECM) synthesis of HconF, a small interfering RNA (siRNA) approach was applied. TGF-β1-induced migration and transition of fibroblasts to myofibroblasts characterized by α-smooth muscle actin (α-SMA) expression, supported by increased endogenous expression of CLC-2 protein and mRNA transcripts. ECM (collagen I and fibronectin) synthesis in HConF was enhanced by TGF-β1. CLC-2 siRNA treatment reduced TGF-β1-induced cell migration, transition of fibroblasts to myofibroblasts, and ECM synthesis of HConF. CLC-2 siRNA treatment in the presence of TGF-β1 inhibited phosphorylation of PI3K and Akt in HConF. These findings demonstrate that CLC-2 chloride channels are important for TGF-β1-induced migration, differentiation, and ECM synthesis via PI3K/Akt signaling in HConF.
Rationale: Generally, there is no lipoprotein in aqueous humor, and chyle usually exists transiently in the body. Therefore, persistent chylous aqueous humor is rare. Patient concerns: We report a case of a 39-year-old man with persistent milky white appearance over the right eye. Diagnoses: The patient had a history of poorly controlled diabetes for the past 2 years and central retinal vein occlusion of the same eye for the past 2 weeks. The patient’s right eye had a uniform milky appearance in the anterior chamber, transparent cornea, and no keratic precipitate in the posterior cornea. Color Doppler ultrasound of the affected eye showed no obvious inflammation in the vitreous cavity. Laboratory tests revealed severe chylemia. The patient was finally diagnosed as chylous aqueous humor. Interventions and outcomes: After conventional hypolipidemia and hypoglycemia treatment and locally glucocorticoid treatment. The milky white changes in the anterior chamber improved considerably and finally disappeared. Lessons: Although the impact of hyperlipidemia on the cardiovascular system and digestive system is well known, its impact on the eyes is often overlooked. We report a rare case of unilateral chylous aqueous humor caused by hyperlipidemia. Through the analysis of this special case, we recommend that ophthalmologists should pay attention to the impact of blood lipid change on eyes.
