This study evaluates the accuracy and repeatability of craniofacial measurements with a 3D light scanner, specifically the EINSTAR scanner, in comparison to traditional caliper measurements for facial anthropometry. Eleven volunteers were assessed by two examiners, one experienced and one inexperienced, who performed direct caliper measurements and indirect measurements using the scanner. Results indicated minimal differences between caliper and scanner results, with overall high accuracy and reliability demonstrated by correlation coefficients. Despite the slightly longer scanning time, the benefits of 3D imaging, including detailed surface mapping and virtual modeling, justify its integration into clinical practice, particularly in maxillofacial surgery and craniofacial assessment. Craniofacial measurements obtained with the EINSTAR scanner showed excellent reliability and accuracy, which qualifies this method for clinical and scientific use.

Background: Medication-related osteonecrosis of the jaw (MRONJ) is a type of jawbone necrosis caused by the use of drugs for some types of cancer and osteoporosis. The current study aimed to evaluate the associations between hyperglycemia and the development of medication-related osteonecrosis of the jaw. Methods: Our research group investigated data collected between 1 January 2019 and 31 December 2020. A total of 260 patients were selected from the Inpatient Care Unit, Department of Oromaxillofacial Surgery and Stomatology, Semmelweis University. Fasting glucose data were used and included in the study. Results: Approximately 40% of the necrosis group and 21% of the control group presented with hyperglycemia. There was a significant association between hyperglycemia and MRONJ (p < 0.05, p = 0.003). Vascular anomaly and immune dysfunction caused by hyperglycemia can lead to necrosis after tooth extraction. Necrosis is more common in the mandible (75.0%) and in the case of parenteral antiresorptive treatment (intravenous Zoledronate and subcutaneous Denosumab). Hyperglycemia is a more relevant risk factor than bad oral habits (26.7%). Conclusions: Ischemia is a complication of abnormal glucose levels, a possible risk factor for necrosis development. Hence, uncontrolled or poorly regulated plasma glucose levels can significantly increase the risk of jawbone necrosis after invasive dental or oral surgical interventions.

Background: Periapical surgery is indicated for persistent periapical lesions that do not respond to conventional endodontic therapy, yet postoperative recovery is often hindered by pain, swelling, and delayed healing. Platelet-rich fibrin (PRF) membranes are autologous biomaterials with regenerative potential, capable of modulating inflammation and promoting tissue repair. Methods: This preliminary randomized controlled trial evaluated the effectiveness of PRF membranes in improving postoperative outcomes—specifically pain, swelling, and quality of life—after apicoectomy. Twenty patients requiring periapical surgery were randomly allocated to a PRF group (n = 10) or a control group (n = 10). In the PRF group, autologous PRF membranes were applied over the resected root-end and into the osteotomy cavity before flap closure. In the control group, no PRF membranes or any additional biomaterial were applied, apart from the standard root-end filling material (MTA), which was identically used in both groups as part of the routine apicoectomy protocol. All patients were blinded to allocation, and outcomes were assessed by an independent blinded evaluator. Facial swelling was quantified by 3D facial scanning, pain was recorded daily using a visual analog scale (VAS), and quality of life was evaluated with the PROMIS-29+2 Profile. Results: The PRF group showed significantly reduced swelling (mean volume difference, 7.12 cm3; p = 0.025), lower pain scores (VAS: 1.80 ± 1.22 vs. 3.80 ± 2.44; p = 0.034), and improved quality-of-life domains, including higher Physical Function (p = 0.032) and lower Sleep Disturbance (p = 0.008) scores. Conclusions: Within the limitations of this pilot study, PRF membranes enhanced postoperative recovery after periapical surgery by reducing swelling and pain while improving patient-reported outcomes. Larger multicenter trials are needed to confirm these preliminary findings.

This case report aimed to describe a rare benign mandibular tumour and assess the outcomes of the most recent reviews, between January 2017 and August 2023. Presenting a detailed clinical case, this study advances our understanding of the diagnostic and therapeutic aspects, ultimately improving the management of similar cases in clinical practice. Orthopantomogram (OPG) revealed a well-defined unilocular radiolucency extending from the midline of the ramus and teeth 47 and 48 were submerged at the base of the mandible. In the presented case, a PLANMECA ROMEXIS PROMAX® three-dimensional (3D) maximum (MAX) cone-beam computed tomography (CBCT) device was used for the 3D examination. An intraoral approach was preferred and the tumour was removed in toto by creating a bone window using a W&H® Dentalwerk Bürmoos GmbH Piezomed piezoelectric device, and the bone plates were fixed with 4 MEDARTIS® microplates, with a primary flap closure. A PANORAMIC 1000, 3DHISTECH Ltd® device was employed for the histological investigation. Odontogenic tumours are rare and typically asymptomatic, often discovered incidentally during routine radiographic examinations. Most of these benign lesions heal well after complete excision and require long-term follow-up. Once diagnosed, ameloblastic fibroma (AF) should be treated immediately to avoid malignant transformation.

The COVID-19 pandemic has significantly impacted healthcare systems worldwide, including dental care. This study aimed to investigate the effects of the pandemic on the management of medication-related osteonecrosis of the jaw (MRONJ). Abnormal blood glucose levels may contribute to the development of MRONJ and act as an important risk factor. This retrospective study included 217 patients with MRONJ. The patients were divided into two groups: the pre-COVID-19 group (16 March 2018 to 16 March 2020; 75 patients; 46 females and 29 males; average age, 74.5 years) and the post-COVID-19 group (1 June 2022 to 1 June 2024; 142 patients; 91 females and 51 males; average age, 69.6 years). Data pertaining to demographic characteristics, length of hospital stay, glucose levels, location of lesions, and underlying diseases were collected. The average length of hospital stays was 4 and 5 days in the pre- and post-COVID-19 groups, respectively. The average fasting glucose levels were 5.5 and 5.9 mmol/L in the pre- and post-COVID-19 groups, respectively. Localization patterns shifted, with a higher incidence in the maxilla in the post-COVID-19 group. These findings suggest a significant increase in MRONJ cases and changes in clinical outcomes due to the pandemic. The increase in the number of patients treated after the pandemic highlights the importance of ongoing vigilance and adaptation in preventing MRONJ, with a particular focus on risk factors.

Oral mucosal and periodontal diseases are commonly associated with persistent inflammation, oxidative stress, impaired wound healing, and reduced oral health-related quality of life. Nano-Bio Fusion (NBF) gingival gel is a bioadhesive nano-formulated oral gel containing propolis, vitamin C, and vitamin E, developed for local application under oral soft tissue conditions. This critical narrative review aimed to evaluate the currently available evidence regarding the clinical applications, safety profile, and therapeutic potential of NBF gel in oral soft tissue therapy. A structured non-systematic literature search was performed using PubMed, Embase, Cochrane Library, and Google Scholar, and 16 relevant studies were included. The available evidence suggests that NBF gel may provide clinical benefits as an adjunct to non-surgical periodontal therapy, with reported improvements in plaque and gingival indices, periodontal probing depth, clinical attachment level, wound healing, and pain-related outcomes. In addition, potential beneficial effects have been reported in oral surgery-related wound healing, alveolar osteitis, desquamative gingivitis, erosive lichen planus, and xerostomia-associated mucositis. Several studies reported outcomes comparable to conventional therapies, including chlorhexidine-based regimens and locally delivered antimicrobials; however, the evidence remains heterogeneous and limited. Furthermore, the proposed biological mechanisms, including antioxidant, antimicrobial, and tissue-modulating effects, are not yet fully supported by mechanistic or pharmacokinetic evidence. The currently available literature is limited by heterogeneity in study design, small sample sizes, short follow-up periods, and limited independent validation. Therefore, further well-designed, adequately powered randomized controlled trials with standardized methodologies are required to better define the clinical role of NBF gel in evidence-based oral soft tissue therapy.
