Takayasu arteritis (TA) is a rare, chronic, inflammatory vasculitis that primarily affects large arteries, causing significant morbidity and mortality. This review provides an overview of the pathophysiology, diagnosis, and management of TA based on current advances in the field. TA is characterized by autoimmune-mediated inflammation, vascular remodeling, and endothelial dysfunction. The disease progresses through three stages (active, chronic, and healing phase) each presenting distinct clinical features. Diagnosis of TA can be challenging due to non-specific clinical manifestations and the lack of specific diagnostic tests. Various imaging modalities, such as angiography, ultrasound, and Doppler techniques, play a crucial role in the diagnosis of TA by visualizing arterial involvement and assessing disease extent. Management of TA involves a multidisciplinary approach, with disease-modifying anti-rheumatic drugs (DMARDs) as the cornerstone of medical therapy. Synthetic and biologic DMARDs are used to induce remission, control inflammation, and prevent complications. Non-pharmacologic interventions, such as resistance exercises and curcumin supplementation, show potential benefits. Invasive interventions, including endovascular therapy and open surgery, are used for managing vascular lesions. However, challenges remain in disease understanding and management, including the heterogeneity of disease presentation and the lack of standardized treatment guidelines. The future of TA management lies in precision medicine, utilizing biomarkers and molecular profiling to personalize treatment approaches and improve patient outcomes. Further research is needed to unravel the underlying mechanisms of TA and develop targeted therapies.

This study sought to evaluate the influence of knowledge, attitude, and practices assessment on diabetes related to obesity in Pakistani society. Data was collected both through door to door and online survey approaches from 518 participants by using a pre-validated questionnaire. A total of 15.6% were underweight, 61.2% were normal weight, 17.2% were overweight and 5.8% were in obesity class I and 2.9% were classified as obesity class II. The co-occurrence of obesity and diabetes was 29% (n = 22) among diabetic individuals (n = 84). A majority of the residents (59.1%) were from cities. While 94% of the participants responded to what obesity was, 83.8% knew what diabetes was. Fast food, soft drinks, and mayonnaise were deemed to be healthy by 75.1% of the respondents. Obesity was viewed as a disease by 94.8%, a major health issue by 78.2%, and a weight-loss necessity by 44.6% of participants. Only 24.9% exercised every day, and 23.9% engaged in any physical activity daily. The majority of respondents (50.6%) never tried to lose weight and 23.2% ate junk food daily. The sociodemographic variables showed that the age ranges of 25–34 years (P < 0.001; OR 0.531), 45–54 years (P < 0.05; OR 0.527), and urban residency (P < 0.001; OR 0.128) had a significant association with knowledge. The factors of urban residency (P < 0.001; OR 3.996), being unmarried (P < 0.001; OR 1.95), and having an income of 51,000–70,000 (P < 0.001; OR 11.29) showed a very highly significant association with a good attitude regarding the relationship of obesity with diabetes (P < 0.05). Similarly, practices of the participants showed significant association with BMI range of 18.5–24.9 and 25–29.9 (P < 0.001). Our study revealed significant knowledge and understanding of the relationship between obesity with diabetes. However, it was observed that majority of respondents exhibited fundamental knowledge regarding obesity and diabetes, there was a notable absence of understanding regarding crucial elements, such as the significance of maintaining a healthy body weight, participating in physical activity, and implementing appropriate dietary strategies for weight control. We recognize the necessity for education initiatives and strongly encourage them to assist individuals in managing diabetes resulting from obesity.

Background: Urinary tract infections caused by bacteria are the most prevalent type of hospital-acquired infection. They are also responsible for the greatest number of patient fatalities, lengths of hospital stays, and overall costs associated with medical care. Even though Escherichia coli is the bacteria that is found the most frequently, several studies have found different levels of prevalence for it. The current dilemma with antibiotic resistance is being caused, in large part, by the inappropriate and excessive use of antibiotics. Methodology: Laboratory records were searched in order to obtain information regarding the organisms that were isolated from urine samples as well as the pattern of antibiotic susceptibility. The urine samples that were obtained were processed according to the normal procedures, and a Kirby-Bauer Disk Diffusion test was used to determine the antibiotic susceptibility of the bacteria. Results: E. coli was found to be the most frequent causative agent of UTI (40.6%) followed by candida spp. (28%), Enterococcus spp (10.6%), Klebsiella spp (9.8%), Staphylococcus aures (1.58%) and Acinatobacter spp (1.97%). E. coli showed variable antimicrobial resistance to various antibiotics such as AMP (95.6%) followed by CRO (91.5%), CAZ (86.5%), CTX (82.09%), and MXF (80.8%) while most sensitive antibiotic was fosfomycin (93.2%) followed by sulzone (83.5%), imepinem (78.1%) and amikacin (77.1%). Keywords: Pathogens, susceptibility pattern, urinary tract infection

Osteoarthritis (OA) of the knee is a relatively prevalent disorder, and prevalence rises with age. Total knee replacement surgery (TKA) is a frequent operation that has significantly improved the quality of life for millions of people with symptomatic knee OA. TKA postoperative care has been discussed regarding the utility of CPM; it has been shown to speed up the recovery of full range of motion, shorten hospital stays, lessen discomfort, speed up wound healing, and lower the risk of DVT. The current study was conducted to compare Effectiveness of CPM protocol as an adjunct to Standard Physiotherapy protocol in Post-operative Rehabilitation of TKA cases. From July to December 2021, three affiliated hospitals hosted the randomised controlled study, which included patients who had complete knee arthroplasty surgery. After surgery, patients were randomly allocated to one of two groups: From the first surgical day till discharge, Group A got twice daily PT and continuous passive motion (CPM), while Group B only received the usual physiotherapy programme from the first postoperative day. Evaluation of the results took place the day after discharge. The participants in the 2 groups' baseline outcome measurement characteristics were comparable.

Background: Candida auris is a significant global health concern, due to its rapid transmission, high mortality rate, and resistance to commonly available antifungal drugs. Methodology: During the current study, a cationic polymeric hydrogel was developed using chitosan (CS), polyethylene glycol (PEG), and methacrylic acid (MAA). The respective solutions were mixed in a volumetric ratio of 2:1:1. After characterization, the hydrogel was assessed using antifungal, antibiofilm, and hemocompatibility assays. Results: The hydrodynamic radius of 554.7 ± 90.1 nm and zeta potential of 15.6 ± 1.09 mV indicate the ideal size and charge for topical applications and in vivo studies, respectively. The formulation exhibited improved thermal stability, enhanced swelling, and a drug release profile for non-Fickian diffusion. The hydrogel effectively inhibited fungal growth in agar plates (42 ± 7.31 mm zone of inhibition), with a mean IC50 of 15.17 ± 4.01 μg/mL and MIC of 29.30 ± 11.72 μg/mL. Calcofluor white (CFW) staining showed diffuse irregular yeast cells, suggesting increased membrane permeability, eventually leading to cell death. The hemocompatibility assay revealed no visible agglutination or hemolysis at the MIC value. The formulation exhibited significantly reduced biofilm formation compared to the growth control (p < 0.05). Additionally, in silico analysis revealed that MAA showed superior oral bioavailability, no inhibitory activity on cytochrome P450 enzymes, and low potential for toxicity through nuclear receptor signaling pathways. Conclusions: Cationic hydrogels show promise as potential antifungal treatments. The development of cost-effective and improved therapeutic methods is crucial to combat this deadly pathogen and to improve patient outcomes.
