Hurthle cell carcinoma makes up 3 to 5% of all thyroid cancers and is considered to be a true rarity. The aim of our study was to analyze clinical characteristics and survival rates of patients with Hurthle cell carcinoma. Clinical data regarding basic demographic characteristics, tumor grade, type of surgical treatment and vital status were collected. Methods of descriptive statistics and Kaplan-Meier survival curves were used for statistical analysis. Cox proportional hazards regression was used to identify independent predictors. During the period from 1995 to 2014, 239 patients with Hurthle cell carcinoma were treated at our Institution. The average age of the patients was 54.3, with female to male ratio of 3.6:1 and average tumor size was 41.8 mm. The overall recurrence rate was 12.1%, with average time for relapse of 90.74 months and average time without any signs of the disease of 222.4 months. Overall 5-year, 10-year and 20-year survival rates were 89.4%, 77.2%, 61.9% respectively. The 5-year, 10-year and 20-year cancer specific survival rates were 94.6%, 92.5%, 87.4%, respectively. When disease free interval was observed, 5-year, 10-year and 20-year rates were 91.1%, 86.2%, 68.5%, respectively. The affection of both thyroid lobes and the need for reoperation due to local relapse were unfavorable independent prognostic factors, while total thyroidectomy as primary procedure was favorable predictive factor for cancer specific survival. Hurthle cell carcinoma is a rare tumor with an encouraging prognosis and after adequate surgical treatment recurrences are rare.

Background: Burn wound dressing and debridement are excruciatingly painful procedures that call for appropriate analgesia—typically multimodal. Better post-procedural pain management, less opioid use, and consequently fewer side effects, which could prolong recovery and increase morbidity, are all benefits of this type of analgesia. Intravenously administered ketamine can be effective as monotherapy or in combination with opioids, especially with procedural sedation such as in burn wound dressing. Methods: This observational study investigated the effect of ketamine administered in subanesthetic doses combined with opioids during burn wound dressing. The study was conducted from October 2018 to October 2021. A total of 165 patients met the inclusion criteria. A total of 82 patients were in the ketamine group, while 83 patients were dressed without ketamine. The main outcome was the effect of ketamine on intraprocedural opioid consumption. The secondary outcome included the effect of ketamine on postprocedural pain control. Results: Patients dressed with ketamine were significantly older (p = 0.001), while the mean doses of intraoperatively administered propofol and fentanyl were significantly lower than in patients dressed without ketamine (150 vs. 220 mg, p < 0.001; and 0.075 vs. 0.150 mg, p < 0.001; respectively). Conclusions: Ketamine was an independent predictor of lower intraoperative fentanyl consumption, according to the multivariate regression analysis (p = 0.015). Contrarily, both groups of patients required postoperative tramadol treatment, while intraoperative ketamine administration had no beneficial effects on postoperative pain management.
Background and Objectives: Lung cancer is the second most common form of cancer in the world for both men and women as well as the most common cause of cancer-related deaths worldwide. The aim of this study is to summarize the radiological characteristics between primary lung adenocarcinoma subtypes and to correlate them with FDG uptake on PET-CT. Materials and Methods: This retrospective study included 102 patients with pathohistologically confirmed lung adenocarcinoma. A PET-CT examination was performed on some of the patients and the values of SUVmax were also correlated with the histological and morphological characteristics of the masses in the lungs. Results: The results of this analysis showed that the mean size of AIS-MIA (adenocarcinoma in situ and minimally invasive adenocarcinoma) cancer was significantly lower than for all other cancer types, while the mean size of the acinar cancer was smaller than in the solid type of cancer. Metastases were significantly more frequent in solid adenocarcinoma than in acinar, lepidic, and AIS-MIA cancer subtypes. The maximum standardized FDG uptake was significantly lower in AIS-MIA than in all other cancer types and in the acinar predominant subtype compared to solid cancer. Papillary predominant adenocarcinoma had higher odds of developing contralateral lymph node involvement compared to other types. Solid adenocarcinoma was associated with higher odds of having metastases and with higher SUVmax. AIS-MIA was associated with lower odds of one unit increase in tumor size and ipsilateral lymph node involvement. Conclusions: The correlation between histopathological and radiological findings is crucial for accurate diagnosis and staging. By integrating both sets of data, clinicians can enhance diagnostic accuracy and determine the optimal treatment plan.
Background: Lung cancer remains the leading cause of cancer-related mortality worldwide, with non-small cell lung cancer (NSCLC) accounting for the majority of cases. Radiomics and artificial intelligence (AI) have emerged as promising tools for quantitative imaging analysis and precision staging. This study aimed to evaluate the ability of an AI-based radiomics model to preoperatively predict tumor (T) and nodal (N) stage, lymphovascular invasion (LVI), and postoperative complications in patients with early-stage NSCLC. Material and Methods: This retrospective study included 51 consecutive patients who underwent anatomical lobectomy with systematic lymph node dissection between 2019 and 2024, at the Clinic for Thoracic Surgery of the University Clinical Center of Serbia. Quantitative imaging features were extracted from preoperative CT scans using the Lesion Scout with Auto ID module (syngo.via VB50 MM, Siemens Healthineers). Radiomics and clinical predictors were analyzed using regularized logistic regression (LASSO) with five-fold cross-validation. Model performance was assessed using AUC, accuracy, sensitivity, specificity, precision, and F1 score, and calibration was evaluated using the Hosmer–Lemeshow test. Groups were compared using parametric and non-parametric tests. Correlation between the variables was assessed using Spearman’s rank correlation coefficient. All p-values less than 0.05 were considered significant. Results: The AI-based model showed excellent performance for predicting the T component (training AUC = 0.89; test AUC = 0.86; F1 = 0.81) and acceptable calibration (p = 0.41). Nodal metastasis (OR = 0.108; 95% CI: 0.011–1.069; p = 0.057) and LVI (OR = 0.519; 95% CI: 0.139–1.937; p = 0.329) were not significantly predicted. Emphysema was identified as a significant independent predictor of postoperative complications (χ2 = 5.13; p = 0.024). Conclusions: The AI-driven radiomics model demonstrated strong predictive ability for the T component and identified emphysema as a clinically relevant predictor of postoperative complications.

Introduction: Pleurocutaneous fistula is a pathological communication of subcutaneous tissue with the pleural cavity, and can occur as a result of infectious, malignant processes and iatrogenic procedures. Colopleural fistula is rare and is mainly caused by processes in the abdomen. The appearance of empyema is usually described as a complication of colopleural fistulas that are the result of pathological processes in the abdomen. Case Presentation: We report an extremely rare case of colopleurocutaneous fistula with pleural empyema present, 8 years after left hemicolectomy due to colon adenocarcinoma. Radiological diagnostic procedures performed confirmed the existence of colopleurocutaneous fistula. The patient was given antibiogram therapy and regular thoracocentesis for empyema, as well as a fistulous canal toilet. Conclusions: After achieving sterility of the fistulous canal and regression of empyema, the fistulous canal was closed with fibrin glue, and during the next 6 months follow-up, there was no reopening of the fistula, nor did the patient have any other complications.
