Introduction: Cardiovascular disease (CVD) remains a leading cause of death. Autoimmune patients face heightened CVD risk due to chronic inflammation and immune dysregulation. This systematic review and meta-analysis aim to synthesize current evidence on the predictive value of advanced or novel autoimmune biomarkers for the occurrence of CVD in middle-aged patients with autoimmune diseases and without cardiovascular history or symptoms. Methods: Abstract was registered prospectively (PROSPERO CRD42024611894) and conducted an advanced, MeSH-based search (2004–2025) for studies on autoimmune diseases in adults (18–65) without prior CVD, in various databases. Pooled adjusted hazard ratios were generated using Stata 18, assessing heterogeneity (Cochran's Q, I2), publication bias (funnel plot, Egger's test), and risk of bias (ROBINS-I), with sensitivity analysis performed to confirm robustness.Result: A comprehensive search in PubMed, Embase, and Medline yielded 3,975 records (after removing 237 duplicates), and after screening 2,488 titles/abstracts and 896 full texts, 69 studies (34 for meta-analysis) with 46,493 participants were included after excluding 188 with pre-existing CVD and 117 with insufficient data. High-sensitivity C-reactive protein (hs-CRP) was consistently associated with elevated CVD risk despite high heterogeneity and potential publication bias. Similarly, lupus anticoagulant, sVCAM-1, and antiphospholipid antibodies demonstrated strong predictive associations. In contrast, rheumatoid factor, anti-CCP, ADMA, homocysteine, NT-proBNP, anti-dsDNA, and TNF-alpha showed borderline significance or inconsistent results. These findings underscore the potential of select inflammatory and immune markers for enhancing CVD risk stratification and guiding targeted prevention strategies. Conclusion: Integrating these biomarkers with traditional risk factors may enable early detection of subclinical atherosclerosis in autoimmune patients, pending further research.Systematic Review Registration: https://www.crd.york.ac.uk/PROSPERO/view/CRD42024611894, identifier CRD42024611894.

Background: Multiple coronary guidance strategies including angiography, physiology-based assessment, and intracoronary imaging are used to optimize percutaneous coronary intervention, yet their comparative effectiveness across clinical outcomes remains uncertain. Methods: A comprehensive network meta-analysis incorporated fifty randomized studies evaluating angiography, FFR, iFR, IVUS, and OCT. The primary outcome was major adverse cardiovascular events (MACE). Secondary outcomes included all-cause mortality, cardiac death, myocardial infarction, stent thrombosis, target lesion revascularization, and target vessel revascularization. Random effects models were applied and interventions were ranked using SUCRA. Results: A total of 50 studies involving 39,863 patients were included, of whom 29,571 were male and 10,031 were female. Across guidance modalities, 15,463 patients underwent angiography-guided PCI, 10,728 IVUS-guided, 6,001 FFR-guided, 3,512 iFR-guided, and 3,849 OCT-guided PCI. In the network meta-analysis, intravascular imaging strategies demonstrated favorable outcomes across evaluated endpoints. Compared with IVUS, angiography-guided PCI was associated with higher rates of major adverse cardiovascular events (RR 1.28, 95% CI 1.13–1.46), all-cause mortality (RR 1.30, 95% CI 0.98–1.63), myocardial infarction (RR 1.73, 95% CI 1.28–2.40), target lesion failure (RR 1.50, 95% CI 1.19–1.93), and stent thrombosis (RR 1.80, 95% CI 1.25–2.70). Physiology-guided PCI using iFR was associated with higher risk estimates for all-cause mortality (RR 1.72, 95% CI 1.06–2.79) and cardiac death (RR 2.21, 95% CI 1.24–4.24) compared with IVUS. OCT demonstrated outcomes comparable to IVUS, with no statistically significant differences in major adverse cardiovascular events (RR 1.00, 95% CI 0.80–1.28) or cardiac death (RR 0.86, 95% CI 0.47–1.59). Sensitivity analyses yielded similar estimates. Overall, probabilistic ranking analyses favored intravascular imaging strategies, although effect estimates among non-angiographic modalities overlapped. Conclusions: Advanced PCI guidance strategies using intravascular imaging or invasive physiological assessment are associated with improved clinical outcomes compared with angiography alone. However, no single non-angiographic modality demonstrates definitive superiority, supporting individualized selection of guidance strategies based on clinical and procedural context. Paroxysmal supraventricular tachycardia (PSVT) describes the narrow zone of tachycardia. PSVT can diagnose with electrocardiography, but most specifically, it can be diagnosed by electrophysiology study (EPS) and further treated with radiofrequency ablation (RFA). Management of PSVT with RFA significantly reduces the reoccurrence of PSVT episodes in future. In the following case, presentation series of three cases will be used for creating panorama of existing knowledge of PSVT, along with diagnostic approach through EPS, and finally, the use of RFA as a treatment method will be demonstrated. Furthermore, development of patient-oriented concepts of PSVT management will be discussed.

An excess build-up of lipids in the arterial wall might result into Atherosclerosis. Lipophagy is the autophagic degradation of lipids that regulates the lipid metabolism in various kinds of cells. Lipophagy replaces intracellular lipid which makes it vital for development and progression of atherosclerosis. This review focuses on advances in lipid metabolism through lipophagy. The role of lipophagy in vascular endothelial cell injury, macrophage lipid accumulation and vascular smooth muscle cells phenotypic shift has been explained by specifying the lipophagy– atherosclerosis relationship. Novel therapeutic choices can be discovered by understanding the significance of lipophagy in these processes which could be a breakthrough in treatment of atherosclerosis.
