Background: Fetal heart rate (FHR) monitoring is a screening tool used by obstetricians to evaluate the fetal state. Because of the complexity and non-linearity, a visual interpretation of FHR signals using common guidelines usually results in significant subjective inter-observer and intra-observer variability. Objective: Therefore, computer aided diagnosis (CAD) systems based on advanced artificial intelligence (AI) technology have recently been developed to assist obstetricians in making objective medical decisions. Methods: In this work, we present an 8-layer deep convolutional neural network (CNN) framework to automatically predict fetal acidemia. After signal preprocessing, the input 2-dimensional (2D) images are obtained using the continuous wavelet transform (CWT), which provides a better way to observe and capture the hidden characteristic information of the FHR signals in both the time and frequency domains. Unlike the conventional machine learning (ML) approaches, this work does not require the execution of complex feature engineering, i.e., feature extraction and selection. In fact, 2D CNN model can self-learn useful features from the input data with the prerequisite of not losing informative features, representing the tremendous advantage of deep learning (DL) over ML. Results: Based on the test open-access database (CTU-UHB), after comprehensive experimentation, we achieved better classification performance using the optimal CNN configuration compared to other state-of-the-art methods: the averaged ten-fold cross-validation of the accuracy, sensitivity, specificity, quality index defined as the geometric mean of the sensitivity and specificity, and the area under the curve yielded results of 98.34, 98.22, 94.87, 96.53 and 97.82%, respectively Conclusions: Once the proposed CNN model is successfully trained, the corresponding CAD system can be served as an effective tool to predict fetal asphyxia objectively and accurately.

For both the acquisition of mobile electrocardiogram(ECG) devices and early warning and diagnosis of clinical work, high-quality ECG signals is particularly important. We describe an effective system which could be deployed as a stand-alone signal quality assessment algorithm for vetting the quality of ECG signals. The proposed ECG quality assessment method is based on the simple heuristic fusion and fuzzy comprehensive evaluation of the SQIs. This method includes two modules, i.e., the quantification and extraction of Signal Quality Indexes (SQIs) for different features, intelligent assessment and classification. First, simple heuristic fusion is executed to extract SQIs and determine the following SQIs: R peak detection match qSQI, QRS wave power spectrum distribution pSQI, kurtosis kSQI and baseline relative power basSQI. Then, combined with Cauchy distribution, rectangular distribution and trapezoidal distribution, the membership function of SQIs was quantified, and the fuzzy vector was established. The bounded operator was selected for fuzzy synthesis, and the weighted membership function was used to perform the assessment and classification. The performance of the proposed method was tested on the database from Physionet ECG database, with an accuracy (Acc) of 97.67%, sensitivity (Se) of 96.33% and specificity (Sp) of 98.33% on the training set. Testing against the test datasets resulted in scores of 94.67%, 90.33% and 93.00%, respectively. There's no gold standard exists for determining the quality of ECGs. However, the proposed algorithm discriminates between high- and poor-quality ECGs, which could aid in ECG acquisition for mobile ECG devices, early clinical diagnosis and early warning.

In this paper, a human electrocardiogram (ECG) identification system based on ensemble empirical mode decomposition (EEMD) is designed. A robust preprocessing method comprising noise elimination, heartbeat normalization and quality measurement is proposed to eliminate the effects of noise and heart rate variability. The system is independent of the heart rate. The ECG signal is decomposed into a number of intrinsic mode functions (IMFs) and Welch spectral analysis is used to extract the significant heartbeat signal features. Principal component analysis is used reduce the dimensionality of the feature space, and the K-nearest neighbors (K-NN) method is applied as the classifier tool. The proposed human ECG identification system was tested on standard MIT-BIH ECG databases: the ST change database, the long-term ST database, and the PTB database. The system achieved an identification accuracy of 95% for 90 subjects, demonstrating the effectiveness of the proposed method in terms of accuracy and robustness.

Background: Electronic fetal monitoring (EFM) is widely applied as a routine diagnostic tool by clinicians using fetal heart rate (FHR) signals to prevent fetal hypoxia. However, visual interpretation of the FHR usually leads to significant inter-observer and intra-observer variability, and false positives become the main cause of unnecessary cesarean sections. Goal: The main aim of this study was to ensure a novel, consistent, robust, and effective model for fetal hypoxia detection. Methods: In this work, we proposed a novel computer-aided diagnosis (CAD) system integrated with an advanced deep learning (DL) algorithm. For a 1-dimensional preprocessed FHR signal, the 2-dimensional image was transformed using recurrence plot (RP), which is considered to greatly capture the non-linear characteristics. The ultimate image dataset was enriched by changing several parameters of the RP and was then used to feed the convolutional neural network (CNN). Compared to conventional machine learning (ML) methods, a CNN can self-learn useful features from the input data and does not perform complex manual feature engineering (i.e., feature extraction and selection). Results: Finally, according to the optimization experiment, the CNN model obtained the average performance using optimal configuration across 10-fold: accuracy = 98.69%, sensitivity = 99.29%, specificity = 98.10%, and area under the curve = 98.70%. Conclusion: To the best of our knowledge, this approached achieved better classification performance in predicting fetal hypoxia using FHR signals compared to the other state-of-the-art works. Significance: In summary, the satisfied result proved the effectiveness of our proposed CAD system for assisting obstetricians making objective and accurate medical decisions based on RP and powerful CNN algorithm.

To further prolong the lifetime of wireless sensor network (WSN), researchers from various countries have proposed many clustering routing protocols. However, the total network energy consumption of most protocols is not well minimized and balanced. To alleviate this problem, this paper proposes an energy-efficient clustering routing protocol in WSNs. To begin with, this paper introduces a new network structure model and combines the original energy consumption model to construct a new method to determine the optimal number of clusters for the total energy consumption minimization. Based on the balanced energy consumption, then we optimize the AGglomerative NESting (AGNES) algorithm, including: (1) introduction of distance variance, (2) the dual-cluster heads (D-CHs) division of the energy balance strategy, and (3) the node dormancy mechanism. In addition, the CHs priority function is constructed based on the residual energy and position of the node. Finally, we simulated this protocol in homogeneous networks (the initial energy = 0.4 J, 0.6 J and 0.8 J) and heterogeneous networks (the initial energy = 0.4–0.8 J). Simulation results show that our proposed protocol can reduce the network energy consumption decay rate, prolong the network lifetime, and improve the network throughput in the above two networks.
