Objectives: The Janus kinase (JAK) inhibitor baricitinib may block viral entry into pneumocytes and prevent cytokine storm in patients with SARS-CoV-2 pneumonia. We aimed to assess whether baricitinib improved pulmonary function in patients treated with high-dose corticosteroids for moderate to severe SARS-CoV-2 pneumonia. Methods: This observational study enrolled patients with moderate to severe SARS-CoV-2 pneumonia [arterial oxygen partial pressure (PaO2)/fraction of inspired oxygen (FiO2) <200 mmHg] who received lopinavir/ritonavir and HCQ plus either corticosteroids (CS group, n = 50) or corticosteroids and baricitinib (BCT-CS group, n = 62). The primary end point was the change in oxygen saturation as measured by pulse oximetry (SpO2)/FiO2 from hospitalization to discharge. Secondary end points included the proportion of patients requiring supplemental oxygen at discharge and 1 month later. Statistics were adjusted by the inverse propensity score weighting (IPSW).Results: A greater improvement in SpO2/FiO2 from hospitalization to discharge was observed in the BCT-CS vs CS group (mean differences adjusted for IPSW, 49; 95% CI: 22, 77; P < 0.001). A higher proportion of patients required supplemental oxygen both at discharge (62.0% vs 25.8%; reduction of the risk by 82%, OR adjusted for IPSW, 0.18; 95% CI: 0.08, 0.43; P < 0.001) and 1 month later (28.0% vs 12.9%, reduction of the risk by 69%, OR adjusted for IPSW, 0.31; 95% CI: 0.11, 0.86; P = 0.024) in the CS vs BCT-CS group. Conclusions: . In patients with moderate to severe SARS-CoV-2 pneumonia a combination of baricitinib with corticosteroids was associated with greater improvement in pulmonary function when compared with corticosteroids alone.Trial registration: European Network of Centres for Pharmacoepidemiology and Pharmacovigilance, ENCEPP (EUPAS34966, http://www.encepp.eu/encepp/viewResource.htm? id = 34967)

Over the last few years, the development of nanotechnology has allowed for the synthesis of many different nanostructures with controlled sizes, shapes, and chemical properties, with dendrimers being the best-characterized of them. In this review, we present a succinct view of the structure and the synthetic procedures used for dendrimer synthesis, as well as the cellular uptake mechanisms used by these nanoparticles to gain access to the cell. In addition, the manuscript reviews the reported in vivo applications of dendrimers as drug carriers for drugs used in the treatment of cancer, neurodegenerative diseases, infections, and ocular diseases. The dendrimer-based formulations that have reached different phases of clinical trials, including safety and pharmacokinetic studies, or as delivery agents for therapeutic compounds are also presented. The continuous development of nanotechnology which makes it possible to produce increasingly sophisticated and complex dendrimers indicates that this fascinating family of nanoparticles has a wide potential in the pharmaceutical industry, especially for applications in drug delivery systems, and that the number of dendrimer-based compounds entering clinical trials will markedly increase during the coming years.

Hodgkin's disease was diagnosed in a 22-year-old HTV-seropositive man in 1986. Alternate MOPP/ ABVD chemotherapy induced a clinical remission. He was asymptomatic until 3 years later when fever and peripheral and mediastinal lymphadenopathy appeared. Lymph node biopsy showed a large-cell anaplastic lymphoma and EBV genome was identified in the malignant cells, suggesting that transformation might had been induced by EBV. The present case affirms that in patients with HIV-related lymphomas who present enlarging lymphadenopathy after stable remission, the development of lymphomas of higher malignancy needs to be ruled out.

Objective Serum antibody levels have been linked to immune protection in SARS-CoV-2 infection. After exposure to the virus and/or vaccination there is an increase in serum antibody titers followed by progressive non-linear waning of antibody levels. Our aim was to find out if this waning of antibody titers would adjust to a mathematical model. Methods We studied serum anti-RBD (receptor binding domain) IgG antibody titers over a ten-month period in a cohort of 54 health-care workers who were either never infected with SARS-CoV-2 (naive, nHCWs group, n = 27) or previously infected (experienced, eHCWs group, n = 27) with the virus after the second dose of the BNT162b2 mRNA vaccine. We have selected a risk threshold of 1000 UA/ml anti-RBD Ab titer for symptomatic infection based on the upper titer threshold for those volunteers who suffered infection prior to the omicron outbreak. Two mathematical models, exponential and potential, were used to quantify antibody waning kinetics and the relative quality of the goodness of fit to the data between both models was compared using the Akaike Information Criterion. Results We found that the waning of anti-RBD IgG antibody levels adjusted significantly to both exponential and potential models in all participants from both the naïve and experienced groups. Moreover, the waning slopes were significantly more pronounced for the naive when compared to the experienced health-care workers. In the nHCWs group, titers would descend below this 1000-units threshold at a median of 210.6 days (IQ range: 74,2). However, for the eHCWs group, the risk threshold would be reached at 440.0 days (IQ range: 135,2) post-vaccination. Conclusions The goodness of the fit of the anti-RBD IgG antibody waning would allow us to predict when the antibody titers would fall below an established threshold in both naive and previously infected subjects.
