Nesting positions are commonly used in procedural analgesic administration in premature neonates. The effectiveness of nesting positions is questioned. The aim of the this study was to assess the pain, stress, comfort and salivary cortisol and melatonin values in nesting positions during the heel lance procedure in premature infants at the NICU. Experimental research; repeated measurement design. The sample comprised 33 premature neonates with gestational age of 31-35 weeks who had been hospitalized in the NICU. Nesting positions were given using linen or towels. The procedure of heel lance was recorded on camera. The camera recordings were evaluated according to the NIPS and the COMFORTneo scale. Saliva samples were obtained five minutes prior to and 30 min after the heel lance procedure. Salivary Cortisol and Melatonin were measured using the Salimetrics Cortisol Elisa Kit and the Salimetrics Melatonin Elisa Kit. The crying time, the mean NIPS score, the COMFORTneo score, the COMFORTneo NRS-pain scores and the COMFORTneo NRS-distress scores for premature neonates who were in the prone position during the procedure were significantly lower than the scores in the supine position (p < 0.000). Furthermore, the level of salivary cortisol five minutes prior to and 30 min after the heel lance procedure had significantly decreased in the prone position; however, there were insignificant differences in the mean levels of salivary melatonin between the positions. Nesting in the prone position has a pain reducing effect, enhancing comfort and reducing stress in premature infants.

In the COVID-19 pandemic period, most students felt lonely, accordingly their motivation for self-learning was low. The study aimed to increase their enthusiasm for work, and to improve their learning of biochemical subjects in the medical education programme during the pandemic period. Small parts of educational materials (microlearning) were presented in different formats such as videos, interactive education exercises, game-based quizzes and final exam was taken. The achievement of learning objectives of the course was evaluated by a survey. Students participating in daily activities gained higher self-confidence in learning and their final exam grade was higher compared to non-participating students (7.35 ± 1.37 vs 7.06 ± 1.88, p = 0.038). Most of the students agreed that daily activities (52%) increased their motivation to work. This approach might be proposed as a practice that supports distance learning, or even traditional education, especially in medical education that includes complex issues.

Recently, digital technology and digital materials have started to be widely used in education from primary school to college worldwide. Microlearning is one of the innovative teaching techniques that use digital technologies. In this review, benefits and disadvantages of microlearning is discussed. Many studies show that microlearning facilitated learning by dividing into smaller pieces encourages students to study. A wide range of activities might be used in this technique and it can be easily integrated into daily routine, it allows on-demand learning for the students. On the other hand, the success of microlearning techniques is closely related to the personal characteristics of learners, teachers’ prone to use digital technology and the external factors such as access to learning materials. Its effectiveness on behavior and outcome which were defined in the third and fourth levels of Kirkpatrick’s learning model is still obscure. In the light of the literature, it should be decided which microlearning method will be used for which educational subjects.

A correlation has been clearly shown between inflammation markers and subclinical atherosclerosis markers in the early stages of atherogenesis in subjects with familial hypercholesterolemia (FH). The aim of this study was to investigate potential inflammation markers in the diagnosis of atherosclerosis in children with FH. A total of 48 dyslipidemic children and 24 healthy age-matched control subjects were taken into study. Inflammation and macrophage activation markers (hsCRP, myeloperoxidase, chitotriosidase, YKL-40, TNF-α, IL-6, IL-18, MMP-1 and MMP-9) and lipid parameters of all patients were measured. Carotid intima-media thickness (cIMT) and flow-mediated dilation (FMD) levels were determined. Our data suggested that clinically evidenced (by cIMT and FMD levels) atherosclerosis starts in the early ages in hypercholesterolemic children. Higher cholesterol levels strongly correlated with macrophage activation markers (ChT, YKL-40 and myeloperoxidase). ChT and YKL-40 seem to be the more predictable markers of atherosclerosis even in early ages (<6 years old) than other classical inflammation markers such as hs-CRP, IL-6 and TNF-α.
