Objectives: Hepatocellular carcinoma (HCC) remains a major indication for liver transplantation (LT), but accurate pretransplant risk stratification is critical to improve long-term outcomes. Traditional morphometric criteria such as tumor size and number are limited in predicting recurrence and survival. The HALP (hemoglobin, albumin, lymphocyte, platelet), gamma-glutamyl transpeptidase to platelet ratio (GPR), and FIB-4 indices are emerging systemic inflammatory and nutritional biomarkers that may provide additional prognostic value in HCC patients undergoing LT. Materials and Methods: This retrospective, two-center cohort study included 200 patients who underwent LT for HCC between 2012 and 2023. Preoperative HALP, GPR, and FIB-4 scores were calculated, and their associations with overall survival (OS) and recurrence-free survival (RFS) were assessed using ROC analyses and Cox proportional hazard models. Cut-off values were determined for each biomarker, and survival outcomes were analyzed using Kaplan–Meier methods. Results: A low HALP score (≤0.39) was independently associated with reduced OS but not with RFS. Conversely, low GPR (≤0.45) and FIB-4 (≤3.1) values were significantly associated with both poor OS and higher recurrence risk. Tumor size, number of lesions, and microvascular invasion also independently predicted poor outcomes. Multivariate analysis confirmed HALP, GPR, and FIB-4 as significant preoperative predictors of prognosis in this population. Conclusions: HALP, GPR, and FIB-4 are readily available, cost-effective indices that provide significant prognostic information in HCC patients undergoing LT. Their integration with morphometric criteria may improve pretransplant risk stratification and support individualized clinical decision-making.

Background and Objectives: Erdosteine (Erd) is an antioxidant and anti-inflammatory drug. Vitamin B has been reported to exert anti-inflammatory and antioxidant effects. In this study, we investigated the effect of erdosteine and vitamin B complex on a liver ischemia/reperfusion (I/R) model. Materials and Methods: Thirty-two Wistar Albino male rats weighing 350–400 g were used. The animals were randomly selected and divided into four groups. The groups are as follows: first group (Sham), second group (I/R), third group (I/R + vit B), and fourth group (I/R + vit B + Erd). Rats were subjected to 45 min of hepatic ischemia, followed by a 45 min reperfusion period in the I/R and Vitamin B + Erd groups. An amount of 150 mg/kg/day of erdosteine was given orally for 2 days, and 0.05 mL/kg of i.p. vitamin B complex was given 30 min before the reperfusion. Serum biochemical parameters were measured. Serum Total Antioxidant Status (TAS) and Total Oxidant Status (TOS) were measured, and the Oxidative Stress Index (OSI) was calculated. Hepatic tissue samples were taken for the evaluation of histopathological features. Results: In terms of all histopathological parameters, there were significant differences in the I/R + vit B group and I/R + vit B + Erd group compared with the I/R group (p < 0.01). In terms of aspartate aminotransferase (AST), alanine aminotransferase (ALT), lactate dehydrogenase (LDH), TNF-alpha, and IL-6 levels, there were significant differences between the I/R group and treatment groups (p < 0.01). The lowest TOS and OSI levels were obtained in the treatment groups, and these groups had statistically significantly higher TAS levels compared with the sham and I/R groups (p < 0.01). Conclusions: As a preliminary experimental study, our study suggests that these agents may have potential diagnostic and therapeutic implications for both ischemic conditions and liver-related diseases. These results suggest that the combination of vit B + Erd may be used to protect against the devastating effects of I/R injury. Our study needs to be confirmed by clinical studies with large participation.

Objectives: Systemic inflammation, coagulopathy, and multiorgan dysfunction are common in critically ill patients and contribute significantly to mortality. Serum albumin and D-dimer are routinely used biomarkers that reflect nutritional status and coagulation activity, respectively. This study aimed to investigate the prognostic value of the albumin-to-D-dimer ratio (ADR) in predicting 30-day mortality among patients admitted to the intensive care unit (ICU) and undergoing mechanical ventilation. Methods: This retrospective cohort study included 162 adult patients who underwent invasive mechanical ventilation in the ICU of a tertiary care center between January 2021 and December 2023. Demographic data, comorbidities, and laboratory values—such as serum albumin, D-dimer, lactate, CRP, BUN, creatinine, INR, and platelet count—were recorded within the first 24 h of ICU admission. The albumin-to-D-dimer ratio (ADR) was calculated by dividing serum albumin (g/dL) by D-dimer (μg/mL). The patients were stratified into tertiles based on ADR values: low (1.45). The association between the ADR and 30-day mortality was analyzed using multivariate logistic regression and receiver operating characteristic (ROC) curve analysis. Results: Of the 162 patients included in the study, 61 (37.7%) died within 30 days. The patients who died had significantly lower ADR values at ICU admission compared to survivors (1.02 ± 0.43 vs. 1.56 ± 0.52, p < 0.001). In the multivariate logistic regression model, a lower ADR remained an independent predictor of 30-day mortality (OR: 0.39; 95% CI: 0.26–0.58; p < 0.001), even after adjusting for age, lactate, creatinine, INR, and other relevant clinical variables. ROC curve analysis demonstrated that the ADR had the highest discriminative performance among all the evaluated parameters, with an AUC of 0.802 (95% CI: 0.728–0.875; p < 0.001). The optimal cut-off value for the ADR was identified as <1.05, yielding a sensitivity of 78.7% and a specificity of 71.4% in predicting 30-day mortality. Conclusions: The ADR is independently associated with 30-day mortality in mechanically ventilated ICU patients and may serve as a useful early prognostic marker. However, given the retrospective, single-center nature of this study, these findings should be interpreted with caution. Further prospective, multicenter studies are needed to validate the clinical utility of the ADR.

Background and Objectives: It is known that critical illness and associated neuromuscular problems begin to appear in patients hospitalized in the intensive care unit (ICU) for more than a week. The goal of this study was to research the role of hydroxychloroquine (HCQ) in the treatment of cytokine storm and critical illness neuromyopathy (CINM) in a rat sepsis model. Materials and Methods: Rats were assigned into three groups, and a feces intraperitoneal-injection group (FIP) procedure was carried out on 30 rats to induce a model of sepsis for critical illness polyneuromyopathy (CINM). The study groups were as follows: Group 1: control (nonoperative and orally fed control, n = 10), Group 2: FIP with 0.9% NaCl saline was given as 1 mL/kg/day by oral gavage (n = 10), and Group 3: FIP with 10 mg/kg/day of hydroxychloroquine (Plaquenil 200 mg) administered by oral gavage (n = 10). Electrophysiological recordings (EMG) were conducted six days after surgery. EMG was carried out three times on the right sciatic nerve, which was stimulated with supramaximal intensity utilizing a bipolar needle electrode at the sciatic notch. Tumor necrosis factor-alpha (TNF-α), malondialdehyde (MDA), lactic acid levels, and interleukin-6 (IL-6) were evaluated. Results: In terms of TNF-α, MDA, lactic acid levels, and IL-6, there was a statistically significant decrease in the CINM + 10 mg/kg HCQ group compared to the CINM and saline group (p < 0.0001, p < 0.05, p < 0.05, and p < 0.05, respectively). Compound muscle action potentials (CMAPs) latency and duration were decreased in the CINM + 10 mg/kg HCQ group compared to other groups (p < 0.01 and p < 0.001). However, CMAP amplitude was significantly higher in the CINM + 10 mg/kg HCQ group unlike the CINM and saline group (p < 0.001). Conclusions: This is the first study to demonstrate the effects of HCQ on CINM in a rat model of sepsis. The findings of our research suggest that hydroxychloroquine may be used as a potential therapeutic agent in the treatment of sepsis. Hydroxychloroquine may have an important effect in the pathogenesis of sepsis-associated CINM by reducing cytokine production and oxidative stress.

Background and Objectives: Acute kidney injury (AKI) is common in intensive-care unit (ICU) patients and is associated with increased mortality. Elderly patients tend to have more comorbid chronic diseases and are more prone to AKI than younger populations, resulting in higher rates of hospitalization and a higher incidence of AKI. Our aim in this study was to investigate the prognostic utility of BUN/albumin ratio (BAR) in predicting mortality in elderly critically ill patients with AKI. Materials and Methods: This study was conducted retrospectively on 154 elderly patients with AKI who were admitted to the ICU between October 2023 and September 2024.Data on the following demographic, clinical, and laboratory parameters were retrospectively collected from medical cards and electronic records. Results: In the non-survivor group, among comorbidities, lung disease was higher (p < 0.05), GCS was lower, and APACHE II was higher among clinical scores (p < 0.001). In the non-survivor group, diuretic use (p = 0.03), oliguria, RRT, vasopressor requirement, sepsis, and MV rates (p < 0.001),as well as BUN, phosphate, LDH, Crp, APTT, INR, and BAR rates, were higher (all p < 0.05) and albumin was lower (p = 0.01). Cut-off values of BUN, albumin, and BAR variables according to mortality status were determined by an ROC curve analysis, as follows:48.4 for BUN (p = 0.013), 31.5 for albumin (p = 0.001), and 1.507 for BAR (p = 0.001).According to the results of the ROC analysis performed to predict in-hospital mortality, the BAR level reached an AUC value of 0.655. A BAR value above 1.507 increases mortality by 3.944 times (p = 0.023). Conclusions: BAR is a simple and accessible biomarker that may serve as a predictor of in-hospital mortality in elderly patients with AKI. Its use may aid early risk stratification and decisionmaking in the ICU.

Aims: This study aimed to investigate whether the CALLY index is associated with mortality in patients admitted to intensive care due to decompensated heart failure. Methods: In this retrospective study, 130 patients diagnosed with decompensated heart failure admitted to the intensive care unit between 15 October 2023 and 15 October 2024 were included. Demographic data, comorbidities, stages of heart failure, length of stay in the intensive care unit, and mortality status were examined. CALLY index were calculated, and their relationship with mortality was examined. Results: The CALLY index was found to be statistically significant in distinguishing mortality (p = 0.018). A higher mortality rate was observed in the group with a low CALLY index (<=0.12) (% 26.9% vs % 10.3%) (p=0.013). Conclusion: In patients with decompensated heart failure followed in the intensive care unit, mortality was inversely related to the CALLY index. The CALLY index, a straightforward method for calculation, can serve as a guide for clinicians in determining prognosis.
