The gold standard test for diagnosing dysphagia is the videofluoroscopic swallowing study (VFSS). However, the accuracy of this test varies depending on the specialist’s skill level. We proposed a VFSS-based artificial intelligence (AI) web application to diagnose dysphagia. Video from the VFSS consists of multiframe data that contain approximately 300 images. To label the data, the server separated them into frames during the upload and stored them as a video for analysis. Then, the separated data were loaded into a labeling tool to perform the labeling. The labeled file was downloaded, and an AI model was developed by training with You Only Look Once (YOLOv7). Using a utility called SplitFolders, the entire dataset was divided according to a ratio of training (70%), test (10%), and validation (20%). When a VFSS video file was uploaded to an application equipped with the developed AI model, it was automatically classified and labeled as oral, pharyngeal, or esophageal. The dysphagia of a person was categorized as either penetration or aspiration, and the final analyzed result was displayed to the viewer. The following labeling datasets were created for the AI learning: oral (n = 2355), pharyngeal (n = 2338), esophageal (n = 1480), penetration (n = 1856), and aspiration (n = 1320); the learning results of the YOLO model, which analyzed dysphagia using the dataset, were predicted with accuracies of 0.90, 0.82, 0.79, 0.92, and 0.96, respectively. This is expected to help clinicians more efficiently suggest the proper dietary options for patients with oropharyngeal dysphagia.

Background: Videofluoroscopic swallow study (VFSS) is a key examination for assessing swallowing function. Although several artificial intelligence (AI) models for VFSS interpretation have shown high predictive accuracy through internal validations, AI models that have undergone external validation are rare. This study aims to develop an AI model that automatically diagnoses aspiration and penetration from VFSS videos and to evaluate the model’s performance through multicenter external validation. Methods: Among the 2343 VFSS videos collected, 309 cases of Q1-grade videos, which were free of artifacts and clearly showed the airway and vocal cords, were included in the internal validation dataset. The training, internal validation, and test datasets were divided in a 7:1:2 ratio, with 2012 images (aspiration = 532, penetration = 932, no airway invasion = 548) used for training. The AI model was developed and trained using You Only Look Once version 9, model c (YOLOv9_c). External validation of the AI model was conducted using 138 Q1 and Q2-grade VFSS videos from two different hospitals. Results: According to the internal validation, the YOLOv9_c model showed a training accuracy of 98.1%, a validation accuracy of 97.8%, and a test accuracy of 61.5%. From the confusion matrix analysis, the AI model’s diagnostic accuracy for aspiration in VFSS videos was 0.76 (AUC = 0.70), and for penetration, the diagnostic accuracy was 0.66 (AUC = 0.65). According to the external validation, the AI model demonstrated good performance in diagnosing aspiration (precision: 90.2%, AUC = 0.79) and penetration (precision: 78.3%, AUC = 0.80). The overall diagnostic accuracy of external validation for VFSS videos was 80.4%. Conclusions: We developed an AI model that automatically diagnoses aspiration and penetration when an entire VFSS video is input, and external validation showed good accuracy. In the future, to improve the performance of this AI model and facilitate its clinical application, research using training and validation with VFSS video data from more hospitals is needed.

In the advent of the 4th industrial revolution based on artificial intelligence, Internet of Things (IoT), 3D printer and other new technologies, the hyper-connected, super-intelligent society has been already arrived. The strong pervasive effect of the 4th industrial revolution have influenced on all the fields of the society. The tea culture that has long traditional history has also required to create a new direction to accommodate the drastically changed social environments. This paper analyzes the conceptual factors and then the essential functionalities of the tea culture. The paper shows that these innate functions of the tea culture can play a vital role for resolving the diverse side-effects occurred in the future hyper-connected, super-intelligent society. More ideal future society can be created by means of complementally harmonizing the tea culture which is the traditional analog culture with long history with the digital culture of 4th industrial revolution. According to the widespread 4th industrial revolution, the importance of the tea culture has been strongly emphasized and a new era of the tea culture is now opening.
