Objectives: Fournier’s gangrene is an aggressive, rapidly progressing, and life-threatening necrotizing fasciitis of the perineal and genital regions. Various scoring systems have been developed for predicting survival and prognosis in Fournier’s gangrene. This retrospective study aimed to evaluate the effectiveness of the newly developed Fournier’s gangrene mortality index (FGMI) in predicting mortality associated with Fournier’s gangrene. Methods: The study included patients over the age of 18 years who were followed-up with a diagnosis of Fournier’s gangrene in the general surgery clinics of three different hospitals in Şanlıurfa province between 2014 and 2024. The patients included in this study were divided into two groups: deceased (n = 20) and surviving (n = 149). In FGMI, the parameters used were age, creatinine level, albumin level, lymphocyte percentage, and neutrophil-to-lymphocyte ratio. Based on the total score and risk assessment, <5 points were categorized as low-to-moderate mortality risk and ≥5 points as high mortality risk. Results: A total of 169 patients with a diagnosis of Fournier’s gangrene were included in the study; 87 were men (51.48%). The median age of all patients was 53 (40–63) years; 20 patients (11.8%) died. The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score did not show a statistically significant difference between the deceased and surviving groups (p = 0.5). Compared to the survivors, the deceased had higher neutrophil counts, neutrophil percentages, neutrophil-to-lymphocyte ratios, platelet-to-lymphocyte ratios, and C-reactive protein-to-albumin ratios, whereas lymphocyte counts, lymphocyte percentages, eosinophil counts, eosinophil percentages, monocyte counts, and monocyte percentages were lower, and these differences were statistically significant. According to receiver operating characteristic (ROC) analysis, the ROC-area under the curve for predicting mortality based on an FGMI score of ≥5 was 0.88 (95% CI: 0.80–0.95) with a sensitivity of 90% and a specificity of 70% (p < 0.001). Univariate risk analysis was performed, and the odds ratio revealed that mortality risk in patients followed-up for Fournier’s gangrene with a FGMI score of ≥5 was 20 times higher (4.48–90.91) (p < 0.001). Conclusions: The results reveal that the FGMI score is a scoring system that can predict mortality at the initial clinical presentation of patients with Fournier’s gangrene. Another important finding of the present study is that the LRINEC score was not sufficiently effective in predicting mortality.

Background and Aim: Hydatid cyst is caused by Echinococcus larvae and prevalent in endemic areas worldwide. We analyzed post-procedure complications and outcomes of patients with liver hydatid cyst. Materials and Methods: We included patients who managed either by surgery or percutaneous drainage (PAIR) for hydatid liver cysts at SBÜ University Mehmet Akif İnan Hospital between January 2017 and February 2021. Age, sex, segmental location, size, number, Gharbi classification, treatment modality, length of hospital stays, and complications were recorded. Results: We included total of 209 patients who were managed by hydatid liver cysts. Among them, 74 post-procedural complications were developed in total of 69 (33%) patients. Biliary fistula was the most prevalent complication (n=38, 18.2%). Hospitalization duration was median 5 days (2-36) and was 5 days (2-36) in patients underwent surgery and 3 days (range:2-7) in patients managed by PAIR. Patients with cyst diameter ≥ 9.5 cm were predicted to have an increased risk of complications with 70% sensitivity and 60% specificity. More patients experienced any complications in patients who managed by PAIR than those underwent surgery (65.4% vs 28.4%, p

Hydatid cysts, caused by Echinococcus species, are zoonotic diseases frequently observed in endemic regions worldwide, including Turkey. It is most commonly localized in the liver, followed by the lungs. Anaphylaxis and peritonitis, which develop after cyst rupture, are the most significant complications observed in these patients. Early diagnosis and prompt surgical intervention are extremely important. This study presented an extremely rare case of a hepatic hydatid cyst ruptured into the thorax, which may have high morbidity and mortality. A 24-year-old male patient presented to our emergency department with cough, dyspnea, fever, and allergy symptoms. He had a history of hepatic hydatid cyst surgery 5 years ago. Physical examination revealed urticarial lesions on the skin. The body temperature was 39 °C, and the pulse rate was 120 beats/min. Laboratory tests revealed leukocytosis and elevated C-reactive protein. Contrast-enhanced thoracic and abdominal computed tomography was performed. Diagnostic tests revealed that the patient had a ruptured hepatic hydatid cyst in the right thoracic cavity; hence, he was scheduled for emergency surgery. Laparotomy was performed via a right subcostal incision, and the ruptured cyst was located. It was understood that this cyst had fistulized into the diaphragm during a chronic process and subsequently ruptured acutely into the thorax. Partial cystectomy was performed for a ruptured liver cyst. The diaphragm was repaired, and catheters were placed in the thorax and abdomen for drainage. There were no other postoperative complications. Rupture of hydatid cyst disease is frequently observed in endemic regions. The rupture of a hepatic hydatid cyst into the thorax is an extremely rare event. In patients presenting with pulmonary symptoms who have a history of hepatic hydatid cyst surgery, the abdomen should always be thoroughly evaluated, and emergency surgery should be performed if required.

Background: Hydatid cyst is a parasitic infection, often caused by Echinococcus granulosus. Although it is classified as a benign disease, cyst ruptures in the abdomen can be fatal. Ruptures occur spontaneously or after trauma. We aimed to report data from patients who underwent emergency surgery due to spontaneous intra-abdominal hydatid cyst rupture. Methods: Upon a retrospective review of the records at Department of General Surgery, Faculty of Medicine, Harran University, Şanlıurfa, Turkey, we found that 34 cases were operated on due to hydatid cyst rupture between January 2012 and October 2022. All patients were operated on in an emergency, and partial cystectomy, intra-abdominal irrigation, and drainage were performed using laparotomy. The patients were evaluated in terms of age, sex, symptoms, radiological findings, laboratory results, intraoperative findings, and postoperative follow-ups. Results: Twenty-two (64.7%) female and 12 (35.3%) male patients were enrolled. The mean age was 39.1 (±17.58) years. All patients experienced spontaneous rupture. The ruptured cyst was found in the liver in 32 patients (94%), the spleen in 1 patient (3%), and the pelvis in 1 patient (3%). The diagnosis was determined using ultrasonography in 12 (35.3%) patients, computed tomography in 21 (61.8%) patients, and magnetic resonance imaging in 1 (2.9%) patient. All patients exhibited acute abdomen and leukocytosis. The average length of hospital stay was 5.14 (±1.37) days. Conclusion: Hydatid cyst rupture should be considered in cases of acute abdomen, particularly in regions where the disease is endemic, as in our region. The ruptured hydatid cyst was primarily observed in the liver (94.11% of cases).

Background and Objectives: The aim of this study was to compare percutaneous aspiration injection reaspiration (PAIR), open surgery (OS), and laparoscopic surgery (LS) in the treatment of liver hydatid cysts in terms of effectiveness, complications, and recurrence rates. Materials and Methods: This retrospective cross-sectional study included 383 patients who were treated with a diagnosis of liver hydatid cyst at Harran University Faculty of Medicine between May 2014 and May 2024. Patients were divided into three groups based on the treatment method: PAIR, OS, and LS. The groups were analyzed in terms of demographic and clinical characteristics such as age, sex, number of cysts, cyst location, and cyst diameter. Various factors such as complications, recurrence rates, and biliary fistula development were compared. Statistical analyses were performed using Jamovi and JASP software, and p ≤ 0.05 was considered significant. Results: The risk of biliary fistula development was found to be significantly lower in patients treated using PAIR than in those in the surgical groups (p < 0.001). While the recurrence rate was higher in the PAIR group, the recurrence rates were similar in the OS and LS groups (p = 0.043). The risk of biliary fistula development and catheter removal time were found to be higher in patients with large cysts (p < 0.001). A strong and statistically significant correlation was observed between the length of hospital stay and the duration until catheter removal (p < 0.001). The maximum diameter of the cyst demonstrated a significant positive correlation with both the length of hospital stay (r = 0.363, p < 0.001) and the duration until catheter removal (p < 0.001). Conclusions: This study demonstrates that the PAIR method is effective in reducing biliary fistula development, but the recurrence rates are higher than OS and LS. OS and LS show similar outcomes in terms of recurrence.
