Background: The associations of waist circumference (WC), body mass index (BMI), lipid accumulation product (LAP), Chinese visceral adiposity index (CVAI), and triglyceride-glucose Index (TyG) with the risk of type 2 diabetes mellitus (T2DM) remained uncertain in Chinese middle-aged and elderly hypertensive patients. Methods: A total of 1,965 hypertensive participants aged 45 years and elderly were included in the cross-sectional analysis, and 1,576 hypertensive participants without T2DM for the cohort analysis. In the cross-sectional study, binary logistic regression, restricted cubic splines (RCS), and receiver operating characteristic (ROC) were used to analyze the relationships between WC, BMI, LAP, CVAI, and TyG with and T2DM in hypertensive patients. In the cohort study, Kaplan-Meier survival analyses and Cox regression were further performed to determine the associations of these indicators with incident T2DM risk. Results: In the cohort study, there were 101 incident T2DM cases occurred during a median follow-up of 30 months, with an incident rate was 2.78 per 100 person-years. The cross-sectional study showed that the risk of T2DM increased significantly with higher quartiles of WC, BMI, LAP, CVAI, and TyG (all P-trend < 0.001). In the cohort study, Cox regression model showed that WC (Q4 vs. Q1, HR = 3.30, 95% CI = 1.66-6.59), BMI (Q4 vs. Q1, HR = 2.38, 95% CI = 1.30-4.36), LAP (Q4 vs. Q1, HR = 5.15, 95% CI = 2.40-11.02), CVAI (Q4 vs. Q1, HR = 3.38, 95% CI = 1.76-6.50), and TyG (Q4 vs. Q1, HR = 5.76, 95% CI = 2.82-11.77) were associated with a higher risk of incident T2DM. RCS confirmed the positive dose-response relationships of WC, BMI, CVAI and TyG with T2DM in both study design, except for LAP in the cohort study. Additionally, ROC analysis revealed that TyG had the strongest area under the curve (AUC) of 0.70 (95% CI = 0.67-0.72) in the cross-sectional study, and the AUC of other indicators ranged from 0.55 to 0.57.Conclusion: Higher levels of WC, BMI, LAP, CVAI, and TyG are associated with a higher riskof developing incident T2DM in Chinese elderly hypertensive patients, and TyG might be the most effective predictive indicator.

A methadone maintenance treatment (MMT) program to curb the dual epidemics of HIV/AIDS and drug use has been administered by China since 2004. Little is known regarding the geographic heterogeneity of HIV and hepatitis C virus (HCV) infections among MMT clients in the resource-constrained context of Chinese provinces, such as Guangxi. This study aimed to characterize the geographic distribution patterns and co-clustered epidemic factors of HIV, HCV and co-infections at the county level among drug users receiving MMT in Guangxi Zhuang Autonomous Region, located in the southwestern border area of China. Baseline data on drug users’ demographic, behavioral and biological characteristics in the MMT clinics of Guangxi Zhuang Autonomous Region during the period of March 2004 to December 2014 were obtained from national HIV databases. Residential addresses were entered into a geographical information system (GIS) program and analyzed for spatial clustering of HIV, HCV and co-infections among MMT clients at the county level using geographic autocorrelation analysis and geographic scan statistics. A total of 31,015 MMT clients were analyzed, and the prevalence of HIV, HCV and co-infections were 13.05%, 72.51% and 11.96% respectively. Both the geographic autocorrelation analysis and geographic scan statistics showed that HIV, HCV and co-infections in Guangxi Zhuang Autonomous Region exhibited significant geographic clustering at the county level, and the Moran’s I values were 0.33, 0.41 and 0.30, respectively (P < 0.05). The most significant high-risk overlapping clusters for these infections were restricted to within a 10.95 km2 radius of each of the 13 locations where P county was the cluster center. These infections also co-clustered with certain characteristics, such as being unmarried, having a primary level of education or below, having used drugs for more than 10 years, and receptive sharing of syringes with others. The high-risk clusters for these characteristics were more likely to reside in the areas surrounding P county. HIV, HCV and co-infections among MMT clients in Guangxi Zhuang Autonomous Region all presented substantial geographic heterogeneity at the county level with a number of overlapping significant clusters. The areas surrounding P county were effective in enrolling high-risk clients in their MMT programs which, in turn, might enable people who inject drugs to inject less, share fewer syringes, and receive referrals for HIV or HCV treatment in a timely manner.

Background: Recent studies suggest that the metabolic score for insulin resistance (MetS-IR) is an effective indicator of metabolic disorders. However, evidence on the relationship between MetS-IR and metabolic syndrome (MetS) among the Chinese middle-aged and older adult population is limited. Objective: This cohort study aims to assess the associations of MetS-IR levels with MetS risk and its components. Methods: Data used in this study from the National Basic Public Health Service Project Management System (2020–2023). Multivariable Cox proportional hazards model and restricted cubic spline (RCS) were employed to evaluate the associations of baseline MetS-IR levels with MetS risk and its components, receiver operating characteristic (ROC) curves were further utilized to assess the efficacy of MetS-IR in predicting the risk of MetS and its component. Results: Of 1,498 subjects without MetS at baseline, 392 incident MetS cases were observed during a median of 27.70 months of follow-up. The adjusted multivariable Cox regression analysis indicated an elevated 15% risk of developing MetS for 1-SD increment of MetS-IR [hazard ratios (HRs) and 95% confidence intervals: 1.16 (1.13–1.18)]. Compared to the first tertile of MetS-IR, the HRs of the third tertile and second tertile were 6.31 (95% CI 4.55–8.76) and 2.72 (95% CI 1.92–3.85), respectively. Consistent findings were further detected across subgroups. Moreover, nonlinear associations were observed between MetS-IR and the risk of MetS, abdominal obesity, and reduced high-density lipoprotein concentration (HDL-C) (Pnonlinear < 0.01), with the cutoff of MetS-IR was 32.89. The area under the curve for MetS-IR in predicting MetS was 0.740 (95% CI 0.713–0.768), which was better than those of other indicators. Conclusion: Our cohort study indicates a positive nonlinear association between MetS-IR with incident MetS, abdominal obesity, and reduced HDL-C, but positive linear associations of MetS-IR and elevated blood pressure (BP), elevated fasting blood glucose (FBG), elevated triglycerides (TG) in middle-aged and older adult people, more studies are warranted to verify our findings.

Objective: The associations between obesity- and metabolism-related indices and the risk of non-alcoholic fatty liver disease (NAFLD) in elderly patients with type 2 diabetes mellitus (T2DM) remain unclear. This study aimed to investigate these associations and assess their predictive value for NAFLD in this high-risk population. Methods: A total of 789 elderly T2DM patients recruited between 2020 and 2022 were included in the cross-sectional analysis, and 382 patients without NAFLD were followed in the longitudinal cohort for a median of 25.37 months. Binary logistic regression and Cox models were used to assess associations between obesity- and metabolism-related indices and NAFLD risk. Kaplan–Meier curves, restricted cubic spline (RCS) models, subgroup analysis, and receiver operating characteristic (ROC) curves were applied to explore these relationships further. Results: In the cross-sectional analysis, all obesity- and metabolic-related indices were significantly and positively associated with NAFLD risk, with odds ratio (OR) ranging from 1.014 (95% CI: 1.010–1.018) for LAP to 3.288 (95% CI: 2.414–4.533) for WHtR. RCS analysis revealed significant nonlinear associations for LAP, MetS scores, VAI, CMI, METS-IR, and ABSI. In the cohort analysis, 67 participants developed NAFLD, with an incidence rate of 8.35 per 100 person-years. Baseline LAP (HR = 3.10, 95% CI: 1.48–6.51), and MetS scores (HR = 4.26, 95% CI: 1.99–9.11) were independently associated with increased risk of incident NAFLD. Subgroup analysis demonstrated consistent positive associations across most subgroups. Time-dependent ROC analysis showed that LAP had the highest AUCs at 24 months (AUC = 0.725).Conclusion: The findings from cross-sectional and cohort studies collectively supported that MetS score and LAP may be the most effective predictive indicators for the risk of NAFLD among Chinese elderly T2DM Patients.

In 2023, the American Heart Association (AHA) introduced the Cardiovascular-Kidney-Metabolic (CKM) syndrome concept to address the substantial burden of interrelated cardiovascular, kidney, and metabolic disorders. The framework highlights that chronic kidney disease (CKD) significantly accelerates CKM syndrome progression and increases cardiovascular risk, an effect that may be closely paralleled by aldosterone excess. Excess aldosterone can arise from renin-dependent aldosteronism (RDA), a primarily physiological state (not discussed in this review), or from renin-independent aldosteronism (RIA). RIA is a pathophysiologically relevant condition characterized by persistent autonomous activation, bypassing normal renin-angiotensin-aldosterone system (RAAS) regulation. Its most recognized form is PA, a prevalent, multidimensional disorder spanning a continuum from subclinical to overt autonomous aldosterone production. This leads to inappropriately elevated aldosterone relative to suppressed renin and sodium levels. PA is a leading cause of secondary hypertension and elevates the risk of metabolic and cardiorenal disorders, showing substantial overlap with CKM syndrome. Despite its clinical significance, the specific relationship between PA and CKM syndrome remains insufficiently investigated. This review synthesizes evidence from three key perspectives: (1) Epidemiology and clinical data show that PA spans a spectrum from subclinical to overt stages and is strongly associated with driving and accelerating the progression of CKM syndrome; (2) Therapeutically, targeted treatment of PA mitigates the adverse effects of aldosterone on CKM syndrome progression; and (3) Pathophysiologically, inappropriately elevated aldosterone primarily interacts with widely distributed mineralocorticoid receptors in tissues relevant to CKM syndrome, exacerbating key pathogenic pathways akin to adding fuel to the fire. Building on this synthesis, we emphasize that inappropriately elevated aldosterone is not merely a simple biomarker but an active driver and accelerator of CKM syndrome progression. This review also proposes future directions for integrated PA-CKM screening and management. Incorporating PA into the CKM syndrome framework could not only refine CKM syndrome care but also address the critical underdiagnosis of PA, whose screening rate regrettably remains below 2% in high-risk populations.
