Background: Esophageal squamous cell carcinoma (ESCC) is the sixth most lethal cancer worldwide and the fourth most lethal cancer in China. Genomic characterization of tumors, particularly those of different stages, is likely to reveal additional oncogenic mechanisms. Although copy number alterations and somatic point mutations associated with the development of ESCC have been identified by array-based technologies and genome-wide studies, the genomic characterization of ESCCs from different stages of the disease has not been explored. Here, we have performed either whole-genome sequencing or whole-exome sequencing on 51 stage I and 53 stage III ESCC patients to characterize the genomic alterations that occur during the various clinical stages of ESCC, and further validated these changes in 36 atypical hyperplasia samples. Results: Recurrent somatic amplifications at 8q were found to be enriched in stage I tumors and the deletions of 4p-q and 5q were particularly identified in stage III tumors. In particular, the FAM84B gene was amplified and overexpressed in preclinical and ESCC tumors. Knockdown of FAM84B in ESCC cell lines significantly reduced in vitro cell growth, migration and invasion. Although the cancer-associated genes TP53, PIK3CA, CDKN2A and their pathways showed no significant difference between stage I and stage III tumors, we identified and validated a prevalence of mutations in NOTCH1 and in the NOTCH pathway that indicate that they are involved in the preclinical and early stages of ESCC. Conclusions: Our results suggest that FAM84B and the NOTCH pathway are involved in the progression of ESCC and may be potential diagnostic targets for ESCC susceptibility.

Introduction: At present, cancer remains a persistent public health challenge facing the whole world. Studies have found that PTPN21 is associated with the development of cancer. However, the prognostic potential of PTPN21 in pan-cancer remains unclear. In this work, we aimed to analyze the expression and prognostic value of PTPN21 in pan-cancer and to further study the relationship between PTPN21 and immune infiltration. Material and methods: TCGA and GEO data were used for expression and survival analysis. Genetic alterations in PTPN21 from TCGA cancer were studied in cBioPortal. TIMER2 was used to evaluate the correlation between PTPN21 expression and immune infiltration. The R packages “ggplot2” and “clusterProfiler” were used for GO and KEGG analysis. Results: PTPN21 was found to be a valuable diagnostic biomarker in multiple cancers, including bladder urothelial carcinoma (BLCA), kidney renal clear cell carcinoma (KIRC), kidney renal papillary cell carcinoma (KIRP), and lung squamous cell carcinoma (LUSC). In addition, we observed that PTPN21 expression was associated with a variety of tumor mutations. Our results indicated a correlation between PTPN21 expression and immune infiltration. Enrichment analysis showed that PTPN21 was mainly involved in the regulation of neuroactive ligand-receptor interaction. Conclusions: Our study showed that PTPN21 expression is associated with clinical prognosis, mutation, and immune infiltration of tumors. PTPN21 may be a potential biomarker for many cancers, especially in KIRC.

Fentanyl abuse has been associated with neurological and psychological harm. However, the metabolic changes within the peripheral circulation and central nervous system involved in fentanyl addiction have not been well explored. In the present study, metabolic changes in plasma, caudate putamen (CPu), hippocampus (Hip), and prefrontal cortex (PFC) were investigated in mouse models of drug addiction based on fentanyl-induced conditioned place preference (CPP). Metabolic profiles were measured using untargeted UHPLC-Q-Exactive HFX mass spectrometry. A total of 131, 196, 104, and 52 altered metabolites were identified in plasma, CPu, Hip, and PFC, respectively. The identified metabolites mainly included lipid mediators, carbohydrate metabolites, fatty acids, amino acids (AAs) and their derivatives, and nucleotide metabolites. The disturbed metabolic pathways were primarily involved in lipid metabolism, carbohydrate metabolism, AA metabolism, nucleotide metabolism, and the metabolism of cofactors and vitamins. These findings indicate disturbances in cell membrane metabolism, energy metabolism, AA metabolism, and neurotransmitter systems caused by fentanyl addiction. Our study provides a valuable resource for future investigations aimed at defining the role of metabolites in fentanyl addiction, which may help develop new pharmacotherapies.

Primary hyperparathyroidism (PHPT) most commonly arises from cervical parathyroid pathology, with ectopic disease representing a rare entity. We report a case of PHPT diagnosed during fracture workup in a female patient who manifested persistent hypercalcemia and elevated parathyroid hormone (PTH) despite two prior cervical explorations. Cross-sectional and functional imaging, including PET/CT, localized a PTH-secreting hepatic lesion, which was confirmed by ultrasound-guided biopsy. Laparoscopic resection of the intrahepatic tumor resulted in prompt biochemical normalization. Histopathology confirmed an intrahepatic ectopic atypical parathyroid adenoma. This case underscores that refractory PHPT with persistent biochemical derangements following conventional cervical surgery should prompt consideration of rare ectopic parathyroid neoplasms with atypical histologic features.
