Objective: To investigate the relationship between unexplained recurrent pregnancy loss (URPL) and polymorphisms of homocysteine metabolism-related genes in women. Materials and Methods: A case–control study included 90 women with two or more consecutive unexplained pregnancy losses and 92 controlled women without miscarriage history; the female participants were in the age category of 18– 35 years. The high-resolution melting technique was used to detect the single-nucleotide variants related to homocysteine metabolism disorder, namely MTHFR C677T, MTHFR A1298C, MTR A2756G, and MTRR A66G polymorphism. Results: The MTHFR C677T polymorphism had significantly correlation with URPL. Indeed, the frequency of the677T allele and genotypes (677CT, 677TT) in the URPL group was significantly higher than that in the control group (p < 0.05). However, the allele, as well as genotype distribution of MTHFR A1298C, MTR A2756G, and MTRR A66G polymorphisms showed no significant difference (p > 0.05). MTHFR 677CT-1298AC genotype combination led to a 9.0-fold increased risk of URPL (OR 9.0; 95% CI, 2.25– 35.99; p = 0.001), while the risk increased 10.0-fold (OR 10.0; 95% CI, 1.8– 55.53; p = 0.008) when participants had more than the 3 variant loci. Conclusion: The MTHFR C677T polymorphism was a risk factor for URPL, and determining the MTHFR C677T polymorphism had a potential prediction of URPL risk. Moreover, the MTHFR C677T and MTHFR A1298C joint mutants might have a synergistic effect on URPL. Conversely, there is a lack of evidence suggesting the URPL risk of MTHFR A1298C, MTR A2756G, and MTRR A66G polymorphisms.

Background: The Y chromosome has a specific region, namely the Azoospermia Factor (AZF) because azoospermia is typically reported in the microdeletion of the AZF region. This study aims to assess the characteristics of AZF microdeletion after screening a massive number of low sperm concentration men; and the Microdissection testicular sperm extraction (mTESE) outcomes for retrieving sperm from azoospermic patients. Materials and Methods: This retrospective multiple-center study enrolled a total of 1121 men with azoospermia, cryptozoospermia, and severe oligozoospermia from December 2016 to June 2022. An extension analysis used a total of 17 STSs to detect the position-occurring microdeletion in the AZF region (AZFa, b, c, and/or d loci). Microdissection testicular sperm extraction (mTESE) was performed to retrieve sperm in azoospermic men diagnosed AZFc microdeletion. Results: One hundred and fifty-three men carried AZF microdeletion were detected in the 1121 participants (13.64%). The incidences of AZF microdeletion were confined to AZF a, c, and d regions, both individual and concurrence, with the most common in the AZFc region accounting for 49.67%; There was no significant difference in clinical and paraclinical characteristics between the deleted regions, except FSH level (highest in AZFa microdeletion, p = 0.043). The AZFc region was the most common type of AZF microdeletion (49.67%), including complete microdeletion (4 patients) and gr/gr partial microdeletion (39 patients) with 50.00% and 63.63% in the success rate of mTESE, separately. Conclusion: The absence of AZFa and/or AZFb regions often express the most severe phenotype – azoospermia and the increasing FSH level. The AZFc region played the most common microdeletion. Microdissection testicular sperm extraction (mTESE) was the possible therapy for sperm retrieval from the testis of azoospermia men having AZFc microdeletion.

Introduction: Endothelium dysfunction and decrease of incretin effects occur early in type 2 diabetes mellitus and these changes contribute to diabetic cardiovascular complications such as atherosclerosis, thick intima-media, coronary, and peripheral arterial diseases. In patients with diabetes, the femoral artery is a site of a high incidence of injury in peripheral vascular diseases, and atherosclerotic changes may appear earlier in the femoral artery compared to the carotid artery. This study was conducted to determine the prevalence of increased femoral artery intima-media thickness (IMT) and atherosclerotic plaque and their correlation with serum glucagon-like peptide-1 (GLP-1) levels in newly-diagnosed patients with type 2 diabetes mellitus. Materials and Methods: A cross-sectional study was conducted on 332 patients with nT2D in the National Endocrinology Hospital, Vietnam from January 2015 to May 2018. IMT was measured by Doppler ultrasound and GLP-1 by enzyme-linked immunosorbent assay (ELISA). All data were analyzed with SPSS version 26 for Windows (SPSS Inc, Chicago, IL). Results: Prevalence of thick femoral artery IMT and atherosclerotic plaque was 38.2 and 22.3%, respectively. There was a relationship between IMT and age, waist to hip ratio (WHR), systolic blood pressure (SBP), diastolic blood pressure (DBP), fasting GLP-1, high sensitive CRP (hsCRP) and 24-hour microalbuminuria secretion (24-h MAUS). The fasting serum GLP-1 (fGLP-1) levels were reduced significantly in patients with thickness and atherosclerosis femoral artery (p = 0.001). After adjusting with other related factors, namely, DBP and estimated glomerular filtration rate (eGFR), whilst hsCRP and 24-h MAUS showed a significantly positive correlation to IMT (Standardized B and p of 0.242, 0.004 and 0.178, 0.043, respectively), fGLP-1 showed a significantly negative correlation to IMT (Standardized B = -0.288, p = 0.001). Conclusion: Among n2TD, the percentage for femoral artery thick IMT and atherosclerosis was 38.2% and 22.3% respectively, and serum GLP-1 was negatively correlated with thick IMT and atherosclerosis.

Rationale: Pulmonary sclerosing pneumocytoma (PSP) is a rare benign tumor of the lung, mostly presented in Asian middle-aged women. Initially, it was considered as a vascular origin tumor, but then research evidence showed that it was derived from natural epithelial tissue. On imaging, this tumor may be found as a solitary well-circumscribed lung parenchymal lesion, and is often located in juxtapleural or juxtafissural positions. On histopathology, it consists of cuboidal surface cells and stromal round cells, both of which are positive for thyroid transcription factor-1. Here we report a case of a young PSP male patient and review the relevant literature in order to improve our understanding of this disease. Patient concerns: An 18-year-old man was referred to our hospital after accidentally finding a lesion on chest X-ray. Contrast-enhanced computed tomography showed a soft tissue mass with homogeneous enhancement in the left lower lobe posterior segment. Diagnoses: The diagnosis of PSPs was confirmed by histopathological examination. Interventions and outcomes: The patient underwent a thoracoscopic wedge resection and was followed-up after that. One month later, he had good performance status with no recurrent tumors. Lessons: PSP in a young man is really uncommon, and is confused with malignant tumors. A histopathological examination is considered as the diagnostic gold standard for this uncommon tumor. Surgery is the main treatment.

Background: Chromosomal abnormalities and variations are significant contributors to reproductive challenges. This study aims to investigate the types and incidence of structural autosomal anomalies and autosomal variations in a large Vietnamese population undergoing infertility treatment. Material and Methods: A retrospective analysis was conducted on 19,191 females and 18,584 males who needed assisted reproductive technology (ART) at the Military Institute of Clinical Embryology and Histology, and Andrology and Fertility Hospital of Hanoi from 2020 to 2023. Karyotyping was conducted using the G-band staining method, and the data were analyzed with STATA 16.0 software. Results: Males have a higher overall occurrence of structural autosomal anomalies, with a total of 359 cases (1.932%) compared to 306 cases (1.594%) in females, particularly inversions and robertsonian translocations. Chromosome 9 inversions were equally observed in both genders, while robertsonian translocations and reciprocal translocations were more frequent in males (0.183% and 0.468%, respectively) than in females (0.146% and 0.406%, respectively). Deletions and duplications were more prevalent in males, occurring at rates of 0.215% and 0.016%, respectively, versus 0.036% and 0.021% in females. Total autosomal variants were 1478 (7.953%) in males and 1864 (9.7%) in females. Chromosome 9 exhibits the highest occurrence of the q+/qh+ variant, followed by the one of chromosome 1 and chromosome 16. Chromosomes 21 and 22 show notable numbers of ps+ and pstk+ variants. Conclusion: Structural autosomal anomalies and autosomal variations are common in Vietnamese patients undergoing infertility, highlighting the necessity of genetic testing, particularly karyotyping, in the evaluation and management of infertility.

Background: Erectile dysfunction (ED) and infertility are important health concerns with negative impacts on the quality of life and couple’s relationship, which is emerging as an early marker for decreased general health. Objectives: This pilot study intended to investigate the prevalence and risk factors of erectile dysfunction in men of infertile couples referred to an in vitro fertility center in northern Vietnam. Methods: A total of 138 men of infertile couples were enrolled in this study from January to May 2018. The International Index of Erectile Function questionnaire scale was used to evaluate the severity of ED. Also, several related factors were collected to determine potential risk factors. Results: The prevalence of ED among was 18.1%, including 13 mild ED (9.4%), 7 mild to moderate ED (5.1%), 2 moderate ED (1.4%), and 1 severe ED (0.7%). The logistic regression analysis showed that risk factors associated ED were smoking > 5 pack/years (OR = 3.16, CI = 0.98 - 10.18; P = 0.05), not graduating from junior high school (OR = 5.11; CI = 1.11 - 23.8; P = 0.03) and azoospermia (OR = 7.55; CI = 2.18 - 26.16; P = 0.001). We observed relatively high OR in men with smoking < 5 pack/years (OR = 2.51) and abnormal semen analysis (hypospermia, oligozoospermia, asthenozoospermia, or teratozoospermia) (OR = 2.36), but there was no significant difference compared to the reference group. Conclusions: In the present study, the prevalence of ED among men of infertile couples was higher than non-infertile men. Low level of education, smoking for a long period, and azoospermia were the main risk factors of ED in men. Further large-scale studies are needed to extend the results.
