Aim: This study aimed long-term prognostic factors for percutaneous coronary intervention (PCI) in left main coronary artery (LMCA) disease, focusing on inflammatory parameters like Pan Immune-Inflammation Value (PIV) and Systemic Immune-Inflammation Index (SII).Materials & methods: This retrospective, observational study involved 121 patients receiving unprotected LMCA PCI.The study's primary end point was MACE, including all-cause death, repeat revascularization and myocardial reinfarction. Results: The PIV and SII scores emerged as independent predictors of MACE occurrence in patients undergoing PCI for unprotected LMCA disease (p:0.045,p:0.048;respectively).In addition, age, ejection fraction, creatinine, shock presentation and the residual syntax score were identified as significant independent predictors of MACE in both models. Conclusion: This study showed a significant, independent correlation between PIV, SII and MACE in patients undergoing PCI for LMCA disease, with PIV showing slightly better predictive capability than SII.

To evaluate the prognostic significance of the preprocedural albumin - bilirubin (ALBI) score on short- and long-term outcomes after transcatheter aortic valve implantation (TAVI). We retrospectively analyzed 737 consecutive patients who underwent TAVI. Patients were stratified into tertiles according to ALBI score. Baseline clinical, echocardiographic, laboratory features were compared across tertiles. The primary endpoint was long-term all-cause mortality; secondary endpoints included in-hospital mortality and major complications. Kaplan-Meier analysis assessed survival, and Cox-regression models evaluated predictors of long-term mortality after adjustment for conventional risk factors. Higher ALBI tertiles were associated with a greater comorbidity burden, worse ventricular function, and elevated biomarkers of inflammation and cardiac stress. In-hospital mortality increased across tertiles (5.3%, 7.3%, 11.8%). Long-term mortality also rose progressively (7.3%, 9.8%, 11.8%), with 60-month survival rates of approximately 90% (T1), 88% (T2), and 73% (T3). ALBI score independently predicted long-term mortality (HR 1.86, 95% CI 1.23-2.82), along with chronic kidney disease and lower TAPSE. Restricted cubic spline analysis demonstrated a near-linear association between ALBI and mortality risk. Preprocedural ALBI score is independently associated with both short and long-term mortality after TAVI and may reflect systemic vulnerability not captured by conventional risk models. Incorporating ALBI into preprocedural assessment could enhance risk stratification.

Background: Contrast-induced acute kidney injury (CI-AKI) can be a serious complication for patients with peripheral artery disease(PAD) undergoing peripheral vascular interventions(PVI). An interarm blood pressure difference (IABPD)≥10 mmHg has been identified as an independent risk factor for cardiovascular disease and mortality. This study aimed to evaluate the predictive value of IABPD for the risk of CI-AKI in PAD patients undergoing PVI. Materials and Methods: This prospective study included 171 consecutive patients who underwent PVI. IABPD was defined as the difference in systolic blood pressure between the two arms and was considered significant if it was≥10 mmHg. Patients were categorized into two groups based on the occurrence of CI-AKI. Results: The incidence of CI-AKI after PVI was 21 %. The CI-AKI(+) group had a significantly higher incidence of IABPD >10 mmHg (28% vs. 8%, p<0.001). Multivariable logistic regression analysis identified IABPD (OR:1.135, 95% CI:1.037-1.243, p=0.006) as an independent predictor of CI-AKI. Additionally, hypertension (OR:2.308, 95% CI:1.091-4.885, p=0.03), higher mean blood pressure (OR:1.055, 95% CI 1.001-1.111, p=0.04), lower eGFR (OR 0.963, 95% CI 0.948-0.978, p<0.001), higher CRP levels (OR:1.028, 95% CI:1.006-1.050, p=0.01), and lower LVEF (OR:0.969, 95% CI 0.938-0.998, p=0.04) were significant predictors of CI-AKI. Furthermore, having TASC C-D lesions compared to TASC A-B was associated with a higher risk of CI-AKI (OR:3.304, 95% CI 1.197-9.117, p=0.02). Conclusions: This study demonstrated that IABPD is significantly associated with the development of CI-AKI in patients undergoing PVI for PAD. Assessing IABPD in patients before PVI could help clinicians identify those at an elevated risk for developing CI-AKI. Objectives: The deposition of calcium in the vascular bed can reflect the presence of atherosclerotic lesions, and aortic knob calcification (AKC) has been shown to correlate with a heightened risk of cardiovascular disease (CVD) events. This retrospective research was undertaken with the aim of evaluating the relationship between AKC and the coronary artery calcium score (CACS) by chest X-ray and investigating the possibility of using AKC to predict the risk of CVD. We hypothesized that AKC would be detected more frequently on chest X-rays among patients with severe CACS values. Methods: The study included 282 consecutive patients diagnosed with stable angina pectoris (SAP) who had presented to our cardiology outpatient clinic between 2018 and 2020 and underwent cardiac computed tomography and chest X-ray. Those whose chest X-rays revealed small spots or singular areas of fine calcification were considered to have AKC. CACS values were classified according to the Agatston score and CACS severity was defined as none-mild (100). Results: The mean age of the 282 enrolled patients was 47.5±11.3 years and 169 (59.9%) of the patients were male. CACS values of >100 signifying moderate-severe risk were observed more commonly among patients with diabetes mellitus, hypertension (HT), older age, and smoking habits (p<0.001). Thirty-five patients (12.4%) had AKC on a chest X-ray. Patients with AKC were more likely to be categorized in the moderate-high CACS severity group than those without AKC (p<0.001). Logistic regression analysis demonstrated that HT (odds ratio [OR]: 4.413; 95% confidence interval [CI]: 1.733–11.240; p=0.002) and AKC (OR: 5.619; 95% CI: 2.280–13.848; p<0.001) were independent predictors of moderate-severe CACS values. Conclusion: It is important to establish the risk of CVD in patients with SAP using simple and non-invasive methods. Our study showed that the presence of AKC on chest X-ray is associated with higher CACS values in patients presenting with SAP. Therefore, AKC may be used more frequently for the evaluation of the risk of CVD.

Background: The objective was to examine the predictive value of malnutrition, assessed via the Controlling Nutritional status (CONUT) and Prognostic Nutrition Index (PNI) scores, in the development of contrast-associated acute kidney injury (CA-AKI) following peripheral vascular intervention (PVI). Methods: This retrospective cross-sectional observational study included the enrollment of 243 consecutive patients who underwent PVI. Patients were categorized into two groups based on the occurrence of CA-AKI. Results: Patients with CA-AKI had lower PNI scores and the PNI score was an independent predictor of CA-AKI development (Odds Ratio: 0.518, 95% CI: 2.295-0.908, p = 0.021). Nomogram had higher discriminative ability than both PNI and CONUT scores and discriminative abilities were similar for PNI and CONUT scores. Conclusion: Malnutrition, as identified by the CONUT and PNI, was found to be associated with a high risk of CA-AKI development following PVI.
