We conducted our study to investigate the relationships between hyperemesis gravidarum (HEG) and inflammatory markers such as the neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and monocyte-to-lymphocyte ratio (MLR). A total of 150 pregnant women diagnosed with HEG and 150 controls were included in our study. The data analysed included demographic variables, complete blood count results, and urinary ketonuria levels. We found that the NLR, PLR and MLR were significantly greater in HEG patients than in controls (p < 0.05), indicating a potential role of systemic inflammation in the pathophysiology of HEG. Receiver operating characteristic (ROC) analysis revealed that these markers had moderate discriminative power, suggesting their utility as adjunctive diagnostic tools in clinical settings. However, no correlation was found between inflammatory markers and the severity of ketonuria. The NLR, PLR and MLR can be used as indicators of HEG as a result of the inflammatory process in pregnant women with HEG.

The aim of this study was to analyze the inflammatory markers, including C-reactive protein to albumin (ALB) ratios (CAR), and fibrinogen to ALB ratios (FAR) of pregnant women who visited the obstetrics clinic of our hospital and were diagnosed with previable preterm prelabor rupture of membranes (PPROM), defined as delivery between 20 0/7 and 25 6/7 weeks at the border of fetal viability, and to compare the scores with those of the healthy control group. We also aimed to determine the utility of these laboratory scores in women who aborted or continued their pregnancy and live births. Patients aged 18 to 45 diagnosed with previable PPROM between 16 and 22 weeks with singleton live pregnancies were included. Patients with uterine or fetal anomalies, multiple pregnancies, or history of chronic inflammatory disease were excluded. A total of 79 pregnant women were divided into 2 groups: healthy pregnant controls (control, n = 30) and patients diagnosed with previable PPROM (n = 49). 35/49 (71.4%) preferred expectant management. Of these 35 patients 14/35 (40%) had live birth and 21/35 (60%) had abortions. C-reactive protein (23 mg/dl), CAR (0.84), and FAR values were statistically significantly higher, whereas ALB levels (34.2 g/L) were lower in previable PPROM patients than in controls ( P < .001). More importantly, our findings indicated that the FAR at admission (1.4, P < .001) was positively correlated with abortion in previable PPROM among the patients during expectant management. Receiver operating characteristic analysis revealed that the area under the curve for FAR was 0.840 ( P < .001; sensitivity, 76.2%; specificity, 85.7%), and FAR presented the highest predictive value with a 1.18 cut point for abortion in previable PPROM. While the CAR value was significantly higher in patients with PPROM in the patient and control groups, the FAR value was significantly higher in patients who aborted in the PPROM group than in those with live pregnancies. Routine markers that are readily derived in daily clinical practice, such as the FAR, may have clinical significance in evaluating predicting prognosis in previable PPROM.

To compare the demographic characteristics, maternal and perinatal outcomes and hemoglobin parameters according to stages diagnosed with placental abruption. Patients who underwent cesarean delivery due to clinical suspicion of placental abruption after the 20th week of gestation were included in the study. We eliminated patients with persistent hematological disorders, those on anticoagulant medications, and individuals with uterine deformities. Demographic information, body mass index, initial laboratory results, duration of hospitalization, fetal APGAR scores, and transfers to maternal and neonatal critical care units were all documented from the data system. Patients were categorized into categories based on the stages of placental abruption as outlined in the 2022 emergency obstetric care guide. Stage 0: Asymptomatic patient. Stage I: There is vaginal bleeding, clinical pain in abdomen, no signs of maternal shock or fetal distress. Stage II: There may be vaginal bleeding, no maternal shock. Fetal distress is present. Stage III: There may be maternal shock, intrauterine fetal demise is present. Presence of coagulopathy (30%). The patients were categorized as follows: Group 1 stage 0–I; Group 2 stage II; and Group 3 stage III, and the gathered data were compared among the groups. A total of 150 patients were involved, with 50 patients in group 1. Group 2 comprised 59 patients, whereas Group 3 had 41 patients. Abruptions were most prevalent in group 3. The neutrophil/lymphocyte ratio (NLR) was statistically significantly elevated in groups 2 and 3. The platelet/lymphocyte ratio (PLR) was statistically significantly reduced in group 1. Hemoglobin and fibrinogen levels were deemed statistically significantly reduced in group 3. In group 2, the Apgar scores at 1 min and 5 min were the lowest, and the demand for newborn intensive care was the highest. The analysis of hemogram parameters at the initial assessment may aid in the diagnosis of abruption, complementing ultrasonography, particularly in cases of early clinical suspicion. We believe that commonly conducted, cost-effective, and straightforward hemogram parameters, when augmented by artificial intelligence, can provide therapeutic support through the interpretation of data such as PLR and NLP in instances of ambiguous laboratory diagnoses, such as placental abruption.

Objective: To evaluate whether uterosacral ligament (USL) thickness measured using magnetic resonance imaging (MRI) was associated with overactive bladder (OAB) in otherwise healthy women. Materials and Methods: The study comprised 27 women with OAB and 27 healthy women (control group) who were followed up at the Obstetrics and Gynecology Department of a tertiary referral center. All subjects were evaluated using pelvic MRI to determine the transverse USL thickness. These measurements were compared between the two groups. p values less than 0.05 were considered statistically significant. Results: The mean age of women in the OAB and control groups were 43.88 +/- 9.36 years and 39.92 +/- 5.36 years, respectively. The mean body mass index in the OAB group was 29.77 +/- 4.82 kg/m(2) and 27.49 +/- 3.44 kg/m(2) in the control group. In the comparison of Pelvic Organ Prolapse Quantification system stages between the groups, no statistically significant relationship was determined. In the OAB group, the mean right USL thickness was 2.04 +/- 0.34 mm, and the mean left USL was 2.04 +/- 0.52 mm. In the control group, the mean right USL thickness was 2.17 +/- 0.47 mm, and the mean left USL was 2.09 +/- 0.51 mm. There were no statistically significant differences in terms of USL thickness between the OAB and control groups (p> 0.05). Conclusion: No previous studies have been identified in the literature that have investigated the relationship between USL thicknesses and urinary incontinence. In the present study, no significant relationship could be demonstrated between right and left USL thicknesses of the OAB and control groups. This was a preliminary study, and further research with larger sample sizes is required to reach a final conclusion.

Background/Aim: Increased uterus weight, high body mass index (BMI), and history of abdominal surgeries increase the risk of complications in total laparoscopic hysterectomy (TLH), similar to other types of surgery. However, there are conflicting reports about improving technology. This study aimed to retrospectively investigate the clinical features and postoperative results of TLH cases regarding uterine volume performed for benign reasons in our clinic. Methods: In this retrospective cohort study, 252 patients were included. The demographic data of all patients, including BMI, pre-operative uterine volumes, operation times, number of cesarean sections, history of lower abdominal operation, indications, pre-operative and postoperative hemoglobin differences, complications, length of hospital stay, and final pathologies were reviewed. The uterine volume was measured using the prolate ellipsoid formula before surgery, using the maximum length and anteroposterior and transverse diameters of the uterine corpus. The normal uterine volume with these measurements (8 cm long, 4 cm high, and 5 cm wide) was estimated as 83.2 cm3. The patients were classified into two groups according to uterine volume (normal volume ≤ 83.2 or above). Surgical outcomes of patients were compared between groups. Results: Two-hundred-fifty-two women were included in the study. The mean uterine volumes of groups 1 and 2 were 53.66 cm3 (2.25) and 296.33 cm3 (6.25), respectively. In group 1, the mean operation time was 111.14 (6) min, compared to 118.2 (3.06) min in group 2; there was no significant difference (P = 0.164). The mean postoperative hospital stays of groups 1 and 2 were 3.21 (0.15) and 3.34 (0.09) days, respectively, and there was no significant difference (P = 0.706). The mean blood loss values of groups 1 and 2 were 1.34 g/dl (0.19) and 1.16 g/dl 0.06), respectively. Conclusion: According to our results, TLH is a safe method even in patients with a larger uterus; operating time, blood loss, and postoperative hospital stays did not differ according to uterine volume.

Adenomyosis is increasingly recognized as a significant factor affecting fertility, particularly in the context of assisted reproductive technologies (ART). This study aimed to assess the independent impact of adenomyosis on intrauterine insemination (IUI) success rates in women with unexplained infertility. This retrospective cross-sectional study included 533 IUI cycles in 374 infertile women treated at the University of Health Science, assisted reproductive technologies center, Adana City Hospital. The study identified adenomyosis in 95 patients via 2D transvaginal Doppler ultrasonography and 3D transabdominal ultrasonography, whereas direct and indirect adenomyosis features were not detected in 279 patients. Both groups started ovulation induction on cycle day 2–3 using recombinant follicle stimulating hormone (rFSH) or letrozole combined with rFSH or rFSH combined with recombinant luteinizing hormone (rLH). Clinical and ultrasound findings were recorded systematically. IUI was performed 36 h post-human chorionic ggonadotropin (hCG) administration, with pregnancy defined as a positive β-hCG 12–14 days after IUI. Logistic regression models were used to analyze the independent effects of adenomyosis on clinical pregnancy rates, adjusting for potential confounders. The cumulative pregnancy rate per cycle was significantly lower in women with adenomyosis (12.23%) compared to those without adenomyosis (20.81%). Adenomyosis was identified as a significant negative predictor of IUI success (OR 0.575, 95% CI: 0.335–0.998, p = 0.049). Adenomyosis negatively affects IUI outcomes, suggesting the need for tailored fertility treatment strategies in this population. Routine assessment of adenomyosis should be considered in infertility evaluations to optimize clinical management and treatment success.
