Background.Overexpression of the MUC5B protein is associated with idiopathic pulmonary fibrosis (IPF), but little information is available regarding the pathogenic effects and regulatory mechanisms of overexpressed MUC5B in IPF.Main Body. The overexpression of MUC5B in terminal bronchi and honeycomb cysts produces mucosal host defensive dysfunction in the distal airway which may play an important role in the development of IPF. This review addresses the possible association of overexpression ofMUC5B, withMUC5Bpromoter polymorphism,MUC5Bgene epigenetic changes, effects of some transcriptional factors, and inflammatory mediators in IPF. In addition, the associated signaling pathways which may influence the expression ofMUC5Bare also discussed.Conclusion.This work has important implications for further exploration of the mechanisms of overexpression of MUC5B in IPF, and future personalized treatment.

As the development of urbanization in China, the morbidity of allergic disease rise up prominently even in children, which may be partially associated with the excessively clean environment. It has been reported that common microorganism in rural environment shows protective effects on allergic disease by modulating TLRs-Tregs/Th cell axis. But the mechanism of this protection still needs to be elucidated in detail. We investigated the effects of maternal exposure to farming environment on the neonatal innate immune system, especially on the TLR-Treg-Th (Th1, Th2, Th9, and Th17) axis, in the Jilin province of China. Eighty-four non-farming and 42 farming pregnant women were recruited. Endotoxins and glucans in dust from the living rooms of the pregnant mothers were measured. Cord blood mononuclear cells were challenged with phytohemagglutinin, lipopolysaccharide, or peptidoglycan. Proliferative response of lymphocyte was measured by 3H-TdR incorporation methods, CD4 + CD25 + FOXP3 + T cells percentage was assessed with flow cytometry, Tregs specific genes (FOXP3, LAG3, GITR, CTLA-4 and TGF-β) and TLR2, TLR4 genes expression were detected by RT-PCR, specific cytokines of Th1, Th2, Th9, Th17 and Tregs were measured with flow cytometer, suppressive capacity of Tregs was tested by culturing with effector cells in vitro, and TLR2/4 gene polymorphism was detected. Higher endotoxin content was observed in the living rooms of the farming mothers. Compared with that in the non-farming group, in farming neonatal CBMCs, lymphocyte proliferation declined; the IFN-γ/IL-13 ratio increased; and the quantity of Tregs and gene expression of FOXP3, GITR, CTLA4 and TLR2 increased significantly (P < 0.05). Isolated Tregs suppressed the proliferation of effector T cells and IL-13 production more strongly in vitro (P = 0.04, 0.03, respectively), and the TLR2 polymorphism affected FOXP3 expression and IFN-γ and IL-13 production. Maternal exposure to farming affected the quantity and function of neonatal Tregs upon stimulation with PPG and LPS, which partly contributed to reducing the risk for allergic diseases in the offspring. The results of our study will lay the theoretical foundation for allergic disease prevention in early life.

No systemic evaluation of asthma control in Jilin Province has been reported. Asthma control might provide the basis for asthma management in this region. A multicenter hospital-based cross-sectional study was performed to investigate the asthma control and related factors for severe asthma exacerbations in patients with moderate or severe asthma in Jilin Province, China. The study enrolled 1546 patients in five grade one general hospitals from January to December 2013. Asthma medication, patient self-management, asthma control test (ACT) scores and frequency of severe asthma exacerbations during the follow-up (12 months) were collected via a follow-up questionnaire. In the study, 889 patients provided a complete follow-up questionnaire. Severe asthma exacerbations occurred in 54.89 % of patients. ACT score ≤15, asthma medication ≤ 3 months, severe asthma, income level lower than average Per Capita Disposable Income (PCDI) and a lower educational level were risk factors of a severe exacerbation. Poor adherence to asthma medication, poor asthma symptom control, lower income, a low educational level might be possible reasons for the high incidence of severe asthma exacerbations and poor asthma control in Jilin Province of China.

Corynebacterium striatum (C. striatum) is a Gram-positive bacterium commonly colonizing the skin and mucosa in healthy individuals and hospitalized patients. Traditionally regarded as a contaminant, C. striatum is now increasingly recognized as a potential cause of clinical infections, especially after the coronavirus disease pandemic. It has emerged as a pathogen implicated in severe infections, including pneumonia, bacteremia, meningitis, artificial joint infections, abdominal infections, and endocarditis. C. striatum has been reported in lower respiratory tract infections, mostly as a conditioned pathogen in immunocompromised individuals, particularly in those with chronic structural lung diseases, such as chronic obstructive pulmonary disease, leading to severe pneumonia or exacerbation of the existing disease and high mortality. Additionally, C striatum has been implicated in the community-acquired pneumonia among immunocompetent individuals and nosocomial lung infections, with evidence of person-to-person transmission through caregivers. C. striatum may exhibit multidrug resistance. Vancomycin, alone or in combination, is currently considered the most effective treatment for C. striatum. This review highlights the epidemiological characteristics, drug resistance mechanisms, diagnostics approaches, and treatment options for C. striatum lower respiratory tract infections to enhance clinician awareness and improve patient management strategies.

Acute respiratory distress syndrome (ARDS) induced by sepsis contributes remarkably to the high mortality rate observed in intensive care units, largely due to a lack of effective drug therapies. Histone deacetylase 6 (HDAC6) is a class‑IIb deacetylase that modulates non‑nuclear protein functions via deacetylation and ubiquitination. Importantly, HDAC6 has been shown to exert anti‑cancer, anti‑neurodegeneration, and immunological effects, and several HDAC6 inhibitors have now entered clinical trials. It has also been recently shown to modulate inflammation, and HDAC6 inhibition has been demonstrated to markedly suppress experimental sepsis. The present review summarizes the role of HDAC6 in sepsis‑induced inflammation and endothelial barrier dysfunction in recent years. It is proposed that HDAC6 inhibition predominantly ameliorates sepsis‑induced ARDS by directly attenuating inflammation, which modulates the innate and adaptive immunity, transcription of pro‑inflammatory genes, and protects endothelial barrier function. HDAC6 inhibition protects against sepsis‑induced ARDS, thereby making HDAC6 a promising therapeutic target. However, HDAC inhibition may be associated with adverse effects on the embryo sac and oocyte, necessitating further studies.

In recent years, reports of hypervirulent (hv) carbapenem-resistant (CR) Klebsiella pneumoniae (Kp) (hv-CRKp) have gradually increased. hv-CRKp may emerge from hvKp acquiring mobile genetic elements carrying multiple antibiotic-resistance genes or multi-drug-resistant Kp acquiring virulence genes, with subsequent convergence of resistance and virulence. Thus, hv-CRKp simultaneously harbors resistance and virulence genes and may even show resistance to colistin and tigecycline, suggesting potential for causing severe infections and placing a serious burden on the health care system. In this study, we review and summarize published literature on the epidemiology, virulence, resistance genes, and treatment of hv-CRKp.
