Objectives: The aim of this study is to investigate the effects of vitamin D levels on balance and fall risk on hemiplegic cases. Methods: In our descriptive study, a total of 152 patients who applied to the Physical Medicine and Rehabilitation Polyclinic with the diagnosis of Hemiplegia and were hospitalized in Maçka Ömer Burhanoğlu Physical Therapy and Rehabilitation Hospital between July 2020 and January 2021 were included. Results: A total of the 152 patients enrolled in the study, 57 were women (37.5%) and 95 were men (62.5%). The average age of the patients was 62.0± 12.7. The number of patients enrolled in the study was 19 (12.5%), 98 (64.5%), and 35 (23.0%). The number of patients with vitamin D Levels 20 ng/ml was 32 (21.0%). Again, the vitamin D values measured on the last day of discharge of patients enrolled in the study were <12 ng/ml 46 (31.3%). In the study, the correlation between vitamin D levels during discharge and Berg Balance Scale score and İtaki Fall Risk Scale score was examined.; There is no difference between vitamin D level and Berg Balance Scale score at positive weak level (r=0.184, p<0.05) and İtaki Fall Risk Scale. (p<0.05). Conclusion: This study showed that vitamin D levels had positive effects on balance and decreased risk of falls in hemiplegia patients with low vitamin D levels.

Given how dramatically the pandemic has affected food systems, the economy, and the daily lives of children over the past 2 years, the potential impact of the pandemic on childhood obesity requires careful investigation. The aim of this study was to investigate the change in body mass index z-score in 3-year-old children and the inducing factors during the pandemic period. The body mass index z-scores of all children participating in the study were calculated at the beginning of the pandemic (3-year-old body mass index z-score) and in its second year (5-year-old body mass index z-score). This study, conducted during the 2-year pandemic period, found a strong association between the body mass index z-scores of children aged 3 and 5 years. The mean body mass index z-score increased between these time points for both boys and girls (p=0.013; p=0.034). In two different linear regression models created for the change in body mass index z score, gestational weight gain was found to be related. The regression coefficients (95% confidence intervals) and corresponding p-values were 0.580 (0.217–0.944) and p=0.002 for model 1, whereas they were 0.585 (0.217–0.961) and p=0.002 for model 2. This study showed an increase in body mass index z-scores in early childhood period during the COVID-19 pandemic. To prevent this increase, new strategies should be developed by considering the changes brought by the pandemic period.

A six-year-old male patient who has a twin healthy brother was admitted to our family medicine clinic with complaints of cough, sputum, and nausea. On physical examination, fever was 36.7°C, oropharynx was hyperemic, he had tonsillar hypertrophy, left ear discharge and respiratory sounds were natural. The patient was diagnosed with "Acute Upper Respiratory Tract Infection" and treatment was started. During the examination, the patient was agitated and fearful, unable to concentrate, did not make eye contact, did not respond to questions and warnings, and exhibited distinct behavioral differences from his twin, such as shouting and crying, and aggressive features. When his medical history was questioned, he was learned to have born to a 45-year-old father and a 35-year-old mother through in vitro fertilization method on 35 weeks and 4 days, he was 2650 gram. He was breastfed for 8 months, could speak a word around the age of 2, and there was usually a neurodevelopmental process behind his twin. Considering this situation, the patient was referred to the child psychiatry outpatient clinic due to the suspicion of autism. He was evaluated in the child psychiatry outpatient clinic for the first time three years ago, since his speech did not start at the same time with his twin, and he was recommended to come for a follow-up visit 3 months later with a pre-diagnosis of autism. The patient, who did not come for the control, was diagnosed with "Autism" after the evaluation in our family medicine clinic.

Objective: Aim of our study is showing the changes of factors that affecting early childhood obesity from 2016 to 2019 and to ensure that necessary measures are taken about this regard. Method: 3 years follow-up information of 388 babies whom born in 2013 and 2016 and registered at the Family Health Centers of Rize (Turkey) included to our study. The family doctors participating in this study examined these babies who were registered to them during their 1st, 3rd, 7th, 9th, 12th, 18th, 24th, 30th, 36th months at the family health centers in accordance with Turkish Health Ministry protocols and recorded their height, weight, head circumference, and BMI (Body Mass Index). Results: 388 babies included in study, 177 of them were born in 2013 and 211 of them were born in 2016. Percentage of being overweight or obese was 19.2% in babies born in 2013 and this rate decreased to 18.2% in babies born in 2016. Percentage of being overweight or obese was 16% in girls and 21.2% in boys. We observed that high birth weight (>4000 gr) and excess gestational weight gain are risk factors for being overweight or obese at the age of three. (p=0,048) Conclusion: Result of our study showed us that although prevalence of early childhood obesity decreased, but prevalence is still at critical level. We found that gestational weight gain and high birth weight are risk factors for early childhood obesity. In future obesity prevention studies, taking these risk factors into account will be beneficial.

Background/Objectives: Metabolic syndrome (MetS) and dry eye disease (DED) share inflammatory mechanisms, yet few studies have examined objective tear function and serum inflammatory markers together in this population. The aim of this study was to compare tear function tests and a panel of systemic inflammatory indices between patients with MetS who did and did not have DED. Methods: This observational comparative study enrolled 104 adults with MetS (52 MetS-DED and 52 MetS-nonDED), with group-level comparability for age and sex. All participants underwent ophthalmologic evaluation, including the Ocular Surface Disease Index (OSDI), the Schirmer I test, and fluorescein tear break-up time (TBUT). DED was defined by a mean Schirmer I value of ≤5 mm/5 min. Routine blood tests were used to derive the neutrophil-, platelet-, monocyte-, and eosinophil-to-lymphocyte ratios (NLR, PLR, MLR, ELR), the neutrophil–lymphocyte–platelet ratio (NLPR), the C-reactive protein (CRP)-to-albumin ratio, and the monocyte-to-high density lipoprotein cholesterol (HDL) ratio. Between-group comparisons, receiver operating characteristic (ROC) analysis with Youden’s index, and age- and sex-adjusted binary logistic regression were performed. Results: TBUT and Schirmer I values were significantly lower in the MetS-DED group, whereas OSDI scores did not differ between the groups. NLR, MLR, NLPR, CRP, and the CRP-to-albumin ratio were significantly higher in MetS-DED patients (all p < 0.05), while classic metabolic parameters were comparable. ROC analysis revealed statistically significant but weak-to-modest discriminatory ability (areas under the curve (AUCs) 0.617–0.652). In age- and sex-adjusted models, MLR (odds ratio (OR) = 1.072 per 0.01 unit, p = 0.046), the CRP-to-albumin ratio (OR = 1.062 per 0.01 unit, p = 0.011), and NLPR (OR = 1.123 per 0.001 unit, p = 0.038) remained significantly associated with MetS-DED. Conclusions: Among individuals with MetS, the presence of DED was accompanied by impaired tear function and a more pronounced systemic inflammatory profile despite similar metabolic parameters. However, the modest discriminatory performance and borderline adjusted associations indicate that these findings should be regarded as preliminary signals rather than evidence supporting the clinical use of these markers for screening or risk identification.
