Hepatitis C virus (HCV) genotype distribution varies across geographic regions and populations and may change over time. This study examined the current distribution and temporal trends of HCV genotypes, and their patterns by demographic characteristics and nationality, at a tertiary care hospital in Istanbul, Türkiye. We retrospectively evaluated 188 patients who underwent HCV genotyping between June 2019 and December 2024. Genotype distribution was assessed according to age, sex, and year, while temporal trends were analyzed using the Cochran–Armitage trend test and binomial logistic regression. GT1b was the predominant subtype (54.8%), followed by GT3 (19.1%), GT1a (10.1%), and GT4 (5.9%); mixed-genotype results were detected in 5.9% of cases. Genotype distribution differed significantly by age group and sex (Monte Carlo p = 0.044 and p = 0.002, respectively), but not by year (p = 0.128). After Holm adjustment, GT1a was more frequent among males, whereas GT1b was more frequent among females. No significant linear temporal trends were observed for GT1a, GT1b, GT3, GT4, or mixed-genotype results. Genotype patterns also varied descriptively across nationality groups. These findings underscore the importance of up-to-date regional data and continued genotype surveillance for monitoring changes in HCV epidemiology.

In emergencies and natural disasters, an effective early warning and surveillance system is critical for rapid detection and effective response. Early warning systems involve timely data collection, risk analysis, and notification to relevant institutions to combat biological, chemical, environmental, and radiological threats. The Early Warning and Response System (EUCS) includes the application of syndromic or biological surveillance, preparedness, the creation of risk maps, risk assessment, risk management, and coordination at national and international levels. For quick data collection and analysis within EUCS, different surveillance systems need to be integrated into a national and international surveillance network. Surveillance systems play a significant role in identifying infectious diseases and intervening before they spread. Integrated surveillance is a system in which existing personnel resources and surveillance capacities are combined to make it stronger, more efficient, and cost-effective. Establishing an integrated surveillance system and enabling quick responses through multi-source data sharing are crucial for controlling public health events arising from global epidemics and natural disasters.Laboratory surveillance is an important component of epidemic surveillance and the early warning network. Routine surveillance of notifiable infectious diseases in laboratories is a part of the national surveillance system and holds critical importance. Laboratory surveillance facilitates the monitoring of pathogens, vectors, reservoir hosts, and community immunity, enabling the early detection and verification of outbreaks, tracing infection sources, and determining transmission routes.

Carbapenem resistance driven by carbapenemase production in Gram-negative bacteria is a global threat. Carbapenem-resistant Acinetobacter baumannii (CRAb) is a critical priority pathogen listed by the World Health Organization (WHO). While bla OXA-23 and bla OXA-24/40-like are most frequently reported in CRAb, an increasing prevalence of bla NDM has been noted in recent years. This study aimed to investigate the distribution of carbapenemase genes among CRAb isolates obtained from three tertiary care centres in Türkiye. A total of 336 CRAb isolates recovered from various clinical specimens (blood, respiratory samples, wound/tissue swabs, urine, and catheter tip samples) between January 2023 and October 2024 were included (Centre-1:198; Centre-2:86; Centre-3:52). Antimicrobial susceptibility testing was performed using the VITEK2 or BD Phoenix system, with additional tests carried out using the disc diffusion or broth microdilution methods. The results were interpreted in accordance with EUCAST criteria. The presence of bla OXA-51 , bla OXA-23 , bla OXA-24/40-like , bla OXA-58 , bla VIM , bla IMP , bla NDM and bla KPC was tested by qPCR. The clonal relationship among the isolates producing metallo-β-lactamase (MBL) was assessed using AP-PCR and PFGE methods. Our study detected bla OXA-23 in 305 isolates (90.8%), bla OXA-24/40-like in 88 (26.2%), bla NDM in 21 (6.3%), and bla VIM in one isolate. However, bla OXA-58 , bla IMP and bla KPC were not detected. Dual gene combinations were observed in 73 isolates, while triple combinations were identified in three isolates (two bla NDM / bla OXA-23 / bla OXA-24/40-like , one bla VIM /bla OXA-23 / bla OXA-24/40-like ). AP-PCR/PFGE demonstrated a clonal relationship among 18 isolates, all originating from a single intensive care unit at Centre-1. Because carbapenem resistance is endemic in this unit, the cluster was indistinguishable from the background resistant population and was detected only retrospectively by molecular typing; we therefore describe it as a silent outbreak. In this study bla OXA-23 and bla OXA-24/40-like were the predominant carbapenemase genes in CRAb, with bla NDM detected at a notable frequency. The detection of combined gene profiles and inter-centre differences highlight the potential impact of local antibiotic use and infection control practices on the dissemination of resistance, and the clonal cluster identified here shows that such dissemination can go unrecognised in endemic settings.
