This study aims to present the importance of a multidisciplinary approach to radiofrequency ablation (RFA) treatment in osteoid osteoma (OO) patients by a team of experts in their field in preventing recurrence and complications. For this retrospective study, a team of two orthopedists, two interventional radiologists, and one anesthesiologist was established in January 2013 to manage the diagnosis, follow-up, and treatment process of patients with OO at Bakırköy Dr. Sadi Konuk Training and Research Hospital. A total of 27 patients (15 males, 12 females; mean age 22.9 years; range, 9 to 54 years) were treated by this team between February 2013 and September 2016. The anatomic localization included iliac crest in four patients, the femur in 12 patients, fibula in two patients, humerus in three patients, radius in one patient, tibia in three patients, talus in one patient, and metacarpal in one patient. The procedures were carried out by the same interventional radiologists, same orthopedic surgeons, and same anesthesiologist in the computed tomography (CT) unit under aseptic conditions. After appropriate anesthesia for the localization of OO, the patient was positioned on the CT bed and the localization of the lesion was confirmed with a CT scan mapping. Then, a bone penetration cannula was advanced and bone cortex was penetrated with a charged motor and Kirschner (K)-wire. When the cannula reached the nidus, it was replaced with RFA probe. Ablation of the nidus was performed for five minutes at 90°C. The mean follow-up period was 46 months (range, 25 to 66 months). Patients were evaluated with visual analog scale (VAS) scores preoperatively and at postoperative 15th day, sixth month, and first year. In the last evaluation of the study data, the patients were called by telephone and questioned whether there were any changes in their final status. The mean preoperative VAS score was 7.2. The mean postoperative VAS scores of the 15th day, sixth month, and first year were 1.3, 0.6, and 0, respectively. In the last follow-up, the OO-related pain completely disappeared and none of the patients had any recurrence. There was a significant difference between preoperative and postoperative 15th day and sixth month VAS score measurements. Radiofrequency ablation treatment of OOs is a minimally invasive, safe, low-cost, and efficient method. We believe that with experienced teams and appropriate planning, RFA will take part in practice as the standard treatment of OO.

This study aims to present the results of 21 patients with capitellum fractures treated with open reduction and headless screws by a single experienced surgeon. Twenty-one patients (13 males, 8 females; mean age 39 years; range, 18 to 63 years) who were admitted to our clinic between June 2011 and January 2018 with the diagnosis of capitellum fracture and followed-up for a mean period of 45 months (range, 12 to 90 months) were included in this retrospective study. The fractures were fixed with headless cannulated screws by a single surgeon. The mean range of motion was 102° (range, 65° to 140°) during flexion-extension and 165° (range, 130° to 180°) during supination-pronation. The mean preoperative visual analog scale (VAS) score was 8.5 (range, 6 to 10), whereas the mean postoperative VAS score was 2.2 (range, 0 to 6). According to the Mayo Elbow Performance score, nine patients were evaluated as excellent, six patients as good, four patients as fair, and two as poor. The mean Quick-Disabilities of the Arm, Shoulder and Hand score was 25.1 (range, 4 to 57). Avascular necrosis developed in three patients (14%) and heterotopic ossification was detected in one patient (4%). Capitellum fractures are difficult to diagnose and treat, and good results can only be achieved by an accurate diagnosis, careful surgical technique, and stable fixation. Larger and more comprehensive studies are required to establish a generalization and more accurate inferences on this limitedly studied subject.

Background/aim: Trans-scaphoid perilunate fracture-dislocation (TSPFD) is a rare injury. TSPFD is a fracture-dislocation that severely disrupts the anatomical structure of the carpal bones and may occur as a result of a high energy trauma of the wrist or a fall on an open hand. In this study, the aim is to provide midterm clinical and radiological evaluations of cases diagnosed and treated as TSPFD. Materials and methods: Eleven patients diagnosed with TSPFD as a result of wrist trauma were treated surgically and were analysed retrospectively. Clinical and radiological follow-up of the cases was evaluated. The mean age of the patients was 34 years. All patients were males with a dorsal dislocation according to Herzberg's perilunate fracture-dislocation classification. The mean follow-up time was 33 months. All of the cases were evaluated with preoperative and postoperative standard wrist anteroposterior and lateral radiographs. A dorsal approach was used in all cases. However, in 1 case a volar approach was also required. The Green and O'Brien evaluation scale modified by Cooney was used for the clinical assessment of pain, wrist range of motion, grip strength, and functional status as excellent, good, moderate, or poor. The wrist range of motion was evaluated goniometrically at the final check-up, and a mid-grade disability was observed compared with the uninjured side. A visual analogue scale was used to evaluate the pain. Results: Sufficient union was obtained in all cases with open reduction and internal fixation of the fractures. Grip strength was up to 77.5% of the other side. According to the modified Green and O'Brien clinical evaluation scale, 6 cases were evaluated as good, 3 cases were fair, and 2 cases were poor. No median nerve damage was determined preoperatively or postoperatively and there was no postoperative pin tract infection in any of the patients. Conclusion: This kind of injury represents complex biomechanical damage of the wrist anatomy. If it is diagnosed early and treated with open reduction and stable fixation, a functionally adequate and anatomically integrated wrist can be achieved.

Proximal tibiofibular dislocation (PTFD) is a rare injury. In this article. we report a case of a PTFD, complicated with an intraarticular injury, a bucket-handle tear of the lateral meniscus. A 37-year-old male patient presented to the emergency department with pain and swelling on the lateral side of his right knee due to lateral trauma to the fibula during external rotation of the leg while his foot was on the ground. Closed reduction was performed under sedation. The lateral meniscus was repaired, and the fibular head was fixed with a cannulated screw. The patient had no pain in the tibiofibular joint after eight weeks postoperatively. Results of magnetic resonance imaging for the diagnosis of PTFD should be kept in mind during the presentation of sports trauma patients that experience difficulty in bearing weight on the affected extremity.

INTRODUCTION: The aim of this study to present the results of patients with iliac wing autograft using the mosaicplasty method in order to reduce donor site morbidity and pain, which are two of the most common complications. METHOD: Between 2011-2018, 35 patients (19 men,16 women) who were harvested autograft from the iliac wing were included in the study. The average age of patients was determined to be 42 (10-64) years, the mean follow-up was 39.9 months (12-101). All patients were operated on for pseudoarthrosis surgery. The same orthopedic surgeon harvested all autografts. The patients were evaluated at post-op 15th day, the first month, and the sixth month. Patients were evaluated in their last follow-up (at sixth month) and monofilament test, two-point discrimination test, visual analog scale (VAS), pain duration, numbness, gait problems, major pain area, cosmetic satisfaction were questioned. RESULTS: The mean of the monofilament test was 4.16 (2.83-6.65). The mean two-point discrimination test was 36.5 mm (9-100 mm). The mean VAS was found to be 2.94 (1-4). In the post-op period, the duration of pain was determined as one month in 21 patients, two months in 5 patients, and four months in 2 patients, while seven patients did not complain of pain at all. It was observed that ten patients complained of numbness in the thigh region (28.5%), and 11 patients complained of gait problem and limping (31.4%). Fifteen patients used an assistive walking device after surgery (42.8%). Only two patients complained of pain in the graft area when the major pain region was questioned after surgery (5.7%). Twenty-one patients were found to be cosmetically satisfied (60%) following the surgery scar in the graft region. CONCLUSİON: We believe that iliac autografts taken with the mosaicplasty technique can be used safely in suitable patients with low complication rates and high patient satisfaction
