Background. Current biomarkers have been routinely used noninvasive methods for assessing disease activity of inflammatory bowel disease (IBD), but none of them are specific. This study was aimed to determine the performance of the serological biomarkers for detecting disease activity in patients with IBD. Methods. A prospective study that included 73 ulcerative disease (UC) subjects, 141 Crohn’s disease (CD) subjects, and 30 of them complicated with C. difficile infection (CDI) were diagnosed at a single-institution IBD center. Disease activity was assessed using by Truelove and Witts criteria for UC and Harvey Bradshaw Simple Index for CD. Serological inflammatory biomarkers were compared in different severity groups. Receiver operator curve analyses assessed the performance of each biomarker in discriminating disease states. Results. For UC patients, elevated monocyte counts, C-reactive protein (CRP), and decreased lymphocyte counts and lymphocyte/monocyte ratio (LMR) significantly differed between subjects with active and inactive UC. LMR of 3.1 was 76% sensitive and had a specificity of 67% for active UC. For CD patients, higher values of neutrophils, monocytes, neutrophil/lymphocyte ratio, CRP, fibrinogen, and lower values of LMR and hemoglobin were significantly different between subjects with active and inactive CD. None of the biomarkers included had a good correlation with disease activity (area under the ROC Curve < 0.70). Conclusions. A low LMR represents an inexpensive, readily available test with a promising value to identify disease activity in UC patients, whereas none of the inflammatory biomarkers showed a discriminative value in disease activity of CD.

Non-alcoholic fatty liver disease (NAFLD) is the most common chronic liver disease, resulting in a huge medical burden worldwide. Accumulating evidence suggests that the gut microbiome and bile acids play pivotal roles during the development of NAFLD. Patients with NAFLD exhibit unique signatures of the intestinal microbiome marked by the priority of Gram-negative bacteria, decreased ratio of Firmicutes/Bacteroidetes (F/B), and increased Prevotella and Lachnospiraceae. The intestinal microbiota is involved in the metabolism of bile acids. Ursodeoxycholic acid (UDCA) is a key determinant in maintaining the dynamic communication between the host and gut microbiota. It generally shows surprising therapeutic potential in NAFLD with several mechanisms, such as improving cellular autophagy, apoptosis, and mitochondrial functions. This action is based on its direct or indirect effect, targeting the farnesoid X receptor (FXR) and various other nuclear receptors. This review aims to discuss the current studies on the involvement of the microbiome–UDCA interface in NAFLD therapy and provide prospective insights into future preventative and therapeutic approaches for NAFLD.

Background Ferroptosis and inflammation are involved in cancer progression. The aim of this study was to identify inflammation-associated ferroptosis regulators in hepatocellular carcinoma (HCC). Methods FerrDb database was searched for ferroptosis-related genes. RNA sequencing data and clinicopathologic information of HCC patients were downloaded from the Cancer Genome Atlas (TCGA) database. Weighted gene co-expression network analysis was applied to obtain the genes probably involved in inflammation-associated ferroptosis. Univariate Cox regression analysis was conducted to screen prognostic genes, and 10 machine learning algorithms were combined to find the optimal strategy to evaluate the prognosis of the patients based on the prognosis-related genes. The patients were divided into high risk group and low risk group, and the differentially expressed genes were obtained. Thymosin beta 4 X-linked (TMSB4X) was overexpressed or knocked down in HCC cell lines, and then qPCR, CCK-8, Transwell, flow cytometery assays were performed to detect the change of HCC cells’ phenotypes, and Western blot was used to detect the change of ferroptosis markers. Results 157 genes related to inflammation and ferroptosis in HCC were obtained by WGCNA. rLasso algorithm, with the highest C-index, screened out 29 hub genes, and this model showed good efficacy to predict the prognosis of HCC patients. The patients in high risk group and low risk groups showed distinct molecular characteristics. TMSB4X was the most important gene which dominated the classification, and it was highly expressed in HCC samples. TMSB4X promoted the viability, migration and invasion, and repressed ferroptosis of HCC cells. Conclusion The risk model constructed based on the inflammation-associated ferroptosis regulators is effective to predict the clinical outcome of HCC patients. TMSB4X, involved in inflammation-associated ferroptosis, is a potential biomarker and therapeutic target for HCC.

There is an increased incidence of colorectal cancer in young patients, however, the relationship between polyp characteristics and histology is not clearly understood. This study aimed to investigate the prevalence of different histological types of polyps in young patients and risk factors associated with advanced histology. Young patients (aged <45) who underwent polypectomy at Sir Run Run Shaw Hospital (2015–2017, Zhejiang, China) and West China Xiamen Hospital (2023–2024, Xiamen, China) were included. A database of endoscopes was accessed to classify polyps according to endoscopic features. The distribution of polyps and the risk factors associated with advanced histology were reported. The detection rate of polyps among the young adults was 20%, with 47.6% of adenoma. Hyperplastic polyps were second only to tubular adenomas in frequency. Of the 2776 polyps, nearly 85% were sessile, 29.4% were located in the sigmoid colon, and 25.4% were located in the rectum. Among polyps, 87.3% were <10 mm in diameter. Histological features of advanced adenoma were found in 5.3% of patients. Of these, 30.4% occurred in polyps <10 mm. Furthermore, 69.6% were distributed in the distal colorectum. Multivariate logistic regression analysis demonstrated that polyp size and morphology were independent predictors of advanced adenomas. In young patients, polyps are mainly located in the distal colorectum, with tubular adenoma being the predominant type. Large size and pedunculated morphology were independent predictors of advanced polyp histology in young patients. Nearly one-third of the advanced histology cases were associated with small polyps. It is recommended that endoscopists resect all neoplastic polyps found during colonoscopy, especially pedunculated polyps, and submit them for histology.
