Newborns have high risks of thrombotic and hemorrhagic complications. Despite the fact that the overall frequency of thrombosis and bleeding in the general population of neonates is low, the risks of both thrombosis and hemorrhage are significantly increased when a newborn has some complications, including prematurity. The mechanisms underlying the onset of thrombotic and hemorrhagic complications in newborns are not fully understood and remain controversial. The hemostasis in newborns drastically differs from adult hemostasis and even from hemostasis in children older than a year. Nevertheless, despite the presence of quantitative and qualitative differences of almost all parameters of the hemostasis system from the parameters of adults, healthy newborns as a whole have clinically normal functional hemostasis without a tendency to coagulopathy or thrombosis. Apparently, the neonatal hemostasis system is in some alternative "balance", which differs from the "balance" of hemostasis in adults. The issue regarding the stability of this balance is still open. Due to the peculiarities of the newborn's hemostasis, clinical laboratory diagnostics of the coagulation disorders is very difficult, and the attending physician is forced to focus exclusively on the clinical picture. This review provides basic information on the neonatal hemostasis system, as well as an attempt to critically evaluate existing laboratory tests in terms of applicability for this group of patients.

Background. Cryoprecipitate is made from fresh-frozen plasma (FFP) and contains fibrinogen, factor VIII, factor XIII, von Willebrand factor, fibronectin and fibrinogen.Aim. To provide information on the composition and methods of production, storage, transportation and clinical use of cryoprecipitate.General findings. Cyoprecipitate is manufactured by slowly thawing FFP at 1–6°C. This precipitates out cryoproteins: factor VIII, von Willebrand factor, factor XIII, fibronectin and fibrinogen. After centrifugation, the cryoproteins are resuspended in a reduced volume of plasma. Cryoprecipitate is stored at temperatures not exceeding –25° С for 36 months. Indications for cryoprecipitate transfusion are hemophilia A, von Willebrand disease, factor XIII deficiency, congenital afibrinogenemia and hypofibrinogenemia, acquired hypofibrinogenemia. These indications can occur in obstetrics, neonatology, cardiac surgery, neurosurgery, hematology, orthopaedics, and general surgery during liver transplantation and disseminated intravascular coagulation.

Neonatal sepsis is one of the most severe diseases of the neonatal period with a high probability of developing multiple organ dysfunction, septic shock and an unfavorable outcome. The purpose of this publication is to present draft federal clinical guidelines for the treatment of sepsis in newborns, which were developed taking into account the principles of evidence-based medicine. During the preparation of clinical guidelines, key questions were formulated using the PIPOH formula: P (population) — patient population and disease characteristics (neonate with sepsis and septic shock); I (interventions) — intensive care measures; P (professionals) — target audience of clinical recommendations (anesthesiologists, intensive care physicians, neonatologists, pediatricians, surgeons, infectious disease doctors); O (outcomes) — expected outcomes that can be improved (patient outcomes, health system indicators, epidemiological indicators, etc.); H (health care setting) — place of medical care (outpatient clinic, hospital, intensive care unit). The project includes sections on the etiology, pathogenesis and diagnosis of sepsis in newborns. Particular attention is paid to the issues of microbiological diagnostics, identification of the pathogen, antibacterial therapy, respiratory and nutritional support, extracorporeal hemocorrection, the use of intravenous immunoglobulins. It has been demonstrated that one of the mandatory elements of therapy is the early initiation of enteral nutrition using breast milk, the use of which is one of the most effective methods for the prevention of late sepsis in premature newborns with low and extremely low birth weight.

Perinatal infections occupy a leading place among the causes of neonatal morbidity, maternal and perinatal mortality. Infections are among the main causes of termination of pregnancy and premature birth. The practical recommendations presented in the work are intended for doctors of obstetric institutions in order to make a clinical diagnosis of an infection specific to the perinatal period, the tactics of examination and treatment of newborn children. The clinical recommendations correspond to the latest scientific data on the topic, contain information that is applied to the practical activities of a neonatologist, intensive care specialist and pediatrician. These clinical recommendations contain information about infections specific to the perinatal period, including the definition, frequency of occurrence, etiology of infections, pathogenetic mechanisms of disease development Numerous high-risk factors for infection of the fetus and newborn are described in detail. The document discusses and proposes the classification of the disease, the criteria for the adoption of the diagnosis. The features of the clinical picture of the disease are described, it is noted that the inflammatory process in a newborn child can be localized in any organ or acquire a systemic (generalized) character, in some cases, the ingress of an infectious agent into a macroorganism is not necessarily accompanied by clinical manifestations, which indicates an asymptomatic or subclinical course of infection. The recommendations provide advanced laboratory and instrumental diagnostics. The stages of treatment are described, including the choice and correction of antibacterial therapy, taking into account the peculiarities of the mother’s anamnesis, the child’s gestation period and the etiology of the disease. These clinical recommendations have been prepared taking into account the level of credibility of the recommendations and the level of reliability of the evidence. These practical recommendations are offered for public discussion and are posted in full on the website of the Ministry of Health of the Russian Federation.
