The objective of this study is to substantiate the system of indicators for assessing behavioral risk factors (tobacco use, alcohol consumption, detrimental diet patterns, lack of physical activity) and their impact on public health in the Russian Federation based on the authors’ methodology for assessing the impact of behavioral risk factors on the health of the adult population using the materials of the statistical survey of the health status of the population (conducted by Rosstat in 2021). For each risk factor, two main indicators were calculated: the prevalence of the risk factor and its impact on public health according to demographic characteristics. The scale of such a risk factor as smoking was assessed in terms of consumption of various types of tobacco and nicotine-containing products and the prevalence of nicotine addiction of varying severity. Alcohol consumption as a behavioral risk factor was assessed in the context of the relationship between the amount of consumption of various alcoholic products and the degree of health risk. The impact of a detrimental diet was estimated by the body mass index for overweight and obesity, with the account to the physical activity of Russian citizens. According to the authors, the proposed set of indicators and specific calculation results can be used to organize continuous monitoring of the prevalence of behavioral risk factors and their impact on the health of the population of the Russian Federation. As a result, a more effective response of the healthcare system and strengthening of interagency cooperation to reduce non-communicable diseases is possible.

The article is devoted to the analysis of the results of the global surveys among youth 13–15 years old on tobacco consumption, conducted in 2004, 2015, 2021 in Khabarovsk, Novosibirsk, Cheboksary, Moscow, Pskov and the Russian Federation. The dynamics and trends in consumption of tobacco, electronic nicotine delivery systems (ENDS) and heated tobacco products (HTPs) are analyzed. Materials and methods. The surveys used representative samples formed on the basis of all public schools. In 2004, 2015 and 2021 regional sampling included two stages. In 2021 national sampling included three stages: the selection of municipalities, schools and classes. All students of the selected classes were asked. The surveys used the same questionnaire. The total number of respondents – 38 345, the overall response rate – more than 90%. Results. In the Russian Federation, in 2004–2021 among students aged 13–15, the prevalence of tobacco consumption decreased from 27.3 to 11.0%, passive smoking in homes from 76.4 to 23.1%, in closed public places from 89.4 to 23.7%. The consumption of ENDS and HTPs increased up to 16.7 and 3.5% respectively. Similar trends in consumption of products, passive smoking were identified in all the cities. Conclusion. The adoption in 2013 of the Federal Law "On protecting the health of citizens from the effects of second hand tobacco smoke, the consequences of tobacco consumption or the consumption of nicotine-containing products" had a significant impact on tobacco use by adolescents and reduced the prevalence of tobacco consumption and passive smoking among them by 3 times.

Introduction. At the end of 2020, the clinical cure rate in all registration groups of patients with pulmonary tuberculosis was 48.2 %. There is now ample evidence that additional risk factors influence the more severe course of pulmonary tuberculosis and reduce the effectiveness of standard anti-tuberculosis therapeutic programs. Diabetes mellitus and tobacco use are among the significant risk factors. Complementing standard TB therapeutic programs with nicotine addiction and diabetes mellitus treatment programs with the formation of an individual comprehensive patient treatment plan will increase the effectiveness of TB treatment. Thus, the goal of this work is to demonstrate the high effectiveness of an individualized approach to the treatment of a patient with pulmonary tuberculosis with a comorbid background. The aim of the work was to demonstrate the high effectiveness of an individualized approach to the treatment of a patient with pulmonary tuberculosis with a comorbid background.Materials and methods At the Central TB Research Institute the patient underwent a comprehensive examination, including mandatory, additional and facultative methods of investigation. Based on the findings, the patient underwent complex treatment: antituberculosis chemotherapy, correction of carbohydrate metabolism disorders using oral antidiabetic drugs, nicotine replacement therapy according to the selected scheme, taking into account the degree of nicotine addiction. Results and Discussion Against the background of complex treatment, by four months of therapy the following was achieved: knockdown of systemic inflammatory reactions, cessation of bacterial excretion, compensation of diabetes mellitus. X-ray examination revealed closure of a large decay cavity, resorption and consolidation of focal infiltrative changes. There was regression of infiltrative changes of trachea and bronchi. The patient completely stopped using tobacco due to nicotine replacement therapy. Conclusion This clinical example demonstrates the high effectiveness of an individualized approach to the treatment of tuberculosis, including the need for an individualized regimen of anti-tuberculosis therapy, adequate compensation for diabetes mellitus, and the implementation of relevant smoking cessation techniques among smokers with pulmonary tuberculosis.
