These days human beings are facing many environmental challenges due to frequently occurring drought hazards. It may have an effect on the country’s environment, the community, and industries. Several adverse impacts of drought hazard are continued in Pakistan, including other hazards. However, early measurement and detection of drought can provide guidance to water resources management for employing drought mitigation policies. In this paper, we used a multilayer perceptron neural network (MLPNN) algorithm for drought forecasting. We applied and tested MLPNN algorithm on monthly time series data of Standardized Precipitation Evapotranspiration Index (SPEI) for seventeen climatological stations located in Northern Area and KPK (Pakistan). We found that MLPNN has potential capability for SPEI drought forecasting based on performance measures (i.e., Mean Average Error (MAE), the coefficient of correlation (), and Root Mean Square Error (RMSE)). Water resources and management planner can take necessary action in advance (e.g., in water scarcity areas) by using MLPNN model as part of their decision-making.
The epidemic of opioid addiction is shaping up as the most serious clinical issues of current times. Opioids have the greatest propensity to develop addiction after first exposure. Molecular, genetic variations, epigenetic alterations, and environmental factors are also implicated in the development of opioid addiction. Genetic and epigenetic variations in candidate genes have been identified for their associations with opioid addiction. OPRM1 nonsynonymous single nucleotide polymorphism rs1799971 (A118G) is the most prominent candidate due to its significant association with onset and treatment of opioid addiction. Marked inter-individual variability in response to available maintenance pharmacotherapies is the common feature observed in individuals with opioid addiction. Several therapies are only effective among subgroups of opioid individuals which indicate that ethnic, environmental factors and genetic polymorphism including rs1799971 may be responsible for the response to treatment. Pharmacogenetics has the potential to enhance our understanding around the underlying genetic, epigenetic and molecular mechanisms responsible for the heterogeneous response of maintenance pharmacotherapies in opioid addiction. A more detailed understanding of molecular, epigenetic and genetic variants especially the implication of OPRM1 A118G polymorphism in an individual may serve as the way forward to address the opioid epidemic. Personalized medicine, which involves developing targeted pharmacotherapies in accordance with individual genetic and epigenetic makeup, are required to develop safe and effective treatments for opioid addiction.
Accurate prediction of hydrological processes is key for optimal allocation of water resources. In this study, two novel hybrid models are developed to improve the prediction precision of hydrological time series data based on the principal of three stages as denoising, decomposition, and decomposed component prediction and summation. The proposed architecture is applied on daily rivers inflow time series data of Indus Basin System. The performances of the proposed models are compared with traditional single‐stage model (without denoised and decomposed), the hybrid two‐stage model (with denoised), and existing three‐stage hybrid model (with denoised and decomposition). Three evaluation measures are used to assess the prediction accuracy of all models such as Mean Relative Error (MRE), Mean Absolute Error (MAE), and Mean Square Error (MSE). The proposed, three‐stage hybrid models have shown improvement in prediction accuracy with minimum MRE, MAE, and MSE for all case studies as compared to other existing one‐stage and two‐stage models. In summary, the accuracy of prediction is improved by reducing the complexity of hydrological time series data by incorporating the denoising and decomposition.
Objectives: To develop a logistic regression model to predict the risk of sepsis following emergency medical admission using the patient’s first, routinely collected, electronically recorded vital signs and blood test results and to validate this novel computer-aided risk of sepsis model, using data from another hospital. Design: Cross-sectional model development and external validation study reporting the C-statistic based on a validated optimized algorithm to identify sepsis and severe sepsis (including septic shock) from administrative hospital databases using International Classification of Diseases, 10th Edition, codes. Setting: Two acute hospitals (York Hospital - development data; Northern Lincolnshire and Goole Hospital - external validation data). Patients: Adult emergency medical admissions discharged over a 24-month period with vital signs and blood test results recorded at admission. Interventions: None. Main Results: The prevalence of sepsis and severe sepsis was lower in York Hospital (18.5% = 4,861/2,6247; 5.3% = 1,387/2,6247) than Northern Lincolnshire and Goole Hospital (25.1% = 7,773/30,996; 9.2% = 2,864/30,996). The mortality for sepsis (York Hospital: 14.5% = 704/4,861; Northern Lincolnshire and Goole Hospital: 11.6% = 899/7,773) was lower than the mortality for severe sepsis (York Hospital: 29.0% = 402/1,387; Northern Lincolnshire and Goole Hospital: 21.4% = 612/2,864). The C-statistic for computer-aided risk of sepsis in York Hospital (all sepsis 0.78; sepsis: 0.73; severe sepsis: 0.80) was similar in an external hospital setting (Northern Lincolnshire and Goole Hospital: all sepsis 0.79; sepsis: 0.70; severe sepsis: 0.81). A cutoff value of 0.2 gives reasonable performance. Conclusions: We have developed a novel, externally validated computer-aided risk of sepsis, with reasonably good performance for estimating the risk of sepsis for emergency medical admissions using the patient’s first, electronically recorded, vital signs and blood tests results. Since computer-aided risk of sepsis places no additional data collection burden on clinicians and is automated, it may now be carefully introduced and evaluated in hospitals with sufficient informatics infrastructure.
