Background: Osteoarthritis (OA) is a chronic, debilitating, and degenerative joint disease. However, it is difficult for patients with knee OA to access conventional rehabilitation when discharging from the hospital. Internet-based rehabilitation is one of the promising telemedicine strategies to provide a means combining monitoring, guidance, and treatment for patients with knee OA. Objective: The aim of this study was to conduct a systematic review and meta-analysis for assessing the effect of internet-based rehabilitation programs on pain and physical function in patients with knee OA. Methods: Keywords related to knee OA and internet-based rehabilitation were systematically searched in the Web of Science, MEDLINE, EMBASE, CENTRAL, Scopus, PEDro (Physiotherapy Evidence Database), CNKI, SinoMed, and WANFANG databases from January 2000 to April 2020. Only randomized controlled trials were included. The authors independently screened the literature. The main outcome measures were focused on pain and physical function. A meta-analysis was performed on the collected data. Review Manager (RevMan, version 5.3) was used for all analyses. Results: The systematic review identified 6 randomized controlled trials, 4 of which were included in the meta-analysis, comprising a total of 791 patients with knee OA. The meta-analysis with the fixed-effects model showed that the internet-based rehabilitation programs could significantly alleviate the osteoarthritic pain for patients compared with conventional rehabilitation (standardized mean difference [SMD] –0.21, 95% CI −0.4 to –0.01, P=.04). No significant difference was found in the improvement of physical function in patients with knee OA compared with conventional rehabilitation within 2 to 12 months (SMD –0.08, 95% CI −0.27 to 0.12, P=.43). Conclusions: This systematic review shows that internet-based rehabilitation programs could improve the pain but not physical function for patients with knee OA. However, there was a very small number of studies that could be included in the review and meta-analysis. Thus, further studies with large sample sizes are warranted to promote the effectiveness of internet-based rehabilitation and to develop its personalized design.

Clear cell renal cell carcinoma (ccRCC) is a prevalent tumor in the urinary system, presenting a poor prognosis yet being accompanied by a high degree of immune infiltration. Understanding the mechanisms underlying this abnormal infiltration and identifying prognostic biomarkers in this regard is crucial for improving therapeutic outcomes. The expression of GPD1L in ccRCC was analyzed using a common database (TCGA). The expression of GPD1L in ccRCC cell lines and tissue samples was verified by western blotting, real time qPCR and immunohistochemistry. The predictive value of GPD1L was evaluated by survival analysis, ROC curve and Cox regression analysis. We used GO, KEGG and gene set enrichment analysis (GSEA) to verify each other. Then the single cell sequencing dataset (GEO) was further analyzed and verified, and the functional phenotype of GPD1L in ccRCC was explored by functional experiments. In addition, the correlation between the expression level of GPD1L and drug resistance of AKT-mTOR pathway was analyzed based on Genomics of Drug Sensitivity in Cancer database (GDSC). We identified glycerol-3-phosphate dehydrogenase 1-like (GPD1L) as a tumor suppressor gene in ccRCC, and downregulation of GPD1L may facilitate the adaptive survival of tumor cells via enhanced regulatory T cells (Tregs) infiltration and lipid metabolism reprogramming in ccRCC. Our results suggest that there is a significantly diminished GPD1L in ccRCC patients with poorer survival probability. Mechanically, a significant negative correlation between GPD1L expression and Tregs infiltration, and GPD1L-related metabolic analysis reflected the correlation between Tregs and lipid metabolism. In addition, GPD1L expression levels also influence the malignant phenotype of ccRCC and the drug resistance to AKT and mTOR targeted therapy. Taken together, our results supported GPD1L could be a valuable biomarker for predicting and intervening in ccRCC progression. These insights could shed light on the complex interplay between tumor cell adaptive survival and Treg infiltration, which reflected that the comprehensive and systemic role of GPD1L in ccRCC.

Objective: Many publications on telerehabilitation for hip or knee arthroplasty have been published in recent years. However, no specific studies have attempted to characterize research hotspots, global research collaborations, or trends related to telerehabilitation after hip or knee arthroplasty. Therefore, the aim of this bibliometric analysis was to provide an overview of the current status of research and map the research landscape on telerehabilitation for joint replacement to understand current trends, identify research gaps, and guide future research directions. Methods: The Web of Science Core Collection and PubMed were comprehensively searched to identify all relevant English‐language documents published from 2003 to June 7, 2024. Data from these published studies were then cleaned and structured. CiteSpace and VOSviewer were used to conduct the bibliometric visualization and comparative analysis of countries, institutions, journals, authors, references, and keywords. Then, the map illustrating the research hotspots and knowledge structure was plotted based on the analysis results. Results: A total of 229 records were obtained, and the number of articles published has increased steadily over the investigated period. The largest increase was observed in 2022. With the highest number of publications and centrality, the United States was the most influential country. The University of Sherbrooke was the most productive institution. Author Boissy P. ranked first in terms of the number of publications, while Tousignant M. ranked highest in cited authors, with 7 publications and 65 citations. The Journal of Arthroplasty published the greatest number of articles, with 29 publications. The most popular keywords from 2018 to 2023 were “home telerehabilitation,” “older adults,” and “physical therapy”. In terms of the strongest citation burst, the top five keywords were associated with “total knee arthroplasty,” “in home tele rehabilitation,” “physical activity,” “motion,” and “range.” The frontier keywords were “patient satisfaction,” “mobile application,” “self‐efficacy,” “fear avoidance model,” “home assessment tool,” and “cost benefit analysis.”Conclusions: The current status and trends in telerehabilitation for hip or knee arthroplasty are presented. A major concern at present is physical therapy for home telerehabilitation in the elderly. In the future, mobile app–based telerehabilitation programs for arthroplasty will continue to be encouraged, and some outcomes, such as “patient satisfaction,” “self‐efficacy,” and “cost benefit analysis,” are expected to receive more attention. Our work will serve as a valuable resource, providing fundamental references and a directional guide for future research.
