Multi-label classification (MLC) is considered an essential research subject in the computer vision field, principally in medical image analysis. For this merit, we derive benefits from MLC to diagnose multiple grades of diabetic retinopathy (DR) from various colored fundus images, especially from multi-label (ML) datasets. Therefore, ophthalmologists can detect early signs of DR as well as various grades to initiate appropriate treatment and avoid DR complications. In this paper, we propose a comprehensive ML computer-aided diagnosis (CAD) system based on deep learning technique. The proposed system's main contribution is to detect and analyze various pathological changes accompanying DR development in the retina without injecting the patient with dye or making expensive scans. The proposed ML-CAD system visualizes the different pathological changes and diagnoses the DR grades for the ophthalmologists. First, we eliminate noise, enhance quality, and standardize the sizes of the retinal images. Second, we differentiated between the healthy and DR cases by calculating the gray level run length matrix average in four different directions. The system automatically extracts the four changes: exudates, microaneurysms, hemorrhages, and blood vessels by utilizing a deep learning technique (U-Net). Next, we extract six features, which are the gray level co-occurrence matrix, areas of the four segmenting pathology variations, and the bifurcation points count of the blood vessels. Finally, the resulting features were afforded to an ML support vector machine (SVM) based on a classifier chain to differentiate the various DR grades. We utilized eight benchmark datasets (four of them are considered ML) and six different performance evaluation metrics to evaluate the proposed system's performance. It achieved 95.1%, 91.9%, 86.1%, 86.8%, 84.7%, 86.2% for accuracy, area under the curve, sensitivity, specificity, positive predictive value, and dice similarity coefficient, respectively. The experiments show encouraging results as compared with other systems.

Designing computer-aided diagnosis (CAD) systems that can automatically detect ocular diseases (ODs) has become an active research field in the health domain. Although the human eye might have more than one OD simultaneously, most existing systems are designed to detect specific eye diseases. Therefore, it is crucial to develop new CAD systems that can detect multiple ODs simultaneously. This paper presents a novel multi-label convolutional neural network (ML-CNN) system based on ML classification (MLC) to diagnose various ODs from color fundus images. The proposed ML-CNN-based system consists of three main phases: the preprocessing phase, which includes normalization and augmentation using several transformation processes, the modeling phase, and the prediction phase. The proposed ML-CNN consists of three convolution (CONV) layers and one max pooling (MP) layer. Then, two CONV layers are performed, followed by one MP and dropout (DO). After that, one flatten layer is performed, followed by one fully connected (FC) layer. We added another DO once again, and finally, one FC layer with 45 nodes is performed. The system outputs the probabilities of all 45 diseases in each image. We validated the model by using cross-validation (CV) and measured the performance by five different metrics: accuracy (ACC), recall, precision, Dice similarity coefficient (DSC), and area under the curve (AUC). The results are 94.3%, 80%, 91.5%, 99%, and 96.7%, respectively. The comparisons with the existing built-in models, such as MobileNetV2, DenseNet201, SeResNext50, InceptionV3, and InceptionresNetv2, demonstrate the superiority of the proposed ML-CNN model.

Alopecia areata is an autoimmune form of non‐scarring hair loss Aim: to assess the serum concentration of IFN‐γ and CD8 cell expression in lesional skin biopsies in correlation with the disease severity, activity, duration and trichoscopic findings in patients with alopecia areata. Patients and methods: the study included 30 patients with AA and 15 age and sex matched healthy controls. Trichoscopy was performed and Photographs were captured for the alopecic areas, ELIZA technique was used for serum level of interferon‐gamma assessment and immunohistochemiistry for CD8 cells. Results: IFN‐γ serum level in patients was significantly higher compared to control subjects, and significantly correlated with the activity status and the duration of the disease. CD8+ T‐cells infiltrate intensity significantly correlated with sevrity. Yellow dots, vellus hair, black dot and exclamation marks were the most common trichoscopic findings. The presence of black dots significantly correlated to the disease activity, duration, serum IFN‐γ and CD8+ infiltrate intensity. The presence of yellow dots significantly correlated with the mean serum IFN‐γ level. Exclamation marks significantly correlated with the disease activity and the degree of CD8+ infiltrate. Conclusion: trichoscopy could be a reliable indicator of the IFN‐γ serum level and CD8 + T cell infiltrate intensity in AA patient.
