Background: Ocular biometric parameters can be influenced by race, ethnicity, and genetics; their differences across different populations can probably explain differences in refractive errors in these populations. The aim of this study is to determine the normal range of axial length, anterior chamber depth, lens thickness, and vitreous chamber depth in the population of Shahroud in the north of Iran. Methods: In the first phase of Shahroud Eye Cohort Study, the 40–64 year old population were sampled cross-sectionally; 6311 were invited and 5190 (82.2%) participated in the study. Biometric examinations were done using the LENSTAR/BioGraph (WaveLight AG, Erlangen, Germany) after vision tests and before cycloplegic refraction tests. Any type of eye surgery, extensive pterygium, and lack of cooperation were used as exclusion criteria, and analyses were done with data from 4869 eyes. Results: We found a mean axial length of 23.14 mm (95% confidence interval [CI], 23.11-23.17), mean anterior chamber depth of 2.62 mm (95% CI, 2.60-2.63), mean lens thickness of 4.28 mm (95% CI, 4.27-4.29), and the mean vitreous chamber depth was 15.72 mm (95% CI, 15.70-15.75). Kolmogorov-Smirnov tests showed that the distribution of axial length, anterior chamber depth, lens thickness, and vitreous chamber depth significantly differed from normal; axial length and vitreous chamber depth demonstrated a leptokurtic distribution as well. Axial length, anterior chamber depth, and vitreous chamber depth significantly decreased with age, and lens thickness significantly increased with age (p < 0.001). All indices were significantly higher in men. Conclusions: The distributions of axial length, vitreous chamber depth, and lens thickness are reported for the first time in an Iranian adult population. Compared to other studies, axial length was in the mid range, nonetheless, studying axial length components showed that the Iranian population had smaller anterior chamber depth and lens thickness. Age and gender were significantly associated with all indices assessed in this study.

The Shahroud Eye Cohort Study was set up to determine the prevalence and incidence of visual impairment and major eye conditions in the 40–64-year-old population of Shahroud as a Middle Eastern population. The first phase of the study was conducted in 2009–10. Using random cluster sampling, 6311 Shahroud inhabitants were invited for ophthalmologic examinations; of these, 5190 participants completed phase 1 (participation rate of 82.2%). All participants were interviewed to collect data on participants’ demographics, occupation status, socioeconomic status, history of smoking, and medical and ophthalmic history, as well as history of medication, and the quality and duration of their insurance. DNA and plasma samples, as well as four dots of whole blood were collected from participants. Extensive optometric and ophthalmologic examinations were performed for each participant, including lensometry of current glasses, testing near and far visual acuity; determining objective and subjective refraction; eye motility; cycloplegic refraction; colour vision test; slit-lamp biomicroscopy and intraocular pressure measurement; direct and indirect fundoscopy; perimetry test; ocular biometry; corneal topography; lens and fundus photography; and the Schirmer's (1008 participants) and tear breakup time tests (1013 participants). The study data are available for collaborative research at Noor Ophthalmology Research Center, Tehran, Iran.

To compare the 6-month results of accelerated and standard collagen crosslinking (CXL) treatment of progressive keratoconus. Noor Eye Hospital, Tehran, Iran. Prospective randomized clinical trial. Two groups of eyes (intervention and control) received corneal collagen crosslinking (CXL) treatment. The intervention group received accelerated CXL (18 mW/cm(2), 5 minutes), and the control group received standard CXL (3 mW/cm(2), 30 minutes). The eyes were evaluated for changes in the visual indices, refraction, and topography preoperatively and 1, 3, and 6 months postoperatively and regarding corneal rigidity indices and the endothelial cell count (ECC) preoperatively and at 6 months. The study evaluated 62 eyes (31 patient) in 2 groups. The mean changes in uncorrected (P = .733) and corrected (P = .646) distance visual acuities and manifest refraction spherical equivalent (P = .598) did not differ statistically significantly between the 2 groups. The central corneal thickness was higher in the standard group than the accelerated group (P = .025). The mean decrease in the maximum keratometry (K) (P = .865) and mean K (P = .974) and the mean changes in the asphericity (P = .272) were not statistically significantly different between the 2 groups. The mean changes in corneal hysteresis (CH) (P = .548) and the corneal resistance factor (CRF) (P = 1.000), CH-CRF (P = .282), and the area under the peak 2 (P = .260) were similar in both groups. The mean decrease in the ECC was not statistically significantly different between the 2 groups (P = .218). Based on 6-month results, accelerated and standard corneal CXL arrested the progression of keratoconus similarly. Background: By the lengthening of life span, the incidence of chronic diseases such as hypertension and uncontrolled hypertension has increased. This study aims to determine the prevalence of uncontrolled hypertension and its related factors in the age group of 50–74 years in Shahroud, northeast Iran. Methods: The data of the third phase of the Shahroud Eye Cohort Study were used in this study. This phase of the cohort study included 4394 participants aged 50 to 74 years from the previous phases. In addition to ophthalmological and optometric examinations, demographic characteristics, blood biochemistry tests, and blood pressure measurements were performed in this phase. Individuals with a blood pressure ≥ 140/90 mm/Hg (being treated or not treated with antihypertensive medicines) were defined as uncontrolled hypertension. In patients with diabetes and chronic kidney disease, blood pressure equal to or higher than 130/80 mm/Hg was considered uncontrolled hypertension. Descriptive statistics and multiple logistic regression were used to analyze the data. Findings: Overall, the prevalence of uncontrolled hypertension out of all the participants was 61.7% (95% CI: 60.3–63.2). Multiple regression results showed that the male gender (OR: 2.1, 95% CI: 1.5–2.9), patients with diabetes (OR:3.2, 95% CI: 2.4–4.3), and patients with chronic kidney disease (CKD) (OR: 3.2, 95% CI: 2.5–4.1) increased the risk of uncontrolled hypertension while in patients with cardiovascular disease (OR: 0.6, 95% CI: 0.4–0.8) and polypharmacy (OR: 0.2, 95% CI: 0.1–0.2) reduced the risk of uncontrolled hypertension. Conclusion: The present study showed that uncontrolled hypertension has a high prevalence, and factors such as male gender, diabetes, and CKD are associated with this disorder. So, it is recommended to take the necessary measures to formulate and implement immediate actions to prevent or control hypertension.

Visual impairment has a profound impact on society. The majority of visually impaired people live in developing countries, and since most disorders leading to visual impairment are preventable or curable, their control is a priority in these countries. Considering the complicated epidemiology of visual impairment and the wide variety of factors involved, region specific intervention strategies are required for every community. Therefore, providing appropriate data is one of the first steps in these communities, as it is in Iran. The objectives of this study are to describe the prevalence and causes of visual impairment in the population of Tehran city; the prevalence of refractive errors, lens opacity, ocular hypertension, and color blindness in this population, and also the familial aggregation of refractive errors, lens opacity, ocular hypertension, and color blindness within the study sample. Through a population-based, cross-sectional study, a total of 5300 Tehran citizens will be selected from 160 clusters using a stratified cluster random sampling strategy. The eligible people will be enumerated through a door-to-door household survey in the selected clusters and will be invited. All participants will be transferred to a clinic for measurements of uncorrected, best corrected and presenting visual acuity; manifest, subjective and cycloplegic refraction; color vision test; Goldmann applanation tonometry; examination of the external eye, anterior segment, media, and fundus; and an interview about demographic characteristics and history of eye diseases, eye trauma, diabetes mellitus, high blood pressure, and ophthalmologic cares. The study design and eye examination protocol are described. We expect that findings from the TES will show the status of visual problems and their causes in the community. This study can highlight the people who should be targeted by visual impairment prevention programs.
