We aimed to systematize the results of published studies on the use of Saccharomyces boulardii (SB) for the treatment of various liver disorders (CRD42022378050). Searches were conducted using PubMed and Scopus on 1 August 2022. The PubMed search was updated on 15 June 2024. The review included sixteen studies: ten experimental animal studies (EASs) and six randomized controlled trials (RCTs). The CNCM I-745 strain was used in 68.8% of the included studies. SB reduced the severity of many manifestations of cirrhosis, and lowered the Child–Pugh scores in RCT. SB reduced the serum concentrations of TNF-α, IL-1β, IL-6, and IL-4 in animals with metabolic dysfunction-associated steatotic liver disease (MASLD); lowered the serum TNF-α and IL-6 levels in experimental cirrhosis in rats; and reduced the CRP levels in decompensated cirrhosis. The EAS of MASLD revealed that SB reduced liver steatosis and inflammation and lowered the liver expression of genes of TNF-α, IL-1β, interferon-γ, and IL-10. In studies on experimental cirrhosis and MASLD, SB reduced the liver expression of genes of TGF-β, α-SMA, and collagen as well as liver fibrosis. SB reduced the abundance of Escherichia (Proteobacteria), increased the abundance of Bacteroidetes in the gut microbiota, prevented an increase in intestinal barrier permeability, and reduced bacterial translocation and endotoxemia.

Aim. To study the significance of monitoring high-sensitivity troponin I (hs-cTnI) for predicting anthracycline-induced left ventricular (LV) dysfunction in the treatment of breast cancer in patients with moderate and low risk of cardiotoxicity (CT).Material and methods. The study involved 49 patients with breast cancer aged 50±10 years who underwent neoadjuvant or adjuvant chemotherapy, which included doxorubicin at a course dose of 60 mg/m2 and an average cumulative dose of 251±60 mg/m2. The level of hs-cTnI was determined by an ultrasensitive method before the start of chemotherapy, after each course of anthracyclines and in 18 patients before the administration of anthracyclines. The level of hscTnI >0,017 ng/ml was considered elevated. Echocardiography was performed before the start of chemotherapy, after the end of anthracycline therapy, and every 3 months for 12 months thereafter. CT was defined as a decrease in LV ejection fraction (EF) by ≥10% to <53%.Results. CT risk before chemotherapy was considered low and moderate in 96% of patients. An increase in hs-сTnI was detected ≥1 times in 56,8% of patients: before chemotherapy — in 13,5%, after 1 and 2 courses of anthracycline therapy — in 13,9%, after 3, 4, 5 and 6 courses — in 44%, 62%, 71% and 66% of patients, respectively. The levels of hs-cTnI before and after administration of anthracyclines did not differ significantly. The development of LV dysfunction was observed in 16,3% of patients. There were following prognostic significance of an increase in hs-cTnI at any time of chemotherapy for a decrease in LV EF: sensitivity — 87,5%, specificity — 50%, the positive predictive value — 28%, the negative predictive value — 94,7%. The closest relationship was noted between CT and hs-cTnI value before the start of chemotherapy (β=0,45, p=0,005) and after the 3rd course of anthracycline therapy (β=0,56, p=0,002).Conclusion. An increase in hs-cTnI level before and during anthracycline thera py in patients with a low risk of cardiotoxicity has a prognostic value in relation to the development of left ventricular dysfunction. Hs-cTnI assessment should be performed before the start of therapy, and then starting from the 3rd course of anthracycline therapy in all patients, regardless of the risk of cardiotoxicity.

State-of-the-art therapy improves the five-year survival rate of patients under the age of 20 with cranial and craniospinal tumors by up to 74%. The urgency of dealing effectively with late treatment-associated cardiovascular complications is rising. Objective: We aimed to assess echocardiographic parameters and exercise performance in subjects with a history of complex treatment for cranial and craniospinal tumors in childhood. Methods: the study of 48 subjects who underwent cranial and craniospinal irradiation for CNS tumors in childhood and 20 healthy age- and sex-matched volunteers was conducted. The examination included hormone studies, cardiopulmonary exercise testing, and, in the main group, echocardiography (ECHO). Results: In five (10.4%) patients, ECHO changes were detected after complex anti-cancer treatment: thickening and calcification of the aortic valve leaflets (2%), and reduction in the systolic LV and RV function (8% and 6%, respectively). Irradiation of various areas was a significant predictor for reduced exercise tolerance, hyperventilation at rest and upon exertion, and an increased ventilatory equivalent for carbon dioxide. Low exercise tolerance was associated with a younger age at the time of treatment initiation. Significant differences were noted between the control group and the childhood cancer survivors with endocrine disorders. Conclusions: The obtained data confirm the importance of regular cardiovascular and endocrine monitoring of this group of cancer survivors.

Objective. To generalize and analyze modern data on the role of intercellular adhesion molecules, in particular cadherins, in the regulation of vascular function, inflammatory responses and formation of cardiovascular diseases, including atherosclerosis, to assess the diagnostic and prognostic potential of P-, E- and H-cadherins. Materials and methods. The review includes publications on cell adhesion molecules, predominantly cadherins, and their involvement in the pathogenesis of inflammatory, oncological and cardiovascular diseases. Data of original researches, meta-analyses and reviews from international and domestic sources were used. The analysis was carried out with emphasis on studies in which the expression of P-, E- and H-cadherins, their concentration in the serum and relationship with the progression of atherosclerosis and cardiovascular events were studied. Results. Cadherins provide adhesion between cells of the same type and are involved in signal transmission influencing morphogenesis, immune responses and vascular regulation. Change in expression of these proteins is connected with endothelial dysfunction, inflammation and remodeling of the vascular wall. Elevated concentration of E-cadherin is associated with hemodynamically significant atherosclerosis and risk of developing adverse cardiovascular events. P-cadherin content is determined at early stages of the disease, and its involvement in disrupting endothelial permeability is possible. It is reported that H-cadherin is considered as a regulator of cell adhesion and mobility as well as a modulator of cell behavior under modified tension. The obtained data indicate the diagnostic significance of cadherins in the evaluation of atherosclerosis progression risk. Conclusion. Cadherins as key components of intercellular adhesion are involved in the formation of vascular pathology and can serve as biomarkers for atherosclerotic lesions. Their further investigation is promising for the development of new approaches to diagnosis, risk stratification and prevention of cardiovascular diseases.

July 14, 2021 marks the 220th anniversary of an outstanding scientist and physician — Johann Peter Müller. It is thanks to his works that we know what the «endocrine gland» is, understand the taxonomy of the animal world and the essence of ontogenesis. Professor, researcher, wonderful teacher, director of the anatomical and physiological museum of the University of Berlin — he succeeded in any role in life. Mueller’s discoveries were recognized during his lifetime, textbooks were published under his leadership, and scientific articles were published. The value of the professor’s work is high to this day, which once again proves the colossal contribution of Johann Müller in the formation of world science.
