Purpose: The medical students’ attitude toward pain in people with advanced dementia, while constituting an important factor in care, has rarely been assessed to date. The aim of our study was thus to perform such assessment in medical students in Kazakhstan, to enable an improvement of the existing curriculum (like we previously did in Poland). Materials and Methods: We analyzed the knowledge about pain using a short anonymous questionnaire, which was completed by 112 students of the Medical University of Aktobe, Kazakhstan. Results: On average, students listed symptoms of 1.4 ± 1.2 (out of 6 analyzed) pain areas (median 2.0). The symptoms related to changes in mental status were suggested the most often (57 students: 50.9%). The students who indicated these symptoms also listed a higher number of symptoms from the remaining domains (1,1 ± 1.0 [median 1.0] vs 0.6 ± 0.8 [median 0.0]; p< 0.01). Observational methods in the assessment of the severity of pain in people with dementia were indicated by 44 students (39.3%), but only one participant (0.9%) was able to name an observational scale for pain assessment. Correct answers regarding pain treatment rules were presented by 18 students (16.0%), and the answers of the next 47 participants (42.0%) were very general but suggested the same treatment no matter what the cognitive status. Conclusion: The study revealed gaps in the knowledge of Kazakh medical students regarding pain in advanced stages of dementia. Demographic changes, combined with the coexistence of pain with dementia, indicate that medical students worldwide must have sufficient knowledge and skills to adequately care for the continually growing number of people with these conditions. It is imperative in countries like Kazakhstan, where the dementia burden was unrecognized until now, but it will blow up in the near future.

Background/Objectives: The growing population of cancer survivors has increased the need for reliable, culturally appropriate instruments to identify unmet supportive care needs. The Cancer Survivors’ Unmet Needs Measure (CaSUN) is a survivorship-specific assessment tool, but validated Russian and Kazakh versions have not previously been available. This study aimed to translate and culturally adapt CaSUN, evaluate its psychometric properties in cancer survivors in Kazakhstan, and examine unmet needs relevant to survivorship care. Methods: This cross-sectional psychometric validation study included 404 cancer survivors (Russian version, n = 201; Kazakh version, n = 203). Translation and cultural adaptation involved forward- and back-translation, expert review, cognitive debriefing, and pilot testing. Internal consistency was assessed using Cronbach’s alpha. Construct validity was evaluated using exploratory (EFA) and confirmatory factor analysis (CFA), with measurement invariance examined across language versions. Unmet needs and associated demographic and clinical factors were also examined. Results: Both versions demonstrated excellent internal consistency (Cronbach’s alpha: Russian = 0.966; Kazakh = 0.978). KMO values were 0.920 and 0.940, respectively (Bartlett’s tests, p < 0.001). EFA yielded a five-factor solution explaining 68.1% and 64.2% of the variance in the Russian and Kazakh versions, respectively; however, parallel analysis supported five factors in the Russian sample but suggested three in the Kazakh sample, indicating greater structural uncertainty for the Kazakh version. CFA demonstrated good incremental fit in both language groups (Russian: CFI = 0.975, TLI = 0.973; Kazakh: CFI = 0.973, TLI = 0.971), although RMSEA (0.096 and 0.102, respectively) and SRMR (0.116 and 0.099, respectively) indicated residual model misfit. Metric invariance was supported, while scalar invariance was supported by conventional fit index criteria but should be regarded as provisional. The most frequent unmet needs concerned medical care, up-to-date information, and involvement in health management. Younger age, male sex, urban residence, a history of cancer recurrence, and Kazakh language completion were independently associated with more unmet needs. Conclusions: The Russian and Kazakh CaSUN versions demonstrated high internal consistency and evidence supporting construct validity and cross-language comparability. These culturally adapted versions may support systematic needs assessment and patient-centered survivorship care in multilingual oncology services in Kazakhstan.

Background.Patient-centered care is increasingly recognized as a key quality domain in infertility treatment. However, little is known about barriers to implementing patient-centered fertility services in Central Asian settings. Aim. To explore barriers, unmet needs, and improvement priorities in infertility care from the perspectives of women receiving fertility treatment and healthcare professionals in Kazakhstan. Materials and methods. A qualitative study was conducted using semi-structured interviews with women undergoing fertility treatment and reproductive healthcare professionals in in vitro fertilization (IVF) clinics. Data were analyzed using qualitative content analysis. Themes were identified inductively and compared across participant groups. Results. Five major themes emerged: (1) patient information; (2) patient’s emotional state; (3)psychological support; (4) organization of the treatment process; and (5) client service. Participants highlighted fragmented information, emotional distress, unrealistic expectations, limited access to psychological support, insufficient coordination between specialists, logistical barriers, and the need for more personalized and convenient care. Women emphasized anxiety, uncertainty, and emotional stress, whereas professionals more often described time pressure, workflow constraints, and communication overload. Conclusion.Patient-centered infertility care in Kazakhstan requires not only clinical excellence but also organizational and communication reforms. Introducing patient navigators, psychological support services, clearer information systems, and multidisciplinary coordination may substantially improve patient experience and treatment quality. Introduction: Rational use of medicines (RUM) is the cornerstone of safe, effective, and affordable outpatient care. We assessed how practices in Shymkent polyclinics align with WHO indicators. Objective: To evaluate WHO indicators for prescribing and patient care in the outpatient setting and to identify variability requiring targeted improvements. Materials and methods: A cross-sectional study based on WHO methodology (December 2024–April 2025) was conducted in 13 polyclinics. A total of 390 outpatient visits of patients were analyzed for patient-care indicators and 1300 prescriptions for prescribing indicators. Descriptive statistics, Welch’s ANOVA, and permutation χ²-tests were applied. Composite performance indices were constructed for benchmarking. Results: The mean consultation duration was 13.5±6.2 minutes (range 3–35 across centers; p<0.001). Dispensing time averaged 205±139 seconds (45–960; p<0.001). On average, 94.4% of prescribed medicines were dispensed; 86.4% of patients correctly knew their dosages, both showing significant inter-center variation. An average of 1.8±0.9 medicines was prescribed per visit; 100% by INN and from the EML. Antibiotics were prescribed in 3.4% of visits (0–8% across centers; p=0.01). Injections were used in 24.5% of visits (low variability; p=0.992). Composite indices identified center 9 as best-performing in patient care and center 7 in prescribing; centers 2, 6, and 13 consistently lagged (e.g., in center 6 only 77% of medicines were dispensed, and dosage knowledge reached just 60%). Conclusions: Outpatient care in Shymkent demonstrates strengths - low polypharmacy, universal INN prescribing, alignment with essential medicines, and relatively low antibiotic use. However, key challenges remain: wide variability in consultation and dispensing times, insufficient patient counseling, stock-outs of medicines in some centers, and persistently high injection prescribing. Priority measures: Standardization of consultation and dispensing processes, reduction of unnecessary injections through clinical protocols and provider training, strengthening of pharmaceutical counseling and patient education, and ensuring equitable medicine availability - especially in underperforming centers.

Introduction. Infertility remains one of the pressing problems of modern medicine, affecting not only the physical health of patients but also their psycho-emotional state, social relationships, and overall quality of life. With the increasing prevalence of infertility and the widespread use of assisted reproductive technologies (ART), there is a growing need for a more humanistic and holistic approach to medical care. One of the key directions in improving treatment effectiveness is the implementation of patient-centered medicine principles. Patient-centered care implies active involvement of the patient in decision-making, respect for their values, provision of information, and ensuring comfortable treatment conditions. The application of such approaches is especially important in reproductive medicine, where every intervention touches upon deeply personal and socially sensitive aspects of patients’ lives. Aim. To conduct a review of current scientific literature devoted to the implementation of a patient-centered approach in infertility treatment. Search strategy. The review included scientific publications addressing patient-centered approaches in infertility treatment using ART and in the organization of reproductive care. The search query included the following keywords: “patient-centered care,” “infertility,” “patient experience,” “reproductive medicine,” “ART,” “shared decision-making,” as well as their equivalents in Russian. The search was conducted in PubMed, Scopus, Web of Science, and Google Scholar databases for the period from 2006 to 2024. Results. Patients’ experience of patient-centered care (PCC) in infertility treatment varies across different countries. To the best of our knowledge, no studies have been conducted in Kazakhstan to investigate the perceptions of healthcare professionals and patients regarding the implementation of patient-centered care in infertility. Conclusion. The key components of the patient-centered model are active patient involvement in decision-making, provision of accessible and reliable information, emotional support, and individualization of the treatment process.
