Recent evidence revealed that an adequate preoperative physiological reserve is crucial to overcome surgical stress response. Consequently, a new concept, called prehabilitation, emerged, aiming to improve the preoperative functional reserve of patients who will undergo major abdominal surgery. During the interval between diagnosis and surgery, a multimodal approach consisting of physical exercise and nutritional and psychological support could be employed to enhance physiologic reserve. Physical activity interventions aim to improve aerobic capacity, muscle strength and endurance. Nutritional support addressing malnutrition and sarcopenia also contributes to the achievement of the above-mentioned goals, particularly in patients undergoing cancer-related procedures. Psychological interventions targeting anxiety, depression and self-efficacy, as well as risk behavior modification (e.g., smoking cessation) seem to enhance recovery. However, there is a lack of standardization regarding these interventions, and the evidence about the impact of this multidisciplinary approach on the postoperative outcomes is still contradictory. This narrative review focuses on the physiological basis of surgical stress response and on the efficacy of prehabilitation, reflected mainly in the length of hospitalization and rates of postoperative complications. Multidisciplinary collaboration between surgeons, nutritionists, psychologists and physiotherapists was identified as the key to the success of prehabilitation programs. Synergizing prehabilitation and ERAS protocols significantly improves short-term surgical outcomes. Recent well-designed, randomized clinical trials revealed that this approach not only enhanced functional reserve, but also decreased the rates of postoperative complications and enhanced patient’s overall quality of life, emphasizing the importance of its implementation in routine, elective, surgical care.

Background/Objectives: This paper brings to light the results of extensive research on the dimension of dental care in Romania in terms of priorities and policies defined at the governmental level for a 25-year period (1999–2023). This research aims to present both individual and ecosystem perspectives on the influence of the way oral health strategies are shaped in Romania. Methods: The data collected are analyzed through the prism of the “iron triangle of healthcare”, focusing on the evolution of dental care from the perspective of the interrelationship of three important components: cost, quality, and access to healthcare services. Results: The paper provides insight and clarity into the context in which the doctor–patient relationship is constructed and developed, as well as a vision of how oral health policies should be designed to meet the individual needs of the patient, which will have an impact on the health and quality of life of the whole community. Conclusions: This study emphasizes the need to reconfigure oral health policies in Romania. Despite some input achievements like a considerable number of dentists, the systemic challenges that developed over these 25 years (such as insufficient funding, the reduced reimbursed procedures, and the limited dentists contracting with NHIH) have significantly contributed to a stagnation or decline of the oral health outcomes at the societal level.

Acute necrotizing pancreatitis (ANP), characterized by necrosis of pancreatic and/or peripancreatic tissues, is a potentially severe and life-threatening complication of acute pancreatitis, exhibiting a considerable mortality rate, particularly in the presence of infection, with rates ascending to 20–30%. Contrast-enhanced computed tomography is the definitive diagnostic standard, although treatment is determined by illness severity and the presence of secondary infection. The management of this condition has undergone considerable evolution, transitioning from initial surgical intervention to a more progressive, minimally invasive strategy. Initial management emphasizes aggressive fluid resuscitation, nutritional support, and monitoring of organ dysfunction. Infected necrosis is a critical factor influencing prognosis and requires intervention, typically starting with percutaneous drainage or endoscopic necrosectomy prior to evaluating surgical debridement. Recent advancements, such as the implementation of endoscopic and minimally invasive techniques, have enhanced outcomes by decreasing morbidity and mortality linked to open surgery. Despite these advancements, optimal treatment strategies are patient-specific and necessitate a multidisciplinary approach. Additional research is necessary to enhance guidelines and optimize patient outcomes.

Background/Objectives: Standard dental practice is being reshaped by digital technologies, and artificial intelligence (AI) is emerging as one of the most challenging recent innovations. Methods: The present study assessed the interest of Romanian dentists in the integration of AI into their current practice through an anonymous questionnaire distributed to 200 respondents. The questionnaire addressed the integration of AI in dentistry by analyzing the following areas of intervention: stages of patient care, perceived impact on the doctor–patient relationship, data security, implementation costs, and the legislative framework. Results: The results showed that 53.6% of dentists reported low difficulties, 37.3% reported moderate difficulties, and 9.1% reported high difficulties with using digital tools. Dentists’ reported willingness to adopt AI-based solutions was as follows: 58.6% were very willing, 30% were moderately willing, and only 11.4% were not very willing. Currently, 80.5% already use digital techniques in their daily practice. The participants emphasized the need to maintain a strong doctor–patient relationship while recognizing the benefits of increased efficiency. They were aware of the risk of diminishing human connection and trust. Also, data security and the financial stress associated with implementing and maintaining new systems were seen as major obstacles. Conclusions: The dentists surveyed showed an increased interest in modern digital technologies, provided that there is a clear legal framework, a strong data protection system, and the preservation of the doctor–patient relationship based on trust and confidentiality that defines the medical profession.

This study investigates the role of inflammatory markers in perception disorders associated with major psychiatric pathology, focusing on schizophrenia and acute psychotic disorder. Guided by the vulnerability–stress–inflammation model, this research explores how genetic predispositions, maternal immune activation, and chronic low-grade neuroinflammation contribute to disease onset and progression. A sample of 135 patients (69 with schizophrenia and 66 with acute psychotic disorder) admitted to a psychiatric hospital between October 2024 and February 2025 was analyzed. Demographic and clinical data, along with biological markers—such as white blood cells, neutrophil-to-lymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), platelet-to-lymphocyte ratio (PLR), systemic immune–inflammation index (SII), C-reactive protein (CRP), and erythrocyte sedimentation rate (ESR)—were assessed. Results indicated elevated median values for SII, CRP, and MLR, with statistically significant differences compared to normal reference ranges, suggesting persistent systemic inflammation in psychotic disorders. While acute psychotic disorders showed wider value ranges, schizophrenia patients demonstrated higher median levels, consistent with chronic inflammation. No significant differences were observed between the two groups after Bonferroni correction, though CRP values suggested a trend toward greater inflammation in schizophrenia. These findings reinforce the inflammatory hypothesis of psychosis and highlight the potential of biomarkers to refine diagnosis, guide treatment strategies, and support future research into immunomodulatory therapies.

This study presents a comprehensive bibliometric analysis of the evolving landscape of data protection in medicine, examining research trends, thematic developments, and scholarly contributions from the 1960s to 2024. By analyzing 2159 publications indexed in the Scopus database using the Bibliometrix R package (v.4.3.2), based on R (v.4.4.3), this paper maps key research areas, leading journals, and international collaboration patterns. Our findings reveal a significant shift in focus over time, from early concerns centered on data privacy and management to contemporary themes involving advanced technologies such as artificial intelligence, blockchain, and big data analytics. This transition reflects the increasing complexity of balancing data accessibility with security, ethical, and regulatory requirements in healthcare. This analysis also highlights persistent challenges, including fragmented research efforts, disparities in global contributions, and the ongoing need for interdisciplinary collaboration. These insights offer a valuable foundation for future investigations into medical data governance and emphasize the importance of ethical and responsible innovation in an increasingly digital healthcare environment.
