BACKGROUND: Single embryo transfer is globally recommended during IVF treatments. Hence, there is a growing demand for better embryo selection. Additionally, to morphology and genetics, nutrient uptake/release and metabolome profiles in spent embryo culture media (SECM) are proposed as non-invasive biomarkers. Are they ready to be applied for clinical purposes? OBJECTIVE AND RATIONALE: We reviewed methods of metabolism analysis for embryos, focusing on human SECM. SEARCH METHODS: Until November 2024, the Cochrane Library, PubMed, and Google Scholar were surfed for peer-reviewed English-language studies in the human, with MeSH terms and keywords: ART, IVF, ICSI, aneuploidy, embryo transfer, embryo selection, culture media, metabolome, metabolomics, metabolic profile, artificial intelligence (AI), nutrients, carbohydrates, glucose, pyruvate, lactate, amino acids (AAs), fatty acids (FAs), and spent embryo culture medium (SECM). Also, the reference lists of all relevant articles were searched. OUTCOMES: Forty-nine original publications (1989–2024) were found in which SECM samples were collected from 20 countries, focusing on preimplantation embryo metabolism single biomarker(s) of energy sources (glucose and pyruvate), AAs and free FAs (17 studies), or metabolomic analysis (32 studies). Focal points were blastocyst development, aneuploidy, embryo sex prediction, implantation, and pregnancy outcome. Eleven major companies, which supply embryo culture media, dominate the market. Nutrient composition of their culture media presents major challenges because they are not normally disclosed. In single-biomarker(s) studies, eight studies focus on glucose and pyruvate, eight on AAs, both alone and in combination with glucose or pyruvate, and their ratios. Since the absolute quantities of some AAs or glucose levels were reported in some studies, they all have the potential to become future biomarkers for clinical application. In metabolomics studies, almost all studies reported qualitative results, such as decrease/increase or the metabolite ratios. For absolute concentrations, the basal concentrations of the culture media must be considered. In sum, all differences in the experimental design, the platforms, and the results were analyzed. WIDER IMPLICATIONS: Establishing a unified guideline for the reporting of metabolomics studies and a specific guideline outlining the minimum information required for SECM experiment publication will ensure that future studies provide all necessary and critical information. The metabolomics studies primarily focused on implantation and pregnancy, whereas we, as a first step, preferred multi-omics studies on absolute concentrations of metabolites of good vs poor quality and euploid vs aneuploid embryos. Following this step, these quantitative approaches might lead to more convincing successes. If small numbers of predictive biomarkers were identified, a simple, rapid, and cheap test could be developed for each medium, clinically performed in the fertility center. Furthermore, further research on basal media ingredients is needed, combined with targeted metabolomics. The future could be an integration of all—primarily non-invasive—information, omics, and other, by AI. Background: The epidemiology and prevalence of the Human T-cell leukemia virus type-1 (HTLV-1) infection represent a recommended priority by global health agencies. An in-depth revision to update the status of this infection in countries including those of the Eastern Mediterranean Regional Office (EMRO) of the World Health Organization is hence required. Methods: Ninety-seven studies evaluating the HTLV-1 infection in low- and high-risk populations in EMRO countries were retrieved from the international electronic databases and were used to assess the epidemiological status of the infection in these countries. Results: Most epidemiologic reports were published from Iran, with more than 50% of Iranian prisoners and around 4% of healthy individuals reported to have the infection. In Egypt, a considerable prevalence of the virus spans around 1.11% of blood donors. Foci of HTLV-1 infection are also present in some countries and require a careful epidemiological evaluation. In the other EMRO countries, a lower prevalence that does not exceed 1% was reported. Conclusion: The epidemiology and prevalence of HTLV-1 in the EMRO countries require a tight revision and update. Published studies reveal a scarce distribution of the virus in the African countries of EMRO, while a lower prevalence is denoted in the Asian countries of EMRO, except in Iran, where the prevalence is high.

Coronavirus disease 2019 (COVID-19) is now of global concern due to its rapid dissemination across the globe. The rapid spread of this viral infection, along with many of its unknown aspects, has posed new challenges to the health care systems. The main challenging effects of COVID-19 are rapid dissemination through close contact and varying clinical severity among different individuals. Furthermore, the medical staff in endemic areas are becoming exhausted and deal with a considerable level of job burnout, which can negatively affect their medical decision making. Also, due to the variable pulmonary manifestations of COVID-19, some physicians may misdiagnose patients. To overcome these issues, we proposed a web-based software to aid physicians in detecting possible COVID-19 cases through online consultation with different specialists and educate the not-well experienced physicians. Our results demonstrated that this software could improve the diagnostic rate for not-well experienced physicians.

Background: Phenylketonuria (PKU) is a common metabolic disorder with great burden if left untreated or undiagnosed. Genetic variations in the phenylalanine hydroxylase (PAH) gene may be widely varied across different regions of a country. By knowing the most common mutations, diagnostic work-ups will be offered sooner and with lower costs for patients. The present study defines the most common genetic variations in the PAH gene in Khorasan province of Iran. Methods: The present cross-sectional study took place in Khorasan province of Iran within a 6-year period starting from 2012 to 2018. Every patient who had been referred as suspicious PKU cases or referred for prenatal diagnosis was included in the present study. Results: A total number of 122 individuals with a mean age of 26.22 years were enrolled in the present study. The most frequent genetic variations in the PAH gene were c.1066-11G > A and c.143 T > C. Exon 7 carried the most genetic variations compared to any single exon. Also, three patients had compound heterozygous status for c.727 C > T/c.1066-11 G > A in exon 7 and 11 of the PAH gene. Conclusions: Mutations in the PAH gene are widely varied among different populations, and our results confirmed this fact. Determination of the most prevalent mutations and polymorphisms in each region will reduce the time and cost of diagnosing such preventable diseases and will therefore reduce the disease burden.

Background. Congenital hyperinsulinism (CHI) is a rare and life-threatening genetic disorder. Sirolimus as a mammalian target of rapamycin inhibitor may be helpful in patients with CHI who do not respond well to other treatments including diazoxide and octreotide. However, the safety and efficacy of this therapy are still unclear. This study aimed to evaluate the potential therapeutic effects of sirolimus in CHI patients with mutations in the ABCC8 and KCNJ11 genes. Methods. During the period of this follow-up study, every child with a confirmed diagnosis of unresponsive CHI underwent genetic evaluation. Among those who had positive genetic testing, six families agreed to participate in this study. The participants were evaluated for ABCC8, KCNJ11, or HNF4α gene mutations by polymerase chain reaction (PCR) sequencing. The participants who were unresponsive to diazoxide and octreotide therapy received 0.5 mg/m2/d of sirolimus, and the dose was gradually increased until a serum concentration of 5–15 ng/ml was achieved. Then, the participants were followed up for any possible complications. Results. Among the study participants, only one neonate was completely free of hypoglycemia after one year of follow-up, whereas three others experienced a partial reduction in hypoglycemic episodes over six months. One neonate underwent pancreatectomy despite receiving sirolimus. The oldest participant with a mutation in the ABCC8 gene responded well to sirolimus therapy after surgery and remained asymptomatic for 18 months. Conclusion. This study suggested that sirolimus therapy needs further evaluation to determine which patients will benefit the most. The genetic basis of CHI may have possible implications for determining the patient’s response.
