Introduction: In 1989, the St Vincent declaration aimed to approximate pregnancy outcomes of diabetes to that of healthy pregnancies. We aimed to compare frequency and trends of outcomes of pregnancies affected by type 1 diabetes and controls in 1996–2018.Methods: We used anonymized records of a mandatory nation-wide registry of all deliveries between gestational weeks 24 and 42 in Hungary. We included all singleton births (4,091 type 1 diabetes, 1,879,183 controls) between 1996 and 2018. We compared frequency and trends of pregnancy outcomes between type 1 diabetes and control pregnancies using hierarchical Poisson regression. Results: The frequency of stillbirth, perinatal mortality, large for gestational age, caesarean section, admission to neonatal intensive care unit (NICU), and low Appearance, Pulse, Grimace, Activity, and Respiration (APGAR) score was 2-4 times higher in type 1 diabetes compared to controls, while the risk of congenital malformations was increased by 51% and SGA was decreased by 42% (all p<0.05). These observations remained significant after adjustment for confounders except for low APGAR scores. We found decreasing rate ratios comparing cases and controls over time for caesarean sections, low APGAR scores (p<0.05), and for NICU admissions (p=0.052) in adjusted models. The difference between cases and controls became non-significant after 2009. No linear trends were observed for the other outcomes. Conclusions: Although we found that the rates of SGA, NICU care, and low APGAR score improved in pregnancies complicated by type 1 diabetes, the target of the St Vincent Declaration was only achieved for the occurrence of low APGAR scores.

Objectives: The aim of this study was to analyze the modeling methodologies of fiscal policies on food with health or environmental outcomes. Background: Evidence suggests that fiscal policies on food can contribute to addressing the growing burden of noncommunicable diseases and climate change. These policies should be modeled in advance to see the implications for the environment and health. Methods: A systematic review was conducted of studies that modeled fiscal policies on the food groups targeted by the EAT-Lancet Commission and examined their health or environmental outcomes. The Scopus and PubMed databases were searched on November 30, 2021. The records were double-screened and data on modeling methods were extracted from the included studies. Results: A total of 55 studies were included in the review. The most frequently modeled interventions were fruit and vegetable subsidies (n = 19) and carbon taxes on food (n = 17). One study also included a consumer education campaign to enhance the effect of fiscal policy. The outcomes are highly sensitive to consumption change and price elasticities. None of the studies modeled the health effects of environmental outcomes. Conclusions: A model that covered all the relevant aspects of the issue was not found. Some parts were missing from all the included models. It is advisable to model the stability of the amount of diet consumed, either by keeping the amount of food in the diet stable or by taking a more conservative approach and keeping the consumed calories stable. It is preferable to keep the included diseases and environmental boundaries broad to have more valid outcome estimates on this complex issue. A more comprehensive understanding of fiscal policies would allow us to better anticipate the impact of our actions and inactions and thus could lead to more sophisticated measures taken by policymakers. Mindhárom étrend esetében meghatároztuk a fő élelmiszercsoportokra vonatkozó napi táplálékbeviteli mennyiségeket. Az étrendek egészségességét a Globális Betegségteher Vizsgálat kilenc élelmiszercsoportra vonatkozó ajánlásai alapján értékeltük; ezek a mennyiségek elméleti minimális kockázatot jelentenek a kapcsolódó betegségek tekintetében. A környezeti terheket – üvegházhatásúgáz-kibocsátás és földhasználat – a SHARP Indikátor Adatbázis alapján számoltuk. Eredmények : A vizsgált étrendek közül a Bolygóbarát Egészséges Étrend tekinthető a legegészségesebbnek, mivel 4 élelmiszercsoportban ajánl optimális táplálékbevitelt. A Globális Betegségteher Vizsgálat optimális beviteli értékei megbízhatónak tekinthetők, mivel széles körű szakirodalmi áttekintéseken és metaanalíziseken alapulnak, továbbá nagymintás kohorszvizsgálatokon is validálták őket. A vizsgált étrendek környezeti lábnyoma nagyságrendileg megegyezik a nemzetközi szakirodalomban korábban közölt eredményekkel. Következtetés: A Bolygóbarát Egészséges Étrend hazai adaptációja, illetve ajánlása megfontolandó, mivel a jelenlegi adatok alapján egészségi és környezeti szempontból egyaránt előnyösebb a jelenleg Magyarországon elterjedt Okostányér-ajánlásnál és az átlagos magyar étrendnél. Background: Existing studies on the economic impact of schizophrenia are fragmented and vary in methodology, hindering an ample understanding of the total economic burden. Aims: To quantify the global economic burden of schizophrenia by synthesizing findings from systematic reviews and meta-analyses, and to assess variations by cost category, world region and income level. Method: We conducted a protocol-driven umbrella review registered on PROSPERO (CRD42024504092). A systematic search of MEDLINE, EMBASE, Cochrane Library, and APA PsycINFO identified reviews reporting annual per-patient monetary cost estimates for schizophrenia. Monetary values were adjusted for inflation and converted to 2024 US dollars. Results: Twenty-six systematic reviews involving 152 primary studies were included. The median annual per-patient total cost of schizophrenia was $33,236 (mean $47,872), with direct costs accounting for $23,126 (medical: $19,543; ancillary: $1,152) and indirect costs for $21,333. Costs were varied by income level: in high-income countries, the median total cost was $34,175, compared to $3,345 in upper-middle-income and $3,452 in lower-middle-income countries. Regional disparities were substantial, with Europe & Central Asia reporting the highest median costs among high-income settings. Data from low- and middle-income regions were limited or absent. Methodological heterogeneity, lack of standardization in cost reporting, and underrepresentation of low-resource settings limit generalizability. Conclusions: Schizophrenia poses a significant and unequally distributed global economic burden, driven by hospitalization, long-term care, and productivity losses. To support equitable policy and resource allocation, future research must adopt standardized costing frameworks and expand data collection into low-resource and understudied regions.
