The prevalence of type 2 diabetes mellitus (T2DM) has increased in the United States, contributing significantly to morbidity and mortality. This study analyzes trends in T2DM-related mortality focusing on demographic and regional disparities. The Centers for Disease Control and Prevention Wide-Ranging Online Data for Epidemiologic Research database was utilized to extract death certificate data for adults aged 25 and older from 1999 to 2023. Age-adjusted mortality rates (AAMRs) per 100,000 persons were calculated. Temporal trends were assessed by calculating the annual percent change using Joinpoint regression analysis. From 1999 to 2023, a total of 2,031,626 deaths were attributed to T2DM in the United States. The AAMR more than doubled from 21.54 per 100,000 in 1999 to 53.95 per 100,000 in 2023 with a pronounced increase between 2018 and 2021 (AAMR: 62.7 in 2021, annual percent change: 16.06%; 95% CI: 11.84-19.66). Males had considerably higher AAMR than females (68.82 vs 42.48 in 2023). Among racial and ethnic groups, Hispanic or Latino populations had the highest AAMR in 2023 (69.69), followed by non-Hispanic Black or African American (65.45), non-Hispanic other populations (53.7), and non-Hispanic White group (49.98). The Western region of the United States showed the highest AAMR (78.29), and rural areas consistently had higher mortality rates compared to urban areas (69.88 vs 55.32 in 2020). From 1999 to 2023, cardiovascular disease accounted for 626,706 deaths among adults with T2DM. T2DM-related mortality has increased substantially over the time in the United States, with a peak observed between 2018 and 2021, emphasizing the need for targeted interventions.

Background: Dexmedetomidine, a highly selective α2-adrenergic receptor agonist, may help mitigate postoperative complications in patients undergoing aortic vascular surgery. Methodology: A comprehensive search was conducted across PubMed, the Cochrane Library, and Embase to identify studies assessing the efficacy and safety of dexmedetomidine compared with placebo in patients undergoing aortic vascular surgery. A random effects meta-analysis was performed with R version 4.4.1 using the “meta” package. Results: Our analysis included eleven studies, comprising eight RCTs and three cohort studies, with a combined total of 1731 patients. The results showed that dexmedetomidine significantly reduced acute kidney injury (OR 0.49, 95% CI 0.25 to 0.98), ICU length of stay (MD −0.25 days, 95% CI −0.47 to −0.02), postoperative pulmonary complications (OR 0.55, 95% CI 0.32 to 0.94), and CRP levels 24 h post-surgery (MD −24.73 mg/L, 95% CI −46.29 to −3.16) compared to the control group. The length of hospital stay (MD -0.87 days, 95% CI −2.57 to 1.01), postoperative delirium (OR 0.78, 95% CI 0.43 to 1.42), and in-hospital mortality (OR 0.57, 95% CI 0.29 to 1.12) were not significantly different between the two groups. Conclusion: In patients undergoing aortic vascular surgery, dexmedetomidine administration is associated with reduced acute kidney injury, length of ICU stay, postoperative pulmonary complications, and CRP levels 24 h post-surgery.

The effectiveness of glucagon‐like peptide‐1 receptor agonists in facilitating weight loss among patients with diabetes is widely recognized. However, there are limited data available on the relative effectiveness and safety of once‐weekly semaglutide versus once‐daily liraglutide. We conducted a systematic review and meta‐analysis of randomized controlled trials (RCTs) identified through a comprehensive search of the Cochrane Library, PubMed, and ScienceDirect databases from inception until July 2024. Statistical analysis was conducted using R version 4.4.1 with the “meta” package, employing a random effects model. Three RCTs with a total of 922 patients were included in our meta‐analysis. The results indicated that OW semaglutide significantly reduced body weight (WMD: −4.55; 95% CI: −6.43, −2.67, p < 0.01), HbA1c (WMD: −0.46; 95% CI: −0.84, −0.08; p = 0.02), and fasting plasma glucose levels (WMD: −1.23; 95% CI: −1.51, −0.95; p < 0.01) in comparison to OD liraglutide. The risk of severe adverse effects (OR, 1.66; 95% CI, 0.53–5.16; p = 0.38) and gastrointestinal adverse effects (OR, 1.84; 95% CI, 0.82–4.14; p = 0.14) was comparable between both groups. Once‐weekly semaglutide therapy results in a more pronounced loss in body weight, HbA1c, and fasting glucose levels compared to once‐daily liraglutide.

Background: Non‐ST segment elevation acute coronary syndromes (NSTE‐ACS) are a common cause of hospital admission in older patients. Our study aims to synthesize the available evidence from randomized controlled trials (RCTs) to compare clinical outcomes with invasive versus conservative medical management in this population. Methods: A literature search of online databases including PubMed/MEDLINE, Embase, and the Cochrane Library was conducted from inception to September 1, 2024. The search aimed to identify RCTs that reported clinical outcomes with invasive versus conservative strategies in older patients (≥ 70 years) with NSTE‐ACS. The risk ratios (RRs) were used as summary estimates. Results: Seven RCTs with 2998 patients were included; 1490 patients in the invasive group and 1508 patients in the conservatively managed group. The pooled analysis demonstrated no statistically significant difference between the two strategies for the risk of all‐cause death (RR: 1.03, 95% CI: 0.92–1.15), cardiovascular death (RR: 1.04, 95% CI: 0.82–1.33), stroke (RR: 0.78, 95% CI: 0.53–1.15), and major bleeding (RR: 1.23, 95% CI: 0.90–1.69). However, the invasive strategy was associated with a significantly reduced risk of myocardial infarction (RR: 0.74, 95% CI: 0.57–0.96) and unplanned revascularization (RR: 0.29, 95% CI: 0.21–0.40) compared to the conservative strategy. Conclusion: In older patients with NSTE‐ACS, an invasive strategy reduces the risk of repeat myocardial infarction and unplanned revascularization without a significant increase in stroke or major bleeding. There was no associated reduction in all‐cause or cardiovascular mortality with the invasive strategy compared to conservative management.

Background: The potential role of adjunctive intra‐arterial thrombolysis (IAT) in improving outcomes following mechanical thrombectomy (MT) remains unclear. This meta‐analysis evaluates the efficacy and safety of IAT after MT compared to MT alone in patients with acute ischemic stroke. Methods: We searched PubMed, Embase, and the Cochrane Library from inception to January 2025 with no language restrictions. Additionally, grey literature sources were explored. Randomized controlled trials comparing IAT after MT versus MT alone in acute ischemic stroke were included. Odds ratios (ORs) with 95% CIs were pooled using a random‐effects model in R. Trial sequential analysis was performed for the primary favorable outcomes assuming a 30% relative risk increase, an alpha level of 5%, and 80% power. The primary outcome was excellent functional outcome (modified Rankin Scale score 0–1 at 90 days); secondary outcomes included good functional outcome (modified Rankin Scale score 0–2), symptomatic and any intracranial hemorrhage, and severe adverse events. Results: Four randomized controlled trials encompassing 1395 patients (MT and IAT: 701; MT alone: 694) met the inclusion criteria. The pooled analysis demonstrated significantly improved excellent functional outcome (OR, 1.31 [95% CI, 1.06–1.63]) in patients receiving IAT after MT compared with MT alone. No statistically significant difference was observed for good functional outcome (OR, 1.07 [95% CI, 0.86–1.32]), all‐cause death (OR, 0.92 [95% CI 0.70–1.21]), symptomatic intracranial hemorrhage (OR, 1.31 [95% CI, 0.74–2.34]), any intracranial hemorrhage (OR, 1.30 [95% CI, 0.96–1.76]), and severe adverse events (OR, 1.05 [95% CI, 0.67–1.66]). Trial sequential analysis revealed sufficient evidence to confirm a 30% relative risk increase for excellent functional outcome. The overall quality of evidence was moderate except for good functional outcome, rated as low. Conclusion: IAT following MT was associated with increased odds of excellent functional outcomes at 90 days compared with MT alone, with no significant differences observed in intracranial hemorrhage, all‐cause mortality, or severe adverse events.
