Introduction: Perinatal mortality is labeled as the loss of fetuses at or beyond 22 weeks of gestation, deaths during labor and delivery, as well as early neonatal deaths. Appropriate medical care provided in the perinatal period is an integral indicator of high-quality medical care. Although developed countries managed to decrease perinatal mortality, it remains high in the developing world. This study aims to perform a confidential audit of perinatal mortality (CAPM) across Kazakhstani maternity hospitals. Methods: A descriptive, observational cross-sectional study was conducted from January 2024 to December 2024. The structure of the underlying causes of mortality in the antenatal, intranatal, and early neonatal periods among different maternity hospitals of the Republic of Kazakhstan was analyzed. Results: A total of 116 cases were assessed: 34 antenatal deaths, 6 intranatal, and 76 early neonatal. Most deaths occurred on the second day post-delivery. The analysis revealed that 93% of cases fell into categories indicating substandard or potentially inadequate care (categories 2 and 3). Intraventricular hemorrhage and sepsis emerged as leading causes of neonatal death. Among antenatal and intranatal deaths, significant proportions were associated with comorbid maternal conditions, insufficient antenatal visits, and inadequate perinatal support. Conclusions: CAPM proves to be a critical tool for identifying systemic gaps and guiding improvements in maternity services without attributing blame to health professionals. Findings underscore that many perinatal deaths could have been avoided with timely, evidence-based interventions across antenatal and neonatal care. Broader implementation and institutionalization of CAPM in Kazakhstan could lead to measurable reductions in perinatal mortality and improvements in maternal/newborn care outcomes. Factors such as preconception planning, improving the health of reproductive-age women, administration of folic acid, and reducing primary cesarean sections could assist in achieving the reduction in the perinatal mortality rate.

Introduction. Perinatal mortality audits and reviews are conducted to identify preventable factors in mortality, identify weaknesses in health services, and make recommendations to improve the quality of neonatal care. This study aimed to assess the effectiveness of confidential audit of perinatal mortality through a content analysis of the use of different confidential audit tools in various countries. Research Strategy. Data were searched in medical databases such as Scopus, Cohrane Library, PubMed, and Embase. 50 relevant articles on the topic were selected, in addition, data from WHO and UNICEF manuals and reports. Results. The main methods of confidential audit are verbal and social autopsy, analysis and expert evaluation of medical history and clinical cases, classification of suboptimal help, and a combination of these methods. Identification of modifiable factors is widely and effectively used in many countries. The involvement of bereaved parents in the audit process has benefits and value for parents and healthcare providers, and it has gradually been introduced into the audit process in some countries. In the Netherlands, a decrease in perinatal mortality from 2.3 to 2.0/1000 births (p<0.00001) was observed during the audit between 2010 and 2012 [15]. A meta-analysis of 7 studies also found that perinatal mortality decreased by an average of 30% (95% confidence interval, 21 to 38%) in low- and middle-income countries following perinatal audits [42]. In Uganda, the perinatal mortality rate was 47.9 deaths per 1000 births in 2008 after the introduction of the audit, compared to 52.8 per 1000 births in 2007 [37]. Conclusions. Based on the review, it can be concluded that the main tools of confidential audit, such as identification of suboptimal care, modifiable factors, and expert assessment, are used quite effectively in different countries. According to the meta-analysis and systematic review, perinatal mortality is reduced by audits in both developed and developing countries. Perinatal mortality reviews should be continued to better understand the effectiveness of audit.

Background.Verbal autopsy is a method used to determine the cause of death through interviews with the deceased person's relatives or caregivers. Verbal autopsy was conducted as part of confidential audit of neonatal mortality in the Republic of Kazakhstan. In this study, we aimed to identify gaps that affect the quality of neonatal care, and make an impact on neonatal mortality. Material and methods. A prospective survey was conducted among healthcare professionals involved in the care of deceased neonates. Such problems as equipping perinatal centres with instruments, drugs, problems in instrumental and laboratory diagnostics, an infection control and antibacterial use were included in survey. Results. Totally 607 responses werecollected. Respondents were most often «rather satisfied» with the quality of laboratory tests 89(43.8%), narrow consultations 80(39.4%), the supply of medicines 79(38.9%), and the quality of medical care provided to deceased neonates 91(44.8%). The majority were «not satisfied» with medical equipment 84(41.4%), staffing 76(37.4%), availability of seminars and trainings 69(34%). Respondents were «satisfied» with the medical equipment 90(44.3%), and instrumental examinations 77(37.9%). As staff satisfaction with opportunities to improve their skills increases, so rises staff satisfaction with the quality of neonatal care in their unit (p=0.01, r=0.278). Echocardiography is not performed 24 hours a day in 125(62.5%) cases, although 155(77.5%) have an ultrasound machine available even at night. Regarding the tactics of antibiotic therapy, in case of positive culture, the respondents receive the results of bacteriological blood culture within 3-4 days 83(40.7%) and later than 3-4 days 82(40.2%). If the culture is negative, 89(43.6%) are obtained on the 4th-7th day. With the new bacteriological analyser, bacterial culture results are obtained earlier (p=0.05, r=0.229). Conclusion. Verbal autopsy gives opportunity to reveal factors that may have contributed to neonatal mortality and to develope action plan and prevention measures.
