Chronic leg ulcers are affecting approximately 6.5 million Americans, and they are associated with significant mortality, reduced quality of life, and high treatment costs. Since many chronic ulcers have underlying vascular insufficiency, accurate assessment of tissue perfusion is critical to treatment planning and monitoring. This study introduces a dual-scan photoacoustic tomography system that can simultaneously image the dorsal and plantar sides of the foot to reduce imaging time. To account for the unique shape of the foot, the system employs height-adjustable and articulating base ball stages that can scan along the foot’s contour. In vivo results from healthy volunteers demonstrate the system’s ability to acquire clear images of foot vasculature, and results from patients indicate that the system can image patients with various ulcer conditions. We also investigated various photoacoustic features and examined their correlation with the foot condition. Our preliminary results indicate that vessel sharpness, occupancy, intensity, and density could all be used to assess tissue perfusion. This research demonstrated the potential of photoacoustic tomography for routine clinical tissue perfusion assessment.

Linear-array-based photoacoustic computed tomography (PACT) has been widely used in vascular imaging due to its low cost and high compatibility with current ultrasound systems. However, linear-array transducers have inherent limitations for three-dimensional imaging due to the poor elevation resolution. In this study, we introduced a deep learning-assisted data process algorithm to enhance the image quality in linear-array-based PACT. Compared to our earlier study where training was performed on 2D reconstructed data, here, we utilized 2D and 3D reconstructed data to train the two networks separately. We then fused the image data from both 2D and 3D training to get features from both algorithms. The numerical and in vivo validations indicate that our approach can improve elevation resolution, recover the true size of the object, and enhance deep vessels. Our deep learning-assisted approach can be applied to translational imaging applications that require detailed visualization of vascular features.

Millimeter-wave (mmWave) sensing has emerged as a promising technology for non-contact health monitoring, offering high spatial resolution, material sensitivity, and integration potential with wireless platforms. While prior work has focused on specific applications or signal processing methods, a unified understanding of how mmWave signals map to clinically relevant biomarkers remains lacking. This survey presents a full-stack review of mmWave-based medical sensing systems, encompassing signal acquisition, physical feature extraction, modeling strategies, and potential medical and healthcare uses. We introduce a taxonomy that decouples low-level mmWave signal features—such as motion, material property, and structure—from high-level biomedical biomarkers, including respiration pattern, heart rate, tissue hydration, and gait. We then classify and contrast the modeling approaches—ranging from physics-driven analytical models to machine learning techniques—that enable this mapping. Furthermore, we analyze representative studies across vital signs monitoring, cardiovascular assessment, wound evaluation, and neuro-motor disorders. By bridging wireless sensing and medical interpretation, this work offers a structured reference for designing next-generation mmWave health monitoring systems. We conclude by discussing open challenges, including model interpretability, clinical validation, and multimodal integration.

We developed an automated photoacoustic and ultrasound breast tomography system that images the patient in the standing pose. The system, named OneTouch-PAT, utilized linear transducer arrays with optical-acoustic combiners for effective dual-modal imaging. During scanning, subjects only need to gently attach their breasts to the imaging window, and co-registered three-dimensional ultrasonic and photoacoustic images of the breast can be obtained within one minute. Our system has a large field of view of 17 cm by 15 cm and achieves an imaging depth of 3 cm with sub-millimeter resolution. A three-dimensional deep-learning network was also developed to further improve the image quality by improving the 3D resolution, enhancing vasculature, eliminating skin signals, and reducing noise. The performance of the system was tested on four healthy subjects and 61 patients with breast cancer. Our results indicate that the ultrasound structural information can be combined with the photoacoustic vascular information for better tissue characterization. Representative cases from different molecular subtypes have indicated different photoacoustic and ultrasound features that could potentially be used for imaging-based cancer classification. Statistical analysis among all patients indicates that the regional photoacoustic intensity and vessel branching points are indicators of breast malignancy. These promising results suggest that our system could significantly enhance breast cancer diagnosis and classification.

Photoacoustic tomography (PAT) is an emerging imaging modality with widespread applications in both preclinical and clinical studies. Despite its promising capabilities to provide high-resolution images, the visualization of vessels might be hampered by skin signals and attenuation in tissues. In this study, we have introduced a framework to retrieve deep vessels. It combines a deep learning network to segment skin layers and an adaptive weighting algorithm to compensate for attenuation. Evaluation of enhancement using vessel occupancy metrics and signal-to-noise ratio (SNR) demonstrates that the proposed method significantly recovers deep vessels across various body positions and skin tones. These findings indicate the method’s potential to enhance quantitative analysis in preclinical and clinical photoacoustic research.

Accurate assessment of tissue perfusion is essential for managing chronic foot ulcers in patients with diabetes and peripheral arterial disease. While photoacoustic (PA) imaging enables high-resolution visualization of vascular structures, current perfusion evaluation methods are limited. We propose a fully automated radiomics-based framework for predicting perfusion conditions using single-wavelength clinical PA foot imaging. Radiomics features were extracted from both raw radiofrequency (RF) signals and reconstructed maximum amplitude projection (MAP) images. After reproducibility testing and statistical filtering, features were ranked using a combined minimum redundancy maximum relevance (mRMR) and ReliefF approach. A k-nearest neighbors ensemble model trained on eight selected features achieved an area under the curve (AUC) of 0.90 (training) and 0.94 (test). The selected features corresponded with physiological indicators such as vessel density, tissue structure, and vascular discontinuity. This study demonstrates a reliable and interpretable method for perfusion assessment in PA imaging with strong clinical potential.
