The evaluation of kidney biopsies performed by expert pathologists is a crucial process for assessing if a kidney is eligible for transplantation. In this evaluation process, an important step consists of the quantification of global glomerulosclerosis, which is the ratio between sclerotic glomeruli and the overall number of glomeruli. Since there is a shortage of organs available for transplantation, a quick and accurate assessment of global glomerulosclerosis is essential for retaining the largest number of eligible kidneys. In the present paper, the authors introduce a Computer-Aided Diagnosis (CAD) system to assess global glomerulosclerosis. The proposed tool is based on Convolutional Neural Networks (CNNs). In particular, the authors considered approaches based on Semantic Segmentation networks, such as SegNet and DeepLab v3+. The dataset has been provided by the Department of Emergency and Organ Transplantations (DETO) of Bari University Hospital, and it is composed of 26 kidney biopsies coming from 19 donors. The dataset contains 2344 non-sclerotic glomeruli and 428 sclerotic glomeruli. The proposed model consents to achieve promising results in the task of automatically detecting and classifying glomeruli, thus easing the burden of pathologists. We get high performance both at pixel-level, achieving mean F-score higher than 0.81, and Weighted Intersection over Union (IoU) higher than 0.97 for both SegNet and Deeplab v3+ approaches, and at object detection level, achieving 0.924 as best F-score for non-sclerotic glomeruli and 0.730 as best F-score for sclerotic glomeruli.

The segmentation and classification of cell nuclei are pivotal steps in the pipelines for the analysis of bioimages. Deep learning (DL) approaches are leading the digital pathology field in the context of nuclei detection and classification. Nevertheless, the features that are exploited by DL models to make their predictions are difficult to interpret, hindering the deployment of such methods in clinical practice. On the other hand, pathomic features can be linked to an easier description of the characteristics exploited by the classifiers for making the final predictions. Thus, in this work, we developed an explainable computer-aided diagnosis (CAD) system that can be used to support pathologists in the evaluation of tumor cellularity in breast histopathological slides. In particular, we compared an end-to-end DL approach that exploits the Mask R-CNN instance segmentation architecture with a two steps pipeline, where the features are extracted while considering the morphological and textural characteristics of the cell nuclei. Classifiers that are based on support vector machines and artificial neural networks are trained on top of these features in order to discriminate between tumor and non-tumor nuclei. Afterwards, the SHAP (Shapley additive explanations) explainable artificial intelligence technique was employed to perform a feature importance analysis, which led to an understanding of the features processed by the machine learning models for making their decisions. An expert pathologist validated the employed feature set, corroborating the clinical usability of the model. Even though the models resulting from the two-stage pipeline are slightly less accurate than those of the end-to-end approach, the interpretability of their features is clearer and may help build trust for pathologists to adopt artificial intelligence-based CAD systems in their clinical workflow. To further show the validity of the proposed approach, it has been tested on an external validation dataset, which was collected from IRCCS Istituto Tumori “Giovanni Paolo II” and made publicly available to ease research concerning the quantification of tumor cellularity.

Robot-assisted radical prostatectomy (RARP) has become the most prevalent treatment for patients with organ-confined prostate cancer. Despite superior outcomes, suboptimal vesicourethral anastomosis (VUA) may lead to serious complications, including urinary leakage, prolonged catheterization, and extended hospitalization. A precise localization of both the surgical needle and the surrounding vesical and urethral tissues to coadapt is needed for fine-grained assessment of this task. Nonetheless, the identification of anatomical structures from endoscopic videos is difficult due to tissue distortions, changes in brightness, and instrument interferences. In this paper, we propose and compare two Deep Learning (DL) pipelines for the automatic segmentation of the mucosal layers and the suturing needle in real RARP videos by exploiting different architectures and training strategies. To train the models, we introduce a novel, annotated dataset collected from four VUA procedures. Experimental results show that the nnU-Net 2D model achieved the highest class-specific metrics, with a Dice Score of 0.663 for the mucosa class and 0.866 for the needle class, outperforming both transformer-based and baseline convolutional approaches on external validation video sequences. This work paves the way for computer-assisted tools that can objectively evaluate surgical performance during the critical phase of suturing tasks.

Transarterial chemoembolization (TACE) is a first-line treatment for intermediate-stage hepatocellular carcinoma (HCC) that can cause side effects. An accurate prediction of TACE response is important to improve clinical outcomes and avoid unnecessary toxicity. This study pursues a dual objective: to propose a standardized evaluation pipeline that enables reproducible benchmarking of state-of-the-art approaches on publicly available datasets, including both internal and external validation with public dataset, and to introduce a novel multimodal framework that integrates clinical variables, radiomic and deep features extracted from CT scans using the Vision Transformer MedViT to predict treatment response. Experiments were conducted using two publicly available datasets, the HCC-TACE-Seg, used as training and internal validation sets, and the WAW-TACE cohort, used as external validation set. The results demonstrated that the proposed method outperforms existing approaches. Independent validation on the external WAW-TACE dataset achieved promising results, confirming the robustness of the model and its potential to support treatment planning.
