Introduction: Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is the most common form of familial cerebral small vessel disease in adults and is caused by NOTCH3 variants. Clinical manifestations of CADASIL include recurrent ischemic strokes, dementia, migraine or migraineous headaches, epileptic seizures, and psychiatric disorders. The clinical-radiological phenotype of the disease is also highly variable. In this study, we investigated the variability of clinical, radiological, and genetic data in patients analyzed for NOTCH3 variant in our clinic. Methods: We performed clinical and neuropsychological examination, cerebral magnetic resonance imaging (MRI) and Doppler sonography of cerebral arteries in all patients. Next-generation sequencing test was used for detect variants in NOTCH3 gene from all CADASIL patients. Results: By using the next-generation sequencing method, heterozygous c.380C>T pathogenic variant was detected in the 4th exon of the NOTCH3 gene in 3 patients. This is a previously unreported novel variant and resulted in the replacement of the amino acid Proline at 127th position with Leucine. Discussion and Conclusion: The discovery of this novel pathogenic variant region may contribute to the expansion of the clinical and genetic spectrum of diseases associated with NOTCH3, leading to further research and treatment options for this disease in the future.

Background: This study aimed to assess the prevalence of neuropathic pain in patients diagnosed with lumbar disc herniation and to examine the correlation between neuropathic pain and surgical intervention. Material and methods: We performed a retrospective cohort study involving 217 patients diagnosed with lumbar disc herniation who reported persistent low back pain from January to December 2023. We assessed the existence of neuropathic pain using the Douleur Neuropathique 4 (DN4) questionnaire. We evaluated pain severity using the Visual Analogue Scale (VAS). We categorized the patients into three groups: non-operated, operated without stabilization, and operated with stabilization. Results: The patients had a mean age of 45.09 ± 14.28 years, with 57.6% being male. A DN4 score of > 4 indicated that neuropathic pain was present in more than half (51.2%) of the patients. The prevalence of neuropathic pain was 32.8% in the non-operated cohort, 67.3% in the operated cohort without stabilization, and 85% in the operated cohort with stabilization. Age exhibited a positive link with neuropathic pain score (r = 0.16, p

Objective: The present study aims to explore the relationship between primary headache types and bruxism and determine how these two conditions may affect each other. Material and Methods: A total of 97 cases who applied to the Neurology Polyclinic of our hospital during 01.07.2023-01.08.2023 were included in the study. A comprehensive questionnaire was applied to the participants. The diagnosis of headache was established based on the International Classification of Headache Disorders criteria; the American Academy of Sleep Medicine criteria were used for the diagnosis of sleep bruxism and the Research Diagnostic Criteria for Temporomandibular Disorders were employed for the diagnosis of awake bruxism. Results: Nineteen (19.6%) of the participants were male and 78 (80.4%) were female. Tension-type headache (TTH) was reported by 22 participants (22.7%), 30 participants had migraine-type headache (30.9%), and 45 participants (46.4%) comprised the healthy control group. Sleep bruxism was observed in 29 patients (29.9%), while 11 patients (11.3%) were diagnosed with awake bruxism. Higher rates of sleep bruxism and awake bruxism were seen in patients in the TTH group compared to those in the migraine group and the healthy control group (p
