Point-of-care ultrasonography (POCUS) is increasingly used to confirm endotracheal tube (ETT) placement; however, its overall diagnostic accuracy and speed compared with standard methods in adult emergency and critical care settings remain uncertain. We conducted a systematic review and meta-analysis of observational studies through May 2025 that evaluated POCUS for confirming ETT placement in adults (≥18 years) in emergency departments (EDs) and intensive care units (ICUs). Thirty studies met the inclusion criteria. Point-of-care ultrasonography achieved a pooled sensitivity of 0.91 and a specificity of 0.98. The area under the curve (AUC) was 0.96 [95% confidence interval (CI): 0.88–1.00]. The mean time-to-confirmation was 19.62 seconds for POCUS vs 22.46 seconds for standard methods; however, this finding should be interpreted with caution due to extreme heterogeneity (I² > 99%). Point-of-care ultrasonography is a highly accurate, rapid, and reliable method for confirming ETT placement in adult ED and ICU settings. It demonstrates diagnostic performance comparable to previously reported capnography performance and provides unique advantages as an adjunctive tool, especially in scenarios where capnography may be unreliable. Transcatheter aortic valve implantation (TAVI) has been growing rapidly. We aim to evaluate the performance and outcomes of the Myval transcatheter heart valve (THV) in patients with severe aortic stenosis and its use in quantitative videodensimetry, transcatheter valve-in-valve (ViV), and non-calcified aortic regurgitation (NCAR). A systematic search was done in PubMed, Scopus, Web of Science, Embase, and Cochrane from inception to October 2024. We used the relevant keywords to include studies that reported the outcomes of patients with severe aortic stenosis who underwent TAVI using the Myval THV and off-label usage in transcatheter ViV and NCAR. Data analysis was done using R software. A total of 29 studies were included in this study. The results of the mean aortic gradient at discharge, 30-day, one-year, and 2-year were 9.25 mmHg (95 % CI [8.20, 10.29]), 8.46 (95 % CI [7.57, 9.34]). 10.63 (95 % CI [9.12, 12.14]), and 7.2 (95 % CI [6.78, 7.63]), respectively. Additionally, the pooled percentages of patients with ≥ moderate aortic regurgitation were found in 1 % (95 % CI [1,2]) at discharge, 3 % (95 % CI [2,4]) at 30-day, 4 % (95 % CI [2,7]) at one-year follow-up and 5 % (95 % CI [3,8]) at 2-year. Furthermore, usage of the Myval THV in transcatheter ViV and NCAR led to a reduction in mean aortic gradient and incidence of aortic regurgitation, respectively. The Myval THV showed good safety and efficacy outcomes in short- and long-term follow-ups following the TAVI. Also, it showed promising results during transcatheter ViV and NCAR.

Purpose of Review: Despite advances in diabetes management, recurrent diabetic ketoacidosis (DKA) remains a common and largely preventable complication in paediatric patients with type 1 diabetes mellitus (T1DM). This systematic review and meta-analysis aimed to synthesise available evidence on risk factors associated with recurrent DKA in children and adolescents. Recent Findings: We systematically searched PubMed, Scopus, Web of Science, and the Cochrane Library from inception to October 2025. Observational studies involving children and adolescents (< 21 years) with established type 1 diabetes mellitus and biochemically confirmed DKA were included. Random-effects meta-analyses were performed for predefined predictors, reporting pooled odds ratios (ORs) with 95% confidence intervals (CI). Twelve observational studies comprising 58,911 paediatric patients were included. A prior episode of DKA was the strongest predictor of recurrence, conferring an almost eightfold increased risk (OR = 7.97, 95% CI 4.33–14.67). Longer diabetes duration of 12–15 years was also associated with increased recurrence risk (OR = 1.94, 95% CI 1.21–3.11), whereas duration of 5–11 years was not (OR = 1.13, 95% CI 0.76–1.69). Female sex was not significantly associated with recurrence in the primary analysis. Overall risk of bias was moderate across studies. Summary: Prior DKA episodes and longer disease duration are key predictors of recurrent DKA in paediatric T1DM. Targeted prevention strategies following an initial DKA episode are urgently needed, supported by future prospective research.
