Objective: This study aimed to evaluate the utility of preoperative spinopelvic analysis in reducing dislocation risk and to assess changes in spinopelvic mobility following total hip arthroplasty (THA). Methods: This cohort was assessed between 2018 and 2021, including 123 patients (123 hips) who underwent THA via the posterolateral approach. Group 1 (63 hips, 61 patients) underwent spinopelvic analysis with standing and sitting radiographs preoperatively and at 1- and 2-year follow-up, while Group 2 (62 hips, 62 patients) did not. Patients with Crowe type III-IV dysplasia, neurological or cognitive disorders, or acute trauma were excluded. Sacral slope (SS), spinopelvic tilt (sPT), and pelvic femoral angle were assessed. Intergroup comparisons were performed to evaluate the dislocation rates and changes in spinopelvic mobility. Results: One dislocation occurred in Group 1 and 3 in Group 2. In Group 1, significant changes were observed in sitting SS (preoperative to year 1: P < .001; year 2: P=.003) and sitting sPT (year 1: P=.004; year 2: P=.043), while standing measurements remained stable (SS: P=.762-.470; sPT: P=.683-.600). Mean sitting hip flexion also increased significantly (P < .001). Among 26 patients with hypermobility, 18 demonstrated normalization at 1 year, and 2 additional patients normalized at 2 years. In 1 patient with hypomobility, adjustment of acetabular anteversion based on spinopelvic findings successfully prevented dislocation. Conclusion: Preoperative spinopelvic analysis enabled the identification of mobility abnormalities and allowed for targeted surgical adjustments that may reduce dislocation risk. Notably, dislocations could potentially have been avoided with prior spinopelvic evaluation, underscoring its clinical relevance in preoperative planning. Postoperatively, SS increased exclusively in the sitting position, while remaining unchanged in standing position, reinforcing the necessity of seated imaging for comprehensive assessment. Background/Objectives: This study aimed to compare the histopathological effects of subacromial corticosteroid (CS) and platelet-rich plasma (PRP) injections following in situ repair of bursal-sided partial supraspinatus tears in a rat model. Methods: Twenty-seven Wistar Albino rats were included in the study. Partial bursal-sided supraspinatus tears were surgically created and repaired in situ. Animals were randomly assigned to receive a subacromial injection of 0.6 mL of normal saline (control), methylprednisolone sodium succinate (CS; 4.8 mg/kg, 0.6 mL), or 0.3 mL of activated PRP per rat. After four weeks, the repair sites were harvested for histopathological evaluation of cellular morphology, apoptosis, collagen organization, and vascularity. Results: PRP preserved the normal spindle-shaped morphology of tendon cells at the osseotendinous junction, whereas CS induced a rounded cellular morphology (p < 0.05). CS significantly increased apoptosis scores (p < 0.05), while PRP showed apoptosis scores comparable to those of the control group (p > 0.05). Collagen organization was preserved in the PRP group but significantly disrupted following CS administration (p < 0.05). Vascularity was higher in the PRP group than in the CS group (p < 0.05); however, the vascularity findings should be interpreted cautiously. Conclusions: Following in situ repair of bursal-sided partial rotator cuff tears, PRP preserved tendon architecture and was associated with a more favorable histopathological profile than corticosteroids. In contrast, CS adversely affected histopathological characteristics by increasing apoptosis and disrupting cellular morphology and collagen organization. These findings suggest that PRP may represent a biologically favorable adjunct following in situ repair of partial-thickness rotator cuff tears; however, the present histopathological findings do not establish accelerated healing, functional superiority, or clinical benefit, and further clinical studies are needed to determine whether these histopathological differences translate into improved clinical outcomes.

Objective: This study aimed to retrospectively compare the clinical, functional, and radiographic outcomes of revision total knee arthroplasty (rTKA) performed for septic and aseptic indications, and to evaluate postoperative changes within each group. Methods: A total of 27 patients (30 knees), comprising 18 females and 9 males, were included. Patients were categorized into two groups based on the indication for rTKA: the septic loosening group (13 patients, 13 knees) and the aseptic causes group (14 patients, 17 knees). Results: In the septic group, a comparison of preoperative and postoperative scores revealed significant improvements in the Oxford Knee Score (OKS), Patellar Scoring System, and Knee Society Score (KSS) for both knee and function components (P = .003, P = .004, P = .003, and P = .002, respectively). Similarly, the aseptic group demonstrated significant improvements in OKS, Patellar Scoring System, and KSS knee and function scores postoperatively (P < .001, P < .001, P < .001, and P = .001, respectively). Aseptic loosening occurred more frequently in female patients (P = .02). Postoperative flexion improved significantly in both the septic (P = .006) and aseptic (P = .001) groups. However, a significant extension gain was observed only in the aseptic group (P = .025; septic P = .102). Conclusions: In this retrospective cohort, both septic and aseptic rTKA groups demonstrated significant postoperative improvements in clinical and functional outcomes. Most evaluated postoperative outcomes did not differ significantly between the two groups. These findings describe the clinical and functional outcomes observed in this cohort without establishing superiority or equivalence between revision indications.
