Percutaneous endoscopic gastrotomy (PEG) enables long-term enteral feeding. The aim of this study was to identify biomarkers that may guide the decision of whether to perform the elective procedure of PEG. The medical records of all patients who underwent PEG in our hospital from 2010 to 2016 were screened retrospectively. Patients with mortality within a 30-day follow-up period and those without were compared using the Chi-square test, and continuous variables were compared with the Kruskal-Wallis and Mann-Whitney U tests. Receiver operating characteristic (ROC) curve analysis was used to demonstrate the ability of biomarkers to predict mortality; a cut-off point was determined and its sensitivity, specificity, and positive and negative predictive values were calculated. The Youden index was used to determine the cut-off point. Kaplan-Meier analysis was used to identify PEG-related mortality risk factors and a Cox regression model was applied for risk characterization. A total of 120 patients who underwent PEG were evaluated in the study. The mean age was 67.00 ± 18.00 years. The most common indication for PEG was cerebrovascular disease, in 69 (57.5%) of the patients. Infection of the PEG site was most common within 14 days after PEG tube placement, occurring in 13 patients (10.3%). The mortality rate among patients with post-PEG infection was 68.2%, significantly higher than in patients without infection (P = 0.012). Thirty-four patients (28.3%) died within 30 days of undergoing PEG. CRP values ≥ 78.31 mg/L increased mortality by 8.756-fold, and albumin levels < 2.71 g/dL increased mortality by 2.255-fold. Our results indicate that the presence of both high CRP level and low albumin level were associated with significantly higher rate of mortality (73.1%) in patients who underwent PEG.

Background/Objectives: Preoperative recognition of complicated acute appendicitis remains difficult, although severity influences treatment and perioperative planning. This study examined clinical and laboratory correlates of histopathologically complicated appendicitis and associated short-term outcomes and developed a preoperative prediction model. Methods: This single-center retrospective cohort included 500 adults undergoing appendectomy for suspected acute appendicitis (2021–2025). The primary outcome was gangrenous or perforated appendicitis on histopathology. Fifty preoperative candidate predictors were analyzed using least absolute shrinkage and selection operator (LASSO) logistic regression with nested cross-validation, calibration and predictor-selection reproducibility assessment, comparison with the Appendicitis Inflammatory Response (AIR) and Alvarado scores, and same-center temporal evaluation (2025 cohort). Results: Complicated histopathology was present in 111 patients (22.2%) and was associated with longer symptom duration, physiological disturbance, peritoneal signs, and systemic inflammation. The LASSO model achieved an out-of-fold area under the curve (AUC) of 0.861 (95% confidence interval [CI], 0.828–0.892; Brier score, 0.129; calibration intercept, −0.158; slope, 0.898), compared with 0.841 for AIR (difference, 0.020; 95% CI, −0.011 to 0.054; p = 0.221) and 0.743 for Alvarado (difference, 0.117; 95% CI, 0.067–0.169; p < 0.001). In the 2025 cohort, the AUC was 0.859 (95% CI, 0.783–0.926), but calibration indicated overprediction (intercept, −0.383; slope, 0.895). Thirty-day complications occurred in 15.3% and 8.0% of patients with complicated and non-complicated histopathology, respectively. Conclusions: Routinely available preoperative variables provided good internally validated discrimination, but the LASSO model did not significantly outperform the simpler AIR score. Overprediction in the later same-center cohort indicates that independent multicenter validation and recalibration are required before clinical use.

Objective: Radioguided parathyroidectomy using a handheld gamma probe is widely used to facilitate gland localization, yet its incremental value in routine primary hyperparathyroidism (pHPT) surgery especially in resource-limited settings—remains debated. We evaluated outcomes of parathyroidectomy performed with selective gamma probe use in a single center. Methods: We retrospectively reviewed 50 consecutive patients undergoing parathyroid surgery in 2025. Preoperative localization relied on cervical ultrasound with or without sestamibi scintigraphy. Gamma probe guidance was used selectively at surgeon discretion. We compared operative and early biochemical outcomes between patients with and without gamma probe use. Continuous variables were compared with Welch’s t-test and categorical outcomes with Fisher’s exact test. Results: Gamma probe guidance was used in 14/50 patients (28%). Mean age was 54.6±6.9 years and 82% were female. Mean operative time was similar between groups (46.1±2.9 vs 44.4±6.7 min; p=0.239). Transient hypocalcemia occurred in 8 patients (16%), all in the non–gamma probe group (0% vs 22.2%; p=0.087), and no permanent hypocalcemia occurred. Early biochemical response, assessed by percent PTH reduction from baseline to postoperative measurement, was lower in the gamma probe group (mean 85.2% vs 90.5%; p=0.006).Conclusion: In this real-world single-center cohort, selective gamma probe use was not associated with shorter operative time and was likely used in more complex cases, possibly reflecting case complexity rather than an effect of radioguidance itself. Prospective studies are warranted to clarify which patient subgroups derive measurable benefit from radioguidance.
