Background. A significant increase in the prevalence of malnourishment, obesity, and sarcopenic obesity has been observed in developed countries over the last few decades. In Spain, this especially happens in populations over 65 who are not institutionalized. Differences in lifestyle, medication, and economic capacity partially explain this increase. Objective. To study the nutritional status of a population of 65 year-olds and subjects who are not institutionalized, in the Cádiz region (Spain). Methods. Observational, transversal study carried out on 2621 subjects who are 65 years old and over, with a direct weight and height measurement, in 150 pharmacy offices from 44 locations. A mobile application was designed for homogeneous data collection in all the pharmacy offices. The data required from all subjects was gender, age, postal code, social security contribution regime, if the patient lives alone, type of food consumed as the main meals, level of physical activity, polypharmacy, weight, and height. Results. The prevalence of overweight and obesity amounts to 82.2% of the population (43.2% overweight and 39% obese). We found an inverse relationship between the prevalence of overweight and obesity with carrying out physical activity and having full dinners. Conclusion. We identify the need to reinforce the messages to the elderly aimed at maintaining adequate physical activity and assessing the quality and quantity of dinners, as well as reducing, as much as possible, the treatments that may lead to weight gain.

Objetivo: Estudiar la variabilidad del manejo de la fibrilación auricular (FA) por equipos de emergencias y su adherencia a las guías de tratamiento. Material y método. Estudio descriptivo de episodios de FA atendidos en dos años en Cádiz. Recogemos 236 pacientes. Analizamos antecedentes, actuación del equipo y resultados de control de frecuencia y ritmo. Construimos un índice sintético de adecuación terapéutica (ISAT) con rango de -3 a 3. Resultados. 236 pacientes, 68% varones, las mujeres más jóvenes (66 vs 75 años); 43.8% fueron primeros episodios. El fármaco más usado es la amiodarona. El 16,1% de las asistencias presentaron adherencia completa (ISAT 3) y el 63,1%, adecuación parcial positiva (ISAT 1-2); la mayor variabilidad clínica se produjo en la bradicardización. La adecuación fue: 62,3% para anticoagulación, 68.2% para bradicardización y 56.8% para cardioversión. Existió un 40,7% de control de frecuencia y un 96% de derivación hospitalaria. Conclusión. Debemos aumentar el porcentaje de anticoagulación y empleo de bloqueadores-beta y calcioantagonistas en sustitución de amiodarona. El ISAT permite mejorar la adecuación a las GPC que facilita controlar síntomas, frecuencia y reducir estancias en los servicios de urgencia.

This paper describes the protocol for a study designed to address the high prevalence (40%) of childhood overweight and obesity in the province of Cádiz, Spain, as a reflection of what is happening worldwide. It is widely known that children who suffer from childhood obesity have a higher risk of developing chronic diseases in adulthood. This causes a decrease in the quality of life and an increase in health spending. In this context, it is necessary to intervene promoting healthy lifestyle habits from an early stage. The objective of this project will be to evaluate the effectiveness of a multimodal intervention (individual, school and family) called “PREVIENE-CÁDIZ” [CADIZ-PREVENT]. The intervention will be focused mainly on diet, physical activity, sedentary lifestyle and sleep, to prevent overweight and obesity in schoolchildren from 8 to 9 years old in the province of Cádiz. It will consist of a 10-session education program carried out in the classroom by the teachers. In addition, children will be assigned two workbooks, one to work on in class and the other at home with parents. A workshop aimed at parents will be included to help teach them how to obtain healthier lifestyle habits. The proposed study will involve a quasi-experimental design with a control group.

Objective: Cardiorespiratory fitness (CRF) and a healthy diet may be part of an overall healthy lifestyle. The association between cardiorespiratory fitness and adherence to an overall Mediterranean Diet (MedD) pattern and specific MedD foods has been assessed. Design: Subjects completed a lifestyle survey and dietary pattern, using the validated MedD Adherence 14-item questionnaire and two self-reported 24-h dietary recalls. Participants’ height, body weight, waist circumference (WC), and CRF (maximum oxygen uptake, VO2max, ml/kg/min) were measured. Setting: University of Cádiz, Spain. Subjects: A sample of young adults (n = 275, 22.2 ± 6.3 years). Results: Mean VO2max was 43.9 mL/kg/min (SD 8.5 mL/kg/min). Most participants had healthy CRF (75.9%). The average MedD score was 6.2 points (SD 1.8 points). Participants who consumed more servings of nuts had higher VO2max. Those who showed low CRF performed less physical activity (PA) and had a higher body mass index (BMI) and WC compared with those classified as having healthy CRF. Nut consumption was positively associated with VO2max (β = 0.320; 95% CI 2.4, 10.7; p < 0.002), adjusting for sex, age, smoking PA, BMI, WC, and energy intake, showing the subjects who consumed more nuts were fitter than young adults who consumed less. Conclusions: CRF is positively associated with nut consumption but not with the overall MedD pattern and all other MedD foods in the young adults. The subjects who consumed more servings of nuts were fitter than young adults who consumed less. Moreover, fitter subjects performed more PA and had a lower BMI and WC than those who had lower fitness levels.

La hipertensión arterial (HTA) es el principal factor de riesgo cardiovascular modificable. La HTA se puede relacionar con el consumo elevado de sal. Para medir la ingesta no todas las encuestas de alimentación son comparables y válidas. El procedimiento de referencia para valorar la ingesta de sal consiste en medir la excreción urinaria de sodio en orina recolectada durante 24 h, aunque se han propuesto métodos alternativos, como las recolecciones de muestras de orina puntuales y cronometradas. En esta revisión analizamos qué instrumentos permiten valorar la ingesta de sal y cuáles de ellos han aportado una mayor validez y fiabilidad a través de los estudios de concordancia con la eliminación de sodio en orina. Las encuestas actuales de consumo de alimentos son inadecuados debido a su amplia variabilidad y relativamente baja correlación con la eliminación de sodio en orina de 24 h. Su principal limitación es la necesidad de validación en diferentes grupos poblacionales. En Atención Primaria se debería valorar la ingesta de sal mediante la utilización de cuestionarios de frecuencia de consumo que recojan alimentos con elevado contenido en sal, el consumo de platos preelaborados y preguntas que cuantifiquen la adición de sal en la preparación de alimentos o en la mesa. Para la validación de estos cuestionarios debe emplearse como gold standard la eliminación de sodio en orina de 24 h ajustada según el aclaramiento de creatinina. High blood pressure (HBP) is the main modifiable cardiovascular risk factor. HBP can be related to high salt intake. To measure intake, not all feeding surveys are comparable and valid. The reference procedure for assessing salt intake consists of measuring the urinary excretion of sodium in urine collected during 24 hours, although alternative methods have been proposed, such as the collection of punctual and timed urine samples. In this review, we analyze which instruments allow the assessment of salt intake and which of them have provided greater validity and reliability through studies of concordance with the elimination of sodium in urine. Current food consumption surveys are inadequate because of their wide variability and relatively low correlation with the elimination of sodium in 24-hour urine. Its main limitation is the need for validation in different population groups. In primary care, salt intake should be assessed by using frequency-of-consumption questionnaires that collect foods with a high salt content, the consumption of preprepared dishes and questions that quantify the addition of salt in the preparation of food or at the table. For the validation of these questionnaires, the standard gold elimination of 24-hour urine sodium adjusted according to creatinine clearance should be used.

ObjectiveTo determine the effects of lipid-based nutrient supplements (LNS) on children’s Hb, linear growth and development, compared with supplementation with micronutrient powder (MNP).DesignThe study was a two-arm parallel-group randomized controlled trial, where participants received either LNS or MNP for daily consumption during 6 months. Supplements were delivered by staff at government-run health centres. Hb, anthropometric, motor development, language development and problem-solving indicators were measured by trained research assistants when children were 12 months of age.SettingThe study was conducted in five rural districts in the Province of Ambo in the Department of Huánuco, Peru.SubjectsWe enrolled 6-month-old children (n 422) at nineteen health centres.ResultsChildren who received LNS had a higher mean Hb concentration and lower odds of anaemia than those who received MNP. No significant differences in height-for-age, weight-for-height or weight-for-age Z-score, or stunting and underweight prevalence, were observed. Provision of LNS was associated with a higher pre-verbal language (gestures) score, but such effect lost significance after adjustment for covariates. Children in the LNS group had higher problem-solving task scores and increased odds of achieving this cognitive task than children in the MNP group. No significant differences were observed on receptive language or gross motor development.ConclusionsLNS between 6 and 12 months of age increased Hb concentration, reduced anaemia and improved cognitive development in children, but showed no effects on anthropometric indicators, motor or language development.

Introducción: la declaración de voluntades anticipadas (DVA) es un documento que contiene las preferencias sobre los cuidados y tratamientos sanitarios que se desea recibir cuando no se tenga capacidad para expresarlos.Objetivo: estudiar el contenido de las DVA inscritas en la sede del Registro de Voluntades Anticipadas del Hospital Universitario Puerta del Mar, respecto al soporte nutricional especializado (SNE) e hidratación en el periodo de noviembre 2013 a diciembre 2015.Método: estudio transversal realizado sobre el contenido de las DVA inscritas en el modelo del Decreto 59/2012. Se estudian 9 variables de relevancia relacionadas con el SNE e hidratación en las DVA.Resultados: se analizan 414 DVA de las que el 60% corresponden a mujeres. La edad media de los declarantes es 55,2 años. El 41,7% inscribieron su DVA por automotivación. El 84,78% no desea recibir nutrición parenteral, el 91,06% rechaza la alimentación mediante tubo de gastrostomía y el 89,37% no desea recibir la sonda nasogástrica. La hidratación es rechazada por el 71,26%. Las tres posibles opciones de SNE dependen significativamente de la edad del declarante (p < 0,001) pero no así la hidratación. Ambos, SNE e hidratación, sí se asociaron significativamente con los motivos para inscribir las voluntades anticipadas (p < 0,001).Conclusiones: la edad es el principal factor que condiciona la opción de aceptación o rechazo de SNE. Por el contrario, la elección de la hidratación no está condicionada por la edad. Los motivos por los que se inscriben los otorgantes son otro determinante en la elección de si desean recibir o no SNE e hidratación.

Background and Objectives Radiofrequency ablation of genicular nerves has proved to be successful in relieving pain and incapacity caused by osteoarthritis of the knee. However, long-term efficacy of such a treatment remains to be assessed. The current study aimed to reproduce radiofrequency neurotomy of genicular nerves to manage gonarthrosis pain and disability and establish therapeutic response until 1 year after intervention. Methods This single-center, prospective, observational, noncontrolled, longitudinal study included patients with grade 3 to 4 gonarthrosis suffering from intractable knee pain, scoring 5 or more on the visual analog scale (VAS) during >6 months. Therapy was based on ultrasound guided radiofrequency neurotomy of the superior medial, superior lateral and inferior medial genicular nerves. Visual analog scale and Western Ontario and McMaster Universities Osteoarthritis scores were assessed before therapy and at 1, 6, and 12 months following treatment. Results Radiofrequency neurotomy of genicular nerves significantly reduced perceived pain (VAS) and disability (Western Ontario and McMaster Universities Osteoarthritis) in the majority of participants, without untoward events. The proportion of participants with improvement of 50% or greater in pretreatment VAS scores at 1, 6, and 12 months following intervention were 22/25 (88%), 16/25 (64%) and 8/25 (32%), respectively. Conclusions Ultrasound-guided radiofrequency neurotomy of genicular nerves alleviates intractable pain and disability in the majority of patients with advanced osteoarthritis of the knee. Such a treatment is safe and minimally invasive and can be performed in an outpatient setting. The beneficial effect of treatment started to decline after 6 months, but even 1 year after the intervention, 32% of patients reported 50% improvement or greater in pretreatment VAS scores.

To study the relationship between obesity prevalence and education, employment and socioeconomic levels to improve the designing of intervention programs. Cross-sectional population-based study on 2640 subjects over 15 years of age in Spain. Through interviews conducted in subjects' homes we have obtained socio-demographic, anthropometric data, and information on physical activity, life styles and eating habits. Statistically, we have used prevalence ratios with confidence intervals and logistic regressions. The prevalence of overweight and obesity is 36.4% (IC95% 34.5-38.2) and 17% (IC95% 15.7-18.5), respectively, higher in men, and increasing with age. Obesity prevalence is independent of education level. Higher prevalence of overweight and obesity are found in the lowest occupational categories, and there is an inverse relationship between BMI and employment situations. Obesity is more prevalent in retired people and people that work from home, compared with professions that require activity at work. The association found between obesity and socioeconomic characteristics of the population is different in men and women: in women only, this risk was found to be associated with low or primary education levels (OR 2.4, 1.5-4.0), being unemployed or working at home (OR 1.6, 1.08-2.4) and having a medium income (OR 1.87, 1.03-3.33). We have confirmed a high prevalence of overweight and obesity in our area. Socioeconomic variables have a more predictive value in women than in men.

Conocer las complicaciones cronicas que afectan a una poblacion de diabeticos tipo 2 atendidos en un centro de salud y analizar la relacion que existe entre estas complicaciones y factores de riesgo. Transversal. Centro de Salud Puerto de Santa Maria-Norte (Cadiz). Diabeticos tipo 2 (n = 504) incluidos en el programa de diabetes del centro, mayores de 12 anos, de ambos sexos. La prevalencia registrada de diabetes tipo 2 fue del 2,3%. La media de edad de la poblacion era de 63,9 anos (DE, 10,6), siendo un 57,9% mujeres; con un tiempo de evolucion de la enfermedad de 8,6 anos (DE, 11,4). Recibian insulinoterapia el 17,1% de los pacientes, antidiabeticos orales un 65,4%, y dieta el 17,5%. Los factores de riesgo mas importantes fueron: antecedentes familiares de diabetes (54,6%), obesidad (51,2%), sedentarismo (41,1%) e hipertension (47,2%). Las complicaciones cronicas afectaban al 67,8% de los diabeticos del centro; predominando la vasculopatia periferica (31,7%), retinopatia (30,6%), cardiopatia isquemica (21,2%), ACV (10,7%), neuropatia (8,9%) y nefropatia (6,2%). Los factores de riesgo predictores de complicaciones macrovasculares fueron tiempo de evolucion, edad de diagnostico, consumo de tabaco e hipertrigliceridemia. Los factores predictores de complicaciones microvasculares fueron HbA1c≥7%, tiempo de evolucion y edad. Encontramos una importante prevalencia de complicaciones cronicas a pesar de que el tiempo medio de evolucion de la enfermedad en la poblacion no fue elevado. Sin embargo, los factores que mostraron mayor asociacion con la existencia de complicaciones cronicas fueron en su mayoria susceptibles de modificacion mediante un mejor control metabolico de los pacientes. To know the chronic complications affecting a type-2 diabetic population in a health center, and to analyze the relationship between these complications and risk factors. Transversal study. Puerto de Santa Maria-Norte Health Center, Cadiz, Spain. Type-2 diabetic population (n = 504) included in the diabetes program of the center, aged over 12 years old, of both sexes. The prevalence of type 2 diabetes recorded was 2.3%. Average age of the population was 63.9 years (SD 10.6), women representing 57.9% of total. Average length of evolution of the disease was 8.6 years (SD 11.4). 17.1% of patients were receiving insulin therapy, 65.4% were receiving oral antidiabetics, and 17.5% were on special diet. The most important risk factors found were: family history of diabetes (54.6%), obesity (51.2%), sedentary life style (41.1%), and hypertension (47.2%). Chronic complications affected 67.8% of the diabetic patients attending the center; predominant complications were peripheral vascular disease (31.7%), retinopathy (30.6%), ischemic heart disease (21.2%), cerebrovascular accident (10.7%), neuropathy (8.9%) and nephropathy (6.2%). The risk factors most associated with macrovascular complications were :length of evolution, smoking and age of diagnostic and hypertriglyceridemia. For the microvascular complications were: HbA1c, length of evolution and age. We found a significative prevalence of complications in spite of the average length of disease evolution time in the population not being very high. However the risk factors strongest associated with the existence of chronic complications are susceptible to be modified with a better metabolic control of the patients.

In recent years, eating disorders (Anorexia and Bulimia Nervosa) have increased and are appearing at increasingly younger ages. They affect predominantly adolescent females 12 to 25 years of age. The objective of this study of adolescents is to detect and discuss unhealthy eating behaviour, defined by either of two factors: (1) following a slimming diet not advised or supervised by any person trained in health care; or (2) eating very large quantities at irregular times, not related to anxiety or stress. A transversal study has been undertaken of 630 school children of 14-18 years of age (average: 15.9 years) in Cádiz (Andalucia, Spain), using an anonymous self-reporting questionnaire to collect data on personal and educational situation, on eating habits, on nutritive intake and knowledge of nutrition, and on dieting and physical exercise. The study has considered averages, ratios, statistical significance (chi2) and, as a measure of risk, the Disequality Ratio of Prevalence (DRP). Anomalous eating behaviour was detected in 46.3% (292), with females predominant by a ratio of 2:1. Comparing groups with anomalous and with normal eating habits, significant differences were detected in respect of: perception of body image (p < 0.001), frequency of weighing oneself (p < 0.05), periods of abstinence from eating (DRP 1.66; 95% confidence interval (CI): 1.66-2.37), provocation of vomiting (DRP 2.02; 95% CI: 1.13-3.65), use of laxatives (DRP 4.25: 95% CI: 1.08-9.63), and the exclusion of certain meals and types of food, mainly bread and cereals, fats and sugars. Conclusions are drawn on the substantial scale of unhealthy eating behaviour among adolescents in Cadiz. More adequate education on personal health and related social issues should be provided.
